Child’s Health Procedure Please follow this procedure in the event that your child becomes ill. Kindly keep your child home if they display the following: • Fever • Unexplained Rash • Vomiting In addition to checking your child’s temperature before entering the program, the following questions will be addressed daily: • Have you had any known contact with a person confirmed or suspected to have COVID- 19 in the past 14 days? • Are you experiencing any of the following symptoms: cough, shortness of breath, trouble breathing, fever, chills, muscle pain, headache, sore throat, loss of taste, loss of smell? • Have you tested positive for COVID-19 in the past 14 days? • Have you traveled to NY from one of the posted states on the NY travel advisory list? If the answer is NO to all questions, the child may enter the program. If the answer is YES to any question, the child may not attend the program.
Students will use UPK Supplies their own materials Please have your child bring the following : each day. • backpack- no wheels • two pocket folder • 1 composition notebook • Shoebox or plastic container with the following • 24 pack of crayons • Safety scissors • 2 glue sticks Please label ALL • A bottle of Elmer’s glue items! • 2 pencils • 1 pack of primary thick markers
Supplies continued • Smock/oversized t-shirt • Extra set of clothes for your child in case of accidents (including underclothes, socks and sneakers) • Extra face covering • (reusable/washable) Please label ALL materials! Thank you!
Important Forms • SCOPE Emergency Information Authorization Sheet • For the safety of your child, this form is needed for dismissal. • If your child is being picked up by an alternate pick-up, the name of the adult must be written on this contact form, texted to your child’s teacher’s SCOPE phone or a note needs to be written including this individual’s name before dismissal. • Adults picking up children need a photo ID. • *Adults are considered 18 years or older.* • Health Screening One-Time Attestation • This form is kept on file and attests that you have completed the health screening questionnaire each day. • SCOPE Education Services Media Release Form • Permission to SCOPE Education Services to use child’s name, portrait, photograph or likeness in any media coverage or any advertising material Please be sure to SIGN each form!
Forms continued • Snack Approval Form for Students with Special Dietary Needs • This form is for students with ALLERGIES • Please review the snack list • Students with allergies that may have medicine or an EpiPen need to express this to their child’s teacher, call the school nurse and make an appointment to follow proper protocol and paperwork. ****If your child has been injured, we require a doctor’s note indicating any limitations or restrictions. If there are no limitations or restrictions, this needs to be indicated as well. This note is necessary for their return to school. Please remember to call us in the morning to let us know if your child will be out for the day.****
Snack & Beverage A nut-free snack and beverage will be served daily and provided by the program. No outside food is permitted. *Remember to speak with your child’s teacher regarding dietary restrictions or food allergies before the start of school.*
Classroom Celebrations and Visitors Due to the current situation, classroom celebrations and visitors to our program will not be permitted as well as holiday goody bags of any kind. Your adherence to these guidelines is appreciated.
Events & Important Classroom Information We follow the district calendar with regards to holidays and parent teacher conferences. We will update you monthly with a newsletter to keep you informed of any changes or upcoming events in our classroom. The newsletter will also provide information about the monthly themes introduced in our classroom.
Learning Children will: • learn Fundations for letter recognition, sounds and handwriting • practice name recognition and writing (first and last names) • complete activities focused on themes from the NYS Domains for Pre-Kindergarten (All About Me, Families and Communities, Animals, Plants and Habitats) • construct STEM and STEAM activities with literature • practice number recognition, counting, patterns, 1 to 1 correspondence and writing numbers • listen to monthly author studies • create art projects • take part in movement activities • learn social-emotional strategies
