Introduction The Well Animal Institute dba K-9 Smiles, (hereinafter referred to as the School) offers the finest technology and equipment for student training and is staffed with qualified, approved instructors. The School is owned by Dr. Anna (Turie) Norman and Cindy Lloyd. The officers of the school are Dr. Anna (Turie) Norman, President; Lucinda (Cindy) Lloyd, Vice President. Faculty Members Dr. Anna (Turie) Norman Lucinda (Cindy) Lloyd Programs/Courses Offered Certificate Programs: Program Course Anesthesia-free Canine Dental Practitioner Occupational Objective: The graduate should be able to perform non-anesthetic dental cleanings for dogs Program/Course Costs 2020 Tuition Total Tuition/Fees Program: $3,500.00 See Educational Services on page 5, for more details on Programs/Courses offered. You will be charged a fee for foreign checks depending on current rate. A 3% convenience fee will be charged to your card when the tuition fees are paid by credit card. Entrance Requirements The school does not discriminate based on race, sex, religion, ethnic origin, or disability. It is up to each individual to check the laws in their State, Providence or Country regarding Anesthesia-Free teeth cleaning Prospective students must have a high school diploma or equivalency diploma to be accepted for enrollment. You must be at least 18 years old to take this class. It should be understood that this is a physical job. The applicant should be in good health and physical condition and able to sit for long periods of time on the floor. You will have to complete up to 20 dogs to pass the class. Enrollment Once you fill out the application and other forms and return them signed to our office you will be given a code to enter the school portal. Postponement of Start Date Not Applicable Placement Assistance The Well Animal Institute dba K-9 Smiles does not offer employment assistance to graduates. We will provide information on marketing ideas for your new business. We make no guarantee, expressed or implied, of future employment. Current law prohibits any school from guaranteeing job placement as an inducement to enroll students. You will be required to check the laws in your state on whether you need to do this under veterinary supervision. Attendance Requirements The course will keep track of your hours each time you log in. Students who are unable to continue classes for medical reasons or severe personal problems will be required to withdraw in writing from class but will be allowed to re-start the class if it is done within 90 days of withdrawing. Progress Policy You must pass each section in order to move on to the next. Grading System This class is Pass or Fail on the following subjects: Anatomy, Physiology, Tooth Disease, and Hand scaling, oils, Bach flowers, starting a business. 4
The hands on portion of the course is critical to passing the course and receiving your certificate of completion. Students will need to supply their own dogs to work on during this course. Conduct Policy All students are expected to act maturely and are required to respect other students and faculty members. Possession of weapons, illegal drugs, and alcohol of any kind are not allowed at any time on school property. Any violation of school policies may result in permanent dismissal from school. Dismissal Any student may be dismissed for violations of rules and regulations of the school, as set forth in school publications. Facilities Online only Educational Services Program : Anesthesia-free canine dental practitioner Subject: Anesthesia-free teeth cleaning for dogs Occupational Objective To learn anesthesia-free canine dental techniques. Length of Program: You will have 90 days in which to finish this program, then you may submit a tape of yourself cleaning a dog’s mouth and show before and after pictures and we will send you a certification of completion for the class. Type of Instruction Online only Previous Credits The school does not accept credit from previous training or experience. The school does not guarantee transferability of its credits to another institution unless there is a written agreement with another institution. 5
Student Grievance Procedure Any complaint should be in writing to the school’s Director: Cindy Lloyd 80 Garden Center Bldg C #18 Broomfield, CO 80020 Student Complaints Student Complaints should be brought to the attention of the School Director to attempt resolution. The Director and student are to follow the grievance procedures according to school policy printed in the school catalog. A student may file a written complaint online with the Colorado Division of Private Occupational Schools at: www.highered.colorado.gov/dpos or (303) 862-3001. All student complaints submitted to the Division must be in writing and shall be filed within two years after the student discontinues training at the school. Refund Policy Students not accepted to the school are entitled to all monies paid. Students who cancel this contract by notifying the school in writing within three (3) business days are entitled to a full refund of all tuition and fees paid. Students, who withdraw after three (3) business days, but before commencement of classes, are entitled to a full refund of all tuition and fees paid except the maximum cancellation charge of $150.00 or 25% of the contract price, whichever is less. In the case of students withdrawing with a written notice after commencement of classes, the school will retain a cancellation charge ($150.00) plus a percentage of tuition and fees, which is as described in the table below. The refund is based on the last date of recorded attendance. (Each day is counted as one entire day of class; Partial days up to noon are ½ days. After 12:00 noon it’s considered a full day) Refund Table Student is entitled to upon withdrawal/termination Refund Within first 10% of program (First ½ day of DAY ONE) 90% less cancellation charge After 10% but within first 25% of program (Second ½ of DAY ONE & DAY TWO) 75% less cancellation charge After 25% but within first 50% of program (DAY THREE First or Second 1/2) 50% less cancellation charge After 50% but within first 75% of program (DAY FOUR First or Second 1/2) 25% less cancellation charge After 75% [if paid in full, cancellation charge is not applicable] (DAY FIVE) NO Refund 1. The student may cancel this contract at any time prior to midnight of the third business day after signing this contract. 2. All refunds will be made within 30 days from the date of termination. The official date of termination or withdrawal of a student shall be determined in the following manner: a. The date on which the school receives written notice of the student’s intention to discontinue the training program; or b. The date on which the student violates published school policy, which provides for termination. c. Should a student fail to return from an excused leave of absence, the effective date of termination for a student on an extended leave of 6
absence or a leave of absence d. Is the earlier of the date the school determines the student is not returning or the day following the expected return date. 3. The student will receive a full refund of tuition and fees paid if the school discontinues a program/Stand Alone course within a period of time a student could have reasonably completed it, except that this provision shall not apply in the event the school ceases operation. 4. The policy for granting credit for previous training shall not impact the refund policy. 7
