1. Registration: Registration begins Wednesday, April 23, 2014 at your home school. To find out which attendance area you live in, please refer to the boundary map on page 6. Oak Harbor School District Board Policy, in accordance with Washington State law, requires a child to be five years of age by midnight, August 31, in order to begin kindergarten in the new school year. Immunizations: Students must have a completed and valid Certificate of Immunization Status form on file within 30 days of starting school. However, it is strongly encouraged to be provided before the first day of school. The form is located in the Registration Handbook. 3. Address and Phone Number Changes: Please notify your child’s school as soon as possible if you are moving out of the district or have moved to a different address. Phone number changes should also be reported, as well as new email addresses, to ensure that the school is able to contact you in the event of an emergency. 4. Emergency Contacts/Daycare Information: Emergency contacts are called in the event you cannot be reached by phone or email. These contacts are also allowed to pick up your child from school. If you are new to the area, feel free to stop by the school office once you’ve met an adult who could serve in this role. Please update the school with any additions or deletions on your emergency contact list and to identify other adults who may pick up your child from school. Daycare information, i.e. provider name, phone number, address, and all providers authorized to pick up your child, should also be given to the school and updated as needed. 5. School Lunch/Breakfast: Schools offer breakfast and lunch each day. Free and reduced price meals are available to those who qualify. 6. Transportation: Families who live on their home school’s bus route and would like bus transportation to/from school should complete and submit the transportation request form, located in the Registration Handbook, to the school. The transportation department may take up to three business days to make arrangements. 7. Absences: If your child is ill or going to miss school for any reason on a given day, notify the school by phoning the school attendance line by 8:30 a.m. on the day of the absence. Send a note to the school office upon your child’s return to school. Please check in with the school’s attendance secretary if you are planning an extended absence for your child. Pre-approval may be needed from the principal and teachers need advanced notice of requests for assignments in such cases. 8. Class Lists and the First Two Days of School Schedule: Class lists will be posted at the school for viewing on Saturday, August 30, 2014. The first day of school is Thursday, September 4, 2014. Parents and students attending morning (AM) or afternoon (PM) programs will be invited by the school to attend an assigned small group session on the first two days of school. Olympic View Elementary kindergarten families will be scheduled for parent-teacher conferences on the first two days of school. Oak Harbor Elementary All-Day programs will begin classes on the first day of school. KINDERGARTEN REGISTRATION CHECKLIST Help us serve you better - Please be sure to bring the following to registration: Copy of birth certificate or passport showing student’s legal name and date of birth for students entering kindergarten or first grade. Only legal names can be used in our district’s student record system. Address verification document, such as: utility or cable bills, lease, mortgage information, insurance, documents from public agencies such as the Court or DSHS. Completed and signed Certificate of Immunization Status (CIS) form [This form is included in the Registration Handbook] Important – We are not able to accept a vaccination record from your child’s health care provider. Washington State law requires submission of your child’s immunization information on its official (CIS) form, which is to be signed by the parent/guardian (not the doctor). Please bring records including dates and immunizations received if you need assistance with completing the form. ALL-DAY TUITION-BASED KINDERGARTEN All-Day Kindergarten is offered at Oak Harbor Elementary, which requires students to wear uniforms. Class sizes are lower and classes have extra instructional help. Lunch and breakfast are available every day (free for qualifying students). Before- and after-school daycare is also available on site through a third-party vendor. The program is available five days a week for a fee of $260 per month. Parents may use the application form on page 5 of this handbook and submit it to the Administrative Service Center, located at 350 S. Oak Harbor Street. Applications, with a deposit check of $260, are accepted between May 15 and June 5, 2014, 8:00 a.m. to 4:30 p.m. Research shows All-Day K kids get a head start in reading, math, and general learning skills, develop better social skills, improved behavior, and more involvement in classroom activities. For more information, please contact your home school. FULL-DAY STATE-FUNDED KINDERGARTEN Free All-Day Kindergarten is available to the students residing in the Olympic View Elementary attendance area. FULL-DAY DEVELOPMENTAL AND TITLE I KINDERGARTEN Oak Harbor Elementary hosts full-day Developmental and Title I Kindergarten programs. These programs are for qualified students only. Please call the Special Education Programs department at 360.279.5073 for more information. 4
2014-2015 APPLICATION FOR ALL-DAY KINDERGARTEN The requests for All-Day Kindergarten are accepted between May 15, 2014 and June 5, 2014. Applications: Must be turned into the District Office at 350 S. Oak Harbor Street between 8 a.m. and 4:30 p.m. beginning May 15 and ending June 5. Applications are logged in on a first-come, first- served basis. All-Day Kindergarten classes must have a minimum number of students. Classes will be created depending on the number of requests. Applications submitted after the cut-off will be considered on a space-available basis after the initial placements are made. Tuition: All-Day Kindergarten is $2,340.00 which is to be paid in 9 monthly payments of $260.00. To request a place in All-Day Kindergarten for your student you must submit a deposit with this application in the form of a check for $260.00. You will receive a confirmation letter and receipt of your deposit. After all applications are received, classes will be established and the students placed. Your deposit will be applied to the May 2015 tuition payment. The first payment is due by September 5, 2014. Notification: Confirmed space in a classroom will be sent out by July 15, 2014. If your student cannot be placed you will receive a letter and a refund. If you wish to remain on a waitlist you can request to do so at that time. Transportation: Bus service is only provided for this program if you live in the attendance area of the school where the class is offered (currently Oak Harbor Elementary). Oak Harbor Elementary requires students to wear uniforms.
