2018 MIDWEST PRESENTED BY WHEELCHAIR TENNIS CHAMPIONSHIPS Tennis Tournament at MVP Athletic Club - Crahen JULY 12–15 ITF 3/USTA Category II Thursday, July 12 – 5 p.m. – Registration, Player Reception, Up/Down Social with Prizes Friday, July 13 – 8:30 a.m. – Tournament Play Begins
We look forward to welcoming you to the 2018 Midwest Wheelchair Tennis Championships held at MVP Athletic Club- Crahen in Grand Rapids, Michigan. Our goal is to provide a professionally run tournament with hospitality that ensures this a must-play annual event. To that end, Scott Elbin will act as the ITF Supervisor/Tournament Referee, and we have added new events and services. Be sure to arrive in Grand Rapids early enough to participate in Thursday evening’s Up/Down Social and a night of friends, food, fun and prizes. “At The Hub” services will be provided at the tournament site to help you with any needs that may arise. Also provided are massage therapy services with Kinesio taping, a repair station sponsored by Airway Oxygen and outstanding volunteers to ensure a successful event. See you on the courts, 2018 Midwest Wheelchair Tennis Championships Committee TOURNAMENT DETAILS AND EX TR AS • Prize Money: $14,000 for open divisions and $1,500 for lettered divisions • Excellent spectator viewing at 12 outdoor tennis courts, including four stadium courts for ITF divisions and indoor courts for quads • Start times for lettered division to be posted by July 11 on www.usta.com/tennislink • Proper tennis attire required • Tournament players must be USTA or ITF members with proof of membership at registration • Tournament shirt and gift-bag • Scott Elbin as ITF Supervisor/Tournament Referee • Thursday night registration and activities at MVP Athletic Club- Crahen. Cut-off date for tournament discount is June 7 through July 3, depending on the hotel. Hotels are conveniently located near restaurants and a large department/grocery store. WAIVER For, and in consideration of the ITF, USTA, Mary Free Bed Rehabilitation Hospital and the tournament site, I, the undersigned, for myself, my heirs, successors and assigns, agree to release and forever discharge the ITF, USTA, Mary Free Bed and the tournament site and hosts and its officers, employees and agents from any and all liabilities, demands or claims for loss or damage which may be sustained on account of my participation in the wheelchair tennis tournament, including transportation throughout the tournament. I also consent to medical treatment in case of emergency. I agree to full responsibility for payment of any fees incurred as a result of necessary medical treatment. I hereby agree to abide by the ITF Rules of Tennis, the ITF Rules of Wheelchair Tennis and pay the entry fee as required by the tournament. I further agree to abide by the Code of Conduct enforced by the tournament. For participation in the Main Draw, I hereby agree to abide by the ITF Wheelchair Tennis Code of Conduct and agree to comply with and be bound by all the provisions of the ITF Tennis Anti-Doping Program set out in Bylaw 4 of the Constitution of the International Tennis Federation. I understand and agree that I have a medically diagnosed permanent physical disability as defined in the rules of wheelchair tennis, found at www.itftennis.com/wheelchair/ rules/eligibilityrules.asp and that I am eligible to compete in ITF sanctioned wheelchair tennis tournaments. I understand that if requested by the ITF, I am required to supply appropriate medical documentation that substantiates the disability. Your signature below constitutes permission to be photographed for possible promotion, publicity, or media purposes and constitutes a waiver of any and all claims for compensation from Mary Free Bed. I hereby give my permission to Mary Free Bed, to use my (or my child’s) name and/or likeness, city and state, in the form of photographs, videotape, interviews, slides, movies, or recordings, and the use of statements made by me or attributed to me (or my child) for publicity purposes or similar promotions relating to Mary Free Bed any and all rights to said use without any compensation. It is my understanding that my signature below releases Mary Free Bed from any financial or legal responsibility for the use of the media relations/advertorial/ promotional materials named above.
