    •   Provides both Initial Diagnosis and Second Medical Opinion
    •   Initial Diagnosis Assistance
         •     provided if you do not receive a diagnosis from your doctor within 60 days
         •     medical assistance through one of the Centres of Excellence in the U.S.
                   •   eg. Cleveland Clinic, John Hopkins or Henry Ford
         •     Includes additional diagnostic scans (eg. CT’s, MRI’s or PET scans).
         •     Medical collaboration between the U.S. specialist and your doctor

    • Can be requested after diagnosis is provided by your doctor
    • Includes in-depth review of your current medical file
    • Includes additional diagnostic scans (eg. CT’s, MRI’s or PET scans).
    • Medical collaboration between the U.S. specialist and your doctor
    • Receive a full medical report with recommended treatment plan from
      your reviewing U.S. physician

    (Note: MyCare HBO is subject to a pre-existing condition clause so it is important to
    enroll early and before developing a serious health condition).
