What is Health and Well-Being? • An exciting wellness program by HealthPartners • Encourages employee health while driving down costs • Take the health assessment and complete one wellness program to qualify for the Preferred Benefit 2
2021-2022 Program • Participate between July 1, 2021 and March 31, 2022 • Employees and covered spouse complete the requirements to qualify for the preferred benefit that is effective July 1, 2022 • Voluntary 3

If one chooses not to participate – both the employee and spouse (and dependents on the plan) get the non-preferred benefit level 4
Step 1: Take the health assessment Go to healthpartners.com/wellbeing-login *For Returning Members, log in using your existing username/password. *For New Members, click on “Create an account”. If you don’t remember you username or password, use the Forgot username or password links on the login page 5
Step 2: Complete a wellness program Programs for any health level and all interests • *NEW* myStrength: A flexible and comprehensive digital program to support emotional health and well- being with proven tools and activities for stress, depression, sleep and more. • 10,000 Steps: Track your steps and activities to see how active you can be • Choose to Lose: Try three new habits and track those habits over the course of the activity to encourage healthy weight loss • Frequent Fitness: Reward your body and wallet – get money back each month for working out! • Go for Fruits & Veggies: Try three new habits and track those habits over the course of the activity to increase your fruit and vegetable intake • Healthy Thinking: Build awareness of your stress-related negative thoughts by practicing new habits • Sleep Tracker: Get better sleep by tracking how much (and how well) you sleep • Stress Tracker: Stay on top of your stress by tracking your stress levels and stress triggers • Sugar Smart: Learn all about sugar and what types are OK and commit to new habits • Tackle Stress: Engage in mindful breathing, body scan meditation and the practice of gratitude • Well@Work Screening: Become informed on your overall health and possible risk areas with a Biometric Screening completed at the Well@Work Clinic. Well@Work Biometric Screening* Do you know what a healthy cholesterol level is? How about an optimal blood pressure? Get screened so you can learn more about your overall health and possible risk areas. Well-being Phone coaching Our health coaches partner with you one on one so you can live the life you want. Schedule phone calls when it’s convenient for you. • Reach your health goals with the support of experts in the following areas: • Back health (low back pain) •Weight management • Blood pressure management • Stress • Cholesterol management • Physical activity • Chemical health • Nutrition • Emotional health 8
Frequent Fitness • Participating the Frequent Fitness program can now qualifying as a Wellness Program! • Attend a qualifying fitness center at least 12 times a month for any two months between July 1, 2021 and February 28, 2022 (note shorter deadline than March 31, 2022) • Be sure to still complete the Health Assessment! 9

Mobile app Get healthy with the mobile app. With myHealthPartners, you can take the Health Assessment and log the healthy activities you’re already doing. Step 3: Qualify for Preferred Benefit • Take the health assessment and complete a wellness program (Step 1 and Step 2) (Employee and covered spouse, if applicable) • Qualify for Preferred Benefit – Copay Health Plan – $20 differential of the office visit co-pay – Teacher VEBA, Nutrition Services VEBA, HSA and ACA: – $250 single/$500 family deductible differential For example, if an employee and spouse on the Copay plan choose not to participate, then starting July 1, 2022 the employee and spouse (and dependents on the plan) will have a office visit copay $20 greater than those that completed the program. 12
Important dates • January 31, 2022: Suggested last date to start a wellness program. Wellness programs can take up to 8 weeks to complete. • March 31, 2022: You and your covered spouse (if applicable) must complete the Health and Well-Being program (health assessment and wellness program) by this date to qualify for the Preferred Benefit 13
