2019 TIS DIGITAL AND EOTC CONSENT FORMS NAME: ROOM: YEAR: Please read through the booklet together with your parent/s or caregivers. Fill in the necessary information and sign the forms along with your parents and return to your teacher as soon as you can.
Digital License The Cyber Safety Use Agreement and BYOD Responsible Use Guidelines will be discussed with all learners at the start of the school year. A full version of the cyber safety agreement is available on the school website www.taradaleint.school.nz or a hard copy may be obtained from the school office. The BYOD information requires all devices coming to school to be “registered” with the teacher. Parents: Please read these carefully with your child and sign the following agreement. Student responsibilities include: Read the Cybersafety Use Agreement document carefully with my parent or caregiver. A full copy of the Cybersafety Use Agreement is available on-line at www.taradaleint.school.nz. This can be accessed at any time. Read the BYOD Responsible Use Guidelines carefully with my parent or caregiver. Follow the TIS Digital Citizen Code whenever I use school ICT resources and equipment. Follow the TIS Digital Citizen Code whenever I am using my personal ICT/devices on the school site or at any school-related activity. Have no involvement in use of ICT which could put me at risk, or other members of the school community – this includes the misuse of social media sites. Take proper care when using computers and other school ICT equipment/devices. If I have been involved in the damage, loss or theft of ICT equipment/devices, I may be responsible for the cost of repairs or replacement. Ask my teacher or my parents if I am not sure about something to do with this agreement. I have read and understand my responsibilities, and agree to follow the Cybersafety Use Agreement. I know that if I breach this use agreement, there may be serious consequences. Section for parent/caregiver My responsibilities: I will read the School Cybersafety Use Agreement document and discuss the rules with my child. (Full copy on-line at www.taradaleint.school.nz.) I will ensure this version of the agreement is signed by my child and I. I will support the school’s Digital Citizen Code by encouraging my child to follow the cybersafety rules both at school and at home, and to always ask the teacher if they are unsure about any use of ICT. I have read the Cybersafety Use Agreement and the BYOD Use Guidelines and I am aware of the school’s initiatives to maintain a cybersafe learning environment, including the responsibilities involved. Digidale and Website General Appearance Release Form Dear Parent/Caregiver In todays digital world it is important that we have the permission of parents and learners to publish their work and their images in an electronic format. Please take the time to read the information below. Each classroom will have the opportunity to visit our TV Studio “Digidale’, located on site. The sessions involve learners actively participating in the production of a live television news broadcast. The show is broadcast internally on our school network and then will be available for parent viewing via Vimeo. This letter is a general appearance release form for your child. The following rights are given on the understanding that the recordings will be used only for the publication of learners’ learning. 1. Production: Digidale Studio Live Broadcast 2. Recording Rights: I give Taradale Intermediate the following rights in relation to my child: Child’s Name: ______________________________________________ • The right to record your child for the purposes of a Digidale Programme to be available on VIMEO (password protected website.) I also grant Taradale Intermediate continuing rights to use now, and in the future on the TIS Website: • Reproductions and representations of your child’s physical likeness; and • Sound and voice recordings of your child; in any way they require in order to improve the learning outcomes for children. These reproductions or representations may appear as still camera photography and/or video images and/or digital images. Examples of required uses include (but are not limited to) transmission in any manner, subsequent DVD and/or digital release, publication on the internet or promotional material. Taradale Intermediate “Bring Your Own Device” (B.Y.O.D.) Responsible Use Guidelines Purpose Taradale Intermediate School uses digital technology as one way of enhancing the teaching of skills, knowledge and behaviours learners will need as responsible citizens in the global community. Learners learn collaboration, communication, creativity and critical thinking in a variety of ways throughout the school day. In an effort to encourage and develop these skills, we allow 2 personal devices on our TIS BYOD Student network for learners who follow the responsibilities stated in the Digital License and the guidelines regarding BYOD. An important component of BYOD at our school will be education about appropriate online behaviors. We continuously review cyber-safety guidelines with learners throughout the year and outline what is expected of them as a digital citizen at our school. Learners will be expected to comply with all class and school rules while using personal devices. GUIDELINES Anyone utilizing devices must adhere to the schools Digital License. This includes the Cyber safety Use Agreement, all Board Policies and adherence to the NZ Copyright Act. Each teacher will monitor the use of personal devices in the classroom. Approved devices must be in silent mode while at school, unless allowed by a teacher. Headphones need to be at school daily. • Devices may not be used for non-instructional purposes (such as making personal phone calls, social media and text messaging) during school time. • Learners may not use devices to record, transmit, or post photographic images or video of anyone at school during school hours or during school activities, unless allowed by a teacher and with permission of the other learners. • Devices may only be used to access internet sites which are relevant to the classroom curriculum. • Sharing of personal devices with another child is at the discretion of the parent and child involved. Please have this discussion with your child. • All devices must be clearly named so they can be identified. Secure passwords on devices that have this function must be used.
Learners and Parents/Caregivers acknowledge that: • The school’s network filters will be applied to a device’s connection to the internet and any attempt to bypass the network filters is prohibited. • Devices must have up-to-date anti-virus software installed – this includes mobile devices. This is available through Norton or AVG. • Taradale Intermediate School will reserve the right to collect in any device that is suspected of causing technology problems or was the source of a virus infection. Parents will be asked to collect the device and remedy the problem. • When suspected misuse of a device has occurred (eg access to inappropriate sites, inappropriate use of social media, violation of others privacy), learners will be asked to surrender the device to senior management and parents will be contacted to collect the device. • Printing from personal devices may be possible at school via cloud apps and school based technology. Printing directly from the device will not be possible. • Personal devices must be charged prior to school and run on battery power while at school. Lost, Stolen, or Damaged Devices Learners are responsible for their own device and should use it responsibly and appropriately. Taradale Intermediate takes no responsibility for stolen, lost, or damaged devices, including lost or corrupted data on those devices. Teachers have a lockable storage space in their rooms, but learners have the final responsibility for ensuring their personal devices are locked away. Please check with your homeowner’s policy regarding coverage of personal electronic devices, as many insurance policies can cover loss or damage. Usage Charges Taradale Intermediate School is not responsible for any possible device charges to your account that might be incurred during approved school-related use. Network Considerations All learners will use the TIS BYOD Student wireless network to access the internet. TIS does not guarantee connectivity or the quality of the connection with personal devices. We are not responsible for maintaining or troubleshooting learner BYOD devices.
AGREEMENT This signed agreement is to be returned to your child’s teacher who will keep it on file. Devices will not be permitted at school until this agreement has been returned. I/We understand and will abide by the above policy and guidelines. I/We further understand that any violation may result in the loss of network and/or device privileges as well as other disciplinary action. Personal Responsibility Agreement This is an agreement that covers all learners in the school for out of school activities (EOTC): sporting, cultural, service, learning trips and other such activities. It is about appropriate, responsible, safe and respectful attitude and behaviour that we expect from learners at TIS. It also acknowledges that the school has a duty of care to all learners, our staff and the community of people we may be working with when outside the school. Learner Name I understand that these events are an opportunity for me to learn, practice skills, and gain attitudes and values in an environment outside the classroom. I realise that this requires me to take on the responsibility for my own learning and safety and that of others. I agree to do the following to make this happen: BE SAFE – KIA MARU Look after myself and my personal belongings. Declare medical conditions that could affect participation in the event. Have the necessary equipment for the event or specific to the sporting code that I am involved in. Make sure a staff member knows where I am at all times. BE RESPECTFUL – KIA WHAKAUTE Show courtesy and consideration to others. Follow the rules and instructions of staff and other supervisors at the event. Wear uniform correctly and with pride. Use appropriate language and be a supportive participant/spectator. BE A LEARNER – HE ĀKONGA Accept the rules set by the school for the event, even if they are different from what is accepted at home. I understand that my parents / caregivers will be contacted and I may be sent back to school or home if: My actions are considered unacceptable by staff; I breach the schools positive behaviour for learning code; My actions put me or others in any danger.
