What is Te Tumu Waiora?
o Response to recommendations from He Ora Oranga report
o National model – basis of budget funding $455m 2019
o New way of delivering mental health, addictions and
  wellbeing care through general practice
o Health professionals with specialist training (psychological
  and wellbeing interventions) work as part of the general
  practice team
o Holistic model which supports and addresses the physical,
  emotional and social needs of the person rather than
  focusing on traditional mental health or addiction needs
o Focus on equity and access

Roles - Health Improvement Practitioners (HIPs)
• Mental Health Clinicians

• HIP’s may be psychologists, nurses, GPs,
  social workers, psycho-therapists or other
  health professionals with a mental health
  qualification.

• Deliver focussed evidence informed
  talking-therapies for all age groups.

• Provide advice, training and support for the
  general practice team.

Health Improvement Practitioners (HIPs)
• Low wait times (warm handover)
• Services delivered within the practice
• Interventions 15 – 30 minutes, and only as
  many as are needed to get a result
• Individual, family/whānau and group based
  interventions
• Write in the shared general practice medical
  record.

Health Coaches
• A non-registered role working as part of the
  integrated team.

• Assists patients to gain knowledge,
  skills and confidence to become informed
  active participants in their healthcare.

• Goal setting and self management is a
  core focus.

• May be peer/cultural health coaches.

• No eligibilty criteria –
  any enrolled patient can
  be seen.
• NGO support workers

  (housing, economic
  issues, pre-vocational

Next steps for Te Tumu Waiora

o Continue to implement at pace

o Strengthen relationships with all organisations involved

o Explore existing roles and how these fit into the Te Tumu Waiora model

o Working with smaller practices to develop and implement cluster model

o Continue to focus on our priority populations, particularly Māori, Pasifika
  and Culturally and Linguistically Diverse people, to provide greater choice
  and access to the service
The challenge   • Roll out phased over 4 years so will occur in
                  stages

                • Available workforce capacity is a significant
                  consideration

                • It is really important fidelity to the model is
                  maintained for consistency/success of service
