Environmental Public Health ORDER OF AN EXECUTIVE OFFICER NOTICE OF CLOSURE To: Ka1la Ltd. “the Owner” Youngkang Zhao “the Owner” Hong Yang Jhang “the Owner” RE: The food establishment located in Calgary, Alberta and municipally described as: Hot Monsters 2213 – 2223 Centre Street NW WHEREAS I, an Executive Officer of Alberta Health Services, have inspected the above noted premises pursuant to the provisions of the Public Health Act, RSA 2000, c. P-37, as amended; AND WHEREAS such inspection disclosed that the following conditions exist in and about the above noted premises which are or may become injurious or dangerous to the public health or which might hinder in any manner the prevention or suppression of disease, namely: a. An abundance of mouse droppings was observed along walls, behind equipment, on shelves and hard to reach areas throughout the food establishment. b. The temperature of a bucket of cooked chicken feet and one bowl of raw beef on the floor and on the back kitchen sink counter respectively, measured 19 degrees Celsius. c. Sanitizer solution was not prepared for use on kitchen food contact surfaces. d. Non-food grade containers and bags were used to store food. A department store orange bucket was used to hold cooked chicken feet. e. The handwashing sink near the kitchen cook line was obstructed with items in the sink basin. Liquid soap was not available at the handwashing sink. f. The paper towel dispenser was broken at the front handwashing sink. g. Staff onsite did not have adequate knowledge of safe food handling. Lemon scented bleach was being used for sanitizer solutions. h. There was exposed bare concrete over half of the kitchen floor as the epoxy paint layer had worn off. i. The grease trap was open at the time of the inspection. The grease trap was full of grease that has not been cleaned out. A trail of leaking grease from the grease trap to the floor drain was observed. j. There was accumulation of dirt, grease, and food debris on kitchen equipment surfaces, including, but not limited to: the ventilation canopy inserts, all shelves, pots, refrigerators and freezers.

There was accumulation of dirt, grease, and food debris on kitchen structural surfaces, including, but not limited to: floors, under/behind counters and shelves, kitchen ceiling and in all storage areas. AND WHEREAS such inspection disclosed that the following breaches of the Public Health Act and the Food Regulation, Alberta Regulation 31/2006, the Food Retail and Foodservices Code 2020 and Nuisance and General Sanitation Regulation AR 243/2003 exist in and about the above noted premises, namely: a. An abundance of mouse droppings was observed along walls, behind equipment, on shelves and hard to reach areas throughout the food establishment. This is in contravention of the Food Regulation AR 31/2006 section 21(1), which states that: Subject to subsection (1.1), a commercial food establishment and any surrounding area, premises or facilities supporting the food establishment must be kept free of pests and of conditions that lead to the harbouring or breeding of pests. b. The temperature of a bucket of cooked chicken feet and one bowl of raw beef on the floor and on the back kitchen sink counter respectively, measured 19 degrees Celsius. This is in contravention of the Food Regulation AR 31/2006 section 25 (1)(a)(b), which states that: All high-risk food must be stored, displayed and transported at a temperature of not more than 4ºC or such higher temperature, and not less than 60ºC. c. Sanitizer solution was not prepared for use on kitchen food contact surfaces. This is in contravention of the Food Regulation AR 31/2006 section 28(2), which states that: A commercial food establishment, all equipment and utensils in it and all surfaces in it with which food comes into contact must be maintained in a sanitary condition and, without limiting the foregoing, must be washed and sanitized in a manner that removes contamination. d. Non-food grade containers and bags were used to store food. A department store orange bucket was used to hold cooked chicken feet. This is in contravention of the Food Regulation AR 31/2006 section 23(1), which states that: All food used or to be used in a commercial food establishment must be protected from contamination and handled in a sanitary manner. e. The handwashing sink near the kitchen cook line was obstructed with items in the sink basin. Liquid soap was not available at the handwashing sink. This is in contravention of the Food Regulation AR 31/2006 section 17 (1) (e) & section 30(4), which states that: An operator must ensure that a commercial food establishment has handwashing stations, adequate in number and location, to ensure convenient access to all food handlers. Hand sinks must be maintained and kept supplied. f. The paper towel dispenser was broken at the front handwashing sink. This is in contravention of the Food Regulation AR 31/2006 section 17 (1) (e) & Section 30(4), which states that: An operator must ensure that a commercial food establishment has handwashing stations, adequate in number and location, to ensure convenient access to all food handlers. No person shall create, commit or maintain a nuisance. h. There was exposed bare concrete over half of the kitchen floor as the epoxy paint layer had worn off. This is in contravention of the Food Regulation AR 31/2006 section 17(1) & 19, which states that: An operator shall ensure that a food establishment is of sound construction and in a good state of repair and is so designed as to facilitate the effective cleaning and sanitizing of it and all equipment, utensils and surfaces with which food comes into contact in it. i. The grease trap was open at the time of the inspection. The grease trap was full of grease that has not been cleaned out. A trail of leaking grease from the grease trap to the floor drain was observed. This is in contravention of the Nuisance and General Sanitation Regulation AR 243/2003, section 2 (1), which states that: A practice or condition of this food establishment is creating a nuisance. No person shall create, commit or maintain a nuisance. j. AND WHEREAS, by virtue of the foregoing, the above noted premises are hereby declared to be Closed. NOW THEREFORE, I hereby ORDER and DIRECT: 1. That the Owner immediately close the above noted premises. 2. That the Owner immediately undertake and diligently pursue the completion of the following work in and about the above noted premises, namely: a. Retain the services of a professional pest control company to inspect, treat, and eradicate the food premises of all pests. Provide documentation from the professional pest control company to an Alberta Health Services, Public Health Inspector indicating that the food premises is free of pests. Maintain weekly/monthly ongoing professional pest control maintenance and records.

Remove and safely dispose of all mouse droppings and harborage materials. c. Clean and disinfect all surfaces contaminated with mouse feces or urine and seal all exterior entry points. d. Do not allow garbage, dirt, food debris, or unnecessary equipment to accumulate in the food establishment to aid in the prevention of pest harborage. e. Ensure that an approved sanitizing solution (i.e.100 part per million bleach or 200 part per million Quaternary Ammonium Compounds) is prepared whenever food handling is occurring. f. Do not use scented bleach to mix a sanitizer solution for food contact surfaces. g. Ensure that handwashing sinks are accessible and stocked with liquid soap and single use/disposable paper towel prior to operation. Ensure that all staff practice regular and proper handwashing at designated fully equipped hand wash stations before and between tasks. h. Repair the broken paper towel dispenser at the front handwashing sink. i. Store all potentially hazardous foods at 4°C or less or at/above 60°C. j. Use only food grade containers to store foods. k. At least one member of staff, preferably the cook, must complete a recognized food safety course approved by the Minister of Health again. Submit a copy of the completed food safety course certificate to an Alberta Health Services, Public Health Inspector. l. Refinish the kitchen floor so all floor areas are smooth, water resistant and easily cleanable. m. Ensure the grease trap is properly cleaned on a regular schedule. n. Thoroughly clean and sanitize all surfaces and equipment throughout the food establishment. o. Prepare, implement, and maintain a detailed written sanitation program to facilitate the proper cleaning of all areas of the food establishment, including equipment, and structural surfaces. The sanitation program must include a list of all cleaning and sanitizing agents used within the food establishment. The sanitation program must be submitted to an Alberta Health Services, Public Health Inspector for review. 3. That until such time as the work referred to above is completed to the satisfaction of an Executive Officer of Alberta Health Services; the above noted premises shall remain closed. The above conditions were noted at the time of inspection and may not necessarily reflect all deficiencies. You are advised that further work may be required to ensure full compliance with the Public Health Act and regulations, or to prevent a public health nuisance.
