Maternity care during Covid-19 Introduction The Rotherham NHS Foundation Trust’s Maternity Services remain committed to facilitating support for pregnant women throughout their pregnancy and childbirth. We recognise the value of the support of a partner, relative or friend for pregnant women when accessing maternity care services. We are pleased to say that we can now welcome support partners to all hospital antenatal appointments including Early Pregnancy Assessment Unit and Antenatal Day Unit. We welcome support partners onto Labour Ward for all admissions, induction of labour and on Wharncliffe Ward for two hours per day, at pre-arranged times. The Maternity Team have carried out risk assessments in all areas and we continue to review these in line with national guidelines. 3
Guidance for women and support partners: l Support partners should be a member of your household or support bubble l We ask that no children attend for any maternity appointments or scans l Masks must be worn at all times l Women and support partners must regularly wash sanitise their hands with the washing facilities/hand gels provided l Support partners should not attend if they have symptoms of Covid-19 or isolating l Women and support partners to maintain the two- metre social distance form others We would like to thank all women and families using TRFT Maternity and Gynaecology Services for their ongoing support and patience during these unprecedented times. Maternity care during Covid-19 General information about the Trust’s approach You will see that we are promoting social distancing on our sites by altering seating areas where we can, introducing ‘stop and wait’ points, and asking all patients who have symptoms of Covid-19 to stay at home. Where it is possible and appropriate we are undertaking telephone or video consultations so that our clinics are less crowded. All our staff are trained in the use of Personal Protective Equipment (PPE) including when and how this should be worn. We are following national guidance on PPE, which is subject to change. We have identified and equipped separate areas in the hospital to care for patients with Covid-19. Our maternity unit and neonatal unit also have separate isolation areas identified. We provide hand washing facilities and alcohol hand gel for use at the entrance to all wards and departments, and advise you to wear a mask at all times, unless you are in active labour. If you are worried about yourself or your baby, please get in touch. Your care will be provided by Midwives (and Doctors where necessary) in the normal way during the Covid-19 pandemic. We can make safe arrangements for you and your baby if you have tested positive for Covid-19, have symptoms, or have been in contact with someone within the last 14 days who has tested positive/displayed symptoms. 3
Maternity care during Covid-19 Your Care During Pregnancy Urgent care If your baby’s pattern of movements changes or reduces in number, please call and speak to a Midwife promptly on 01709 427700. If you are advised to attend the Antenatal Day Care Unit or Maternity Triage Unit, please attend at the allotted time. Support partners can attend with you but please be mindful that we are maintaining social distancing rules, so your partner will be invited onto the ward once you have been escorted to the allocated assessment area prior to your assessment beginning. You and your support partner will be asked to attend your appointment wearing a mask, and will be asked about Covid-19 symptoms both prior to and on arrival. Should I still attend my antenatal appointments in the community? We want to take good care of you and your baby. Your appointments are essential to monitor the health of yourself and your baby. Some antenatal appointments will take place over the telephone, your Midwife will advise you of which ones these will be. Before any face-to-face antenatal appointments, you will be telephoned by a Midwife or a Maternity Support Worker to check if you have Covid-19 symptoms before you attend. 6
Maternity care during Covid-19 Your Care During Pregnancy If you or a member of your household is experiencing symptoms of Covid-19 or self-isolating please do not attend; in this case your Midwife will advise you on how we can facilitate your appointment safely. If you and your household are symptom free you will be invited to still attend your antenatal appointment with your Community Midwife. We are awaiting confirmation from GP surgeries about support partners attending Community Midwifery appointments. Will hospital appointments including scans still be running? Antenatal care is essential so hospital appointments will still be carried out. You should continue to attend all hospital appointments, including scans, unless told otherwise. Support partners are now able to attend all hospital antenatal appointments including scans at Greenoaks and Early Pregnancy Assessment Unit (EPAU). On arrival, you may notice we have screens in the waiting areas to help with social distancing measures. 7
Maternity care during Covid-19 You may receive a call from an ‘unknown number’ up to 48 Your Care During Pregnancy hours prior to your appointment, please answer as it may be one of our team trying to get in touch. You can expect to be asked some personal questions so that the hospital team can identify you when they call, for example your full name and date of birth. You will not be asked for any other information such as bank or credit card details. If you or a member of your household is experiencing symptoms of Covid-19 or self-isolating please do not attend your appointment. In this case, our Midwives will advise you on how we can facilitate your appointment safely. If you do need a telephone or video consultation, this will be arranged with you over the telephone. We continue to welcome support partners onto Labour Ward for all admissions, for induction of labour and on Wharncliffe Ward for two hours per day, at pre-arranged times. As antenatal classes are cancelled, is there any further information I can receive? Community Midwifery teams are providing antenatal classes virtually, which they will invite you to book onto during your pregnancy. We have a virtual tour of Delivery Suite available on the Trust’s website and YouTube channel, along with some antenatal education sessions lead by our very own Midwives. We also have an e-Midwife service (see page 13 for how to access this) meaning you are able to ask any questions that you may have via social media messaging services. 8
Maternity care during Covid-19 Admission to Hospital Your Care During Labour Anyone who needs to stay with us will be swabbed for Covid-19 in line with government guidance, unless they have had a positive Covid-19 result within the last 90 days. During labour All women are advised to call the Delivery Suite when in early labour for advice. When you attend Triage or Delivery Suite, you may have one birth partner during this time. Before admission, we will ask questions about your birth partner’s health. If they have any symptoms of Covid-19 or are self-isolating, they will be advised not to attend the unit with you. It is advisable when making your plans for labour and birth to identify alternative birth partners in case this becomes necessary. We advise that anyone who accompanies you to the Delivery Suite wears a mask at all times. You will be unable to change birth partners during labour and relatives will be unable to wait outside the Delivery Suite. This is to protect you, your birth partner, the staff and others in the area. You and your birth partner will have time together following the birth of your baby on Delivery Suite. If it is safe to do so, you may be discharged home from the Delivery Suite. 9
Maternity care during Covid-19 If you have symptoms of or confirmed Covid-19 you will Your Care During Labour be cared for in isolation with a team who have additional protective clothing. In an emergency this may cause a slight delay, as staff are putting this equipment on at speed to reduce the impact of this on you and your baby. Elective caesarean section If you are having a planned caesarean section you can have one birth partner for the duration of your caesarean birth. Once the operation has finished you will return to Delivery Suite where you can spend some time together as a family before being transferred to the ward. We expect discharge to be within 24 hours of the procedure - this is personalised to your individual clinical needs. Emergency caesarean section If this event should occur during labour and it is clinically possible, your partner can accompany you into Theatre & Recovery. However, if you need a General Anaesthetic (GA) it is not possible for your birth partner to accompany you into Theatre, but to wait for you nearby in Recovery. Once the operation has finished you will return to Delivery Suite where you can spend some time together as a family before being transferred to the ward. Your Midwife will support you throughout and will advise accordingly during this time. 10
Maternity care during Covid-19 Your Care During Labour We will invite your birth partner to escort you to Wharncliffe Ward so that they are familiar with where you will be staying, but will ask that they say their goodbyes until their allocated visiting slot the following day. Visiting slots are allocated at the beginning of each day, when a Healthcare Assistant will visit each patient on the ward and ask which two-hour slot they would like their birth partner to visit. Please note, due to social distancing measures there cannot be more than one birth partner within a bay at a time. This is to protect you, your baby, your birth partner, the staff and others in the area. Induction of labour If you require an induction of labour, your named birth partner can accompany you during this assessment. If you require admission to the ward, your birth partner can return for their allocated visiting slot. Once your labour is established or you are transferred to the Delivery Suite for any reason, your birth partner can join you. This is an assessment based on individual needs. 11
Maternity care during Covid-19 Home births Your Care During Labour Women who have booked to birth at home are advised to contact Triage on 01709 427700 for advice in early labour. The Midwife will advise you what to do. She will ask questions to assess you and your unborn baby’s health at that time. The Midwife will contact her colleagues to make sure that everything is in place to support your birth choice. This depends on preparing the available staff for an anticipated home birth and the level of available transport by our healthcare partners, should you need a quick transfer from home. This will be assessed on an individual basis. 12
Maternity care during Covid-19 After Your Baby is Born After your baby is born Once your baby is born and as long as you and your baby are well, we will encourage and support you to go home when you feel ready. Your care will be personalised to your needs and those of your baby. Screening tests available for your baby will be discussed with you and arranged by your Midwife. If you have symptoms of or confirmed Covid-19, you will be cared for in isolation with your baby. You will be encouraged and supported to breastfeed, if you wish. Should you need to stay in hospital, your birth partner will be able to stay with you, but will be advised to isolate with you in your room. Once your birth partner goes home, we ask that they do not return to the hospital to reduce the spread of Covid-19. Once you have been discharged home, we will advise you to follow national guidance by self-isolating at home for the necessary amount of time. Please be aware this is only guidance and may change at any point. We will keep you informed at every step of the way if we need to make changes to any of the outlined above, to keep you and your loved ones safe. 13
Maternity care during Covid-19 After Your Baby is Born Community visits Community Midwives are continuing to visit during this time and you will be contacted prior to the Midwife attending about how this appointment will be carried out. Please be aware these visits will involve minimal hands on contact. We also request that there are no visitors in the house during the visit and that the Midwife only has face-to-face contact with mother and baby. Registering the birth of your baby The registration of births have now resumed. This service has changed slightly in response to the Covid-19 pandemic and is now a two-step process; initial information will be taken via a telephone appointment, followed by a short face-to-face appointment. The Registry Office can provide more information on 01709 823542. Parents can make a claim for Child Benefit or Universal Credit prior to the registration of the birth.
