well as identify what needs to be corrected before surveyors arrive. Survey Readiness: Critical Element Pathways, Observations, Reviews, and Policy Calendar Surveyors use critical element pathways (CEPs) to work through the steps of their investigation and to determine compliance with several federal regulations. In addition to CEPs, other tools including observational guides are also used to direct the surveyors’ review of the facility. These same tools are available for facility leaders to use and conduct a self-evaluation of compliance. By being able to investigate these areas before survey, facility leaders are given an opportunity to correct concerns and improve processes to avoid citations. Member Rate Non-Member Rate Instructions: Schedule a time to conduct in-depth investigations of compliance, using the same tools used by surveyors, along with a review of the facility’s associated policies and procedures. The calendar below is a sample schedule. It includes the survey investigation tools and the corresponding primary clinical topics that require policy and procedures. There can be several more specific policies and procedures included under each policy topic heading. Customize according to your facility’s survey window, underperforming compliance areas, or other needs. The survey investigation tools can be found on CMS’s website in the folder titled “LTC Survey Pathways”.
