All students taking part in Intramural activities are encouraged to have a health history form on file with the Student Health Center.
This form can be filled out at www.fhsu.edu/studenthealth.
The Intramural Office reserves the right to require proof of a health exam prior to specific activities.
Insurance and Injuries
All participants engage in Intramural sports at their own risk.
All participants are encouraged to have a physical examination and obtain adequate health and accident
insurance prior to participating in Intramural activities.
First aid will NOT be administered by the FHSU Athletic Training Staff or Campus Intramural and Recreational Sports Staff.
Injury Report
All accidents must be reported to the Intramural Office and an accident form (Form A) must be filled out.
The Intramural Department is not responsible for injuries incurred during Intramural participation or practice.
Treatment is provided by the Student Health Service during office hours “only.” The Hays Medical Center (office hours only) and the emergency room
of the Hays Medical Center will receive emergencies.
Acknowledgement of Risk
All participants that engage in FHSU Intramural sports and activities are participating at their own risk.
All Intramural activities are potentially dangerous and may cause small scratches and bruises, up to and including, death.
All participants are encouraged to have a physical examination and obtain adequate health accident
insurance prior to participating in Intramural activities. You, the participant, are not covered through the University.
Fort Hays State University Campus Intramural and Recreational Sports or any employees of Fort Hays State University,
are not responsible for injuries incurred during Intramural participation or practice.One of the most important jobs in our department is being an official. That is why we
take so much time in training and guiding our new officials. Every
employee is expected to not only fulfill the jobs of Issue/Tiger Cage,
Secretary, or even Lifeguard but also to be able and willing to work as an
official for any of the numerous events. Our officials gather knowledge for
major sports through experienced officials in the community, our own
instructional program, and by instructional videos. We are always looking for new
officials who would not only be willing to officiate our Intramural games but would
also be willing to learn how to officiate in the community, for the local school
districts. There are many opportunities that are made available through FHSU’s
