WHERE CAN I GET A COVID VACCINE? – HEALTHCARE PROVIDER

   Ascension Michigan
        Administering vaccines to established patients, and Ascension will be contacting eligible patients by email, text & phone to schedule an appointment. If you
         are not an established patient, they recommend checking with your local health department or pharmacy.
   Beaumont Health
        If you are a Beaumont patient and have a myBeaumontChart account, log into the account and make sure you have your accurate & current email address.
         Eligible patients (65+ or 50 + with health issues) will be randomly selected to receive the vaccine. You can schedule when you receive the link to your
         email.
        You can now sign up to save your spot at Beaumont.You must create a free myBeaumontChart account. Ages 16 and older.
   Detroit Medical Center
        You can fill out a vaccination request form online and schedulers will contact you if you meet eligibility requirements [65+, healthcare workers, front-line
         essential workers (teachers, first responders, corrections, child/social care, food processing & agriculture, 50+ with health issues, caregiver family members,
         guardians of children with special health needs)].
   Garden City Hospital
        GCH will be reaching out to eligible patients who have visited the hospital within the last 3 years to schedule appointments as they become available.You
         can also register via their Online Form when appointments are available.
   Henry Ford Health System
        If you are an established Henry Ford Patient, you will be contacted by text, email or phone when you become eligible and appointments are available. You
         will schedule your appointment via your MyChart account after you are notified. Appointments will fill up fast.
WHERE CAN I GET A COVID VACCINE? – HEALTHCARE PROVIDER

   McLaren
        McLaren is partnered with Walgreens to distribute vaccinations in Michigan. You can go to the Walgreens Registration site and put in your location. It will
         tell you if there are appointments in your area. You will then have to create a Walgreens account to see if you meet the eligibility requirements and sign up
         for an appointment.
   MidMichigan Health
        Starting March 4, 2021, MidMichigan Health has vaccine appointments for patients 65+; teachers, school staff & child care workers; healthcare workers, first
         responders, corrections workers, child/social workers, essential workers in food processing & agriculture. To schedule an appointment, call the MidMichigan
         COVID Hotline at (989) 794-7600, press Option 1 (Mon-Fri 7am-6pm).
   MyCare Health Center
        You can sign up for a vaccination appointment at the Center Line Clinic via their Online Form. If you need help or have questions, call (586) 783-2222.
   Sparrow Health
        Eligibility is following State of Michigan. To schedule a vaccine appointment, log into your MySparrow account, click on Visits then Schedule an Appointment
         and fill out the COVID-19 Vaccine (First Dose) schedule form, choose an appointment time. Click correct and put “COVID-19 vaccine in reason for your
         visit before you click Schedule.You will receive a notification to schedule your 2nd dose after your 1st dose has been administered.
   Spectrum Health
        Register for a vaccine using the Spectrum Health Vaccine Registration Questionnaire.You will be notified when an appointment becomes available for you.
         (Ages 16 and older)
   Or call your doctor to see if they are providing patients with COVID vaccines and how to schedule an appointment.
WHERE CAN I GET A COVID VACCINE? – COUNTY HEALTH
                             DEPARTMENTS
   Macomb
       To schedule a vaccination through the Macomb County Health Department in Clinton Township, sign up for a new appointment via their Online Portal.
        You will have to refresh the page until the portal lets you put in your name and date of birth. New appointments are added as they become available.
       For seniors (60+) with mobility issues, & those will disability/health issues (16+) and their caregivers, Macomb County has partnered SMART to provide
        vaccinations at Richmond Lenox/EMS & Sterling Heights Senior Center. Those who cannot use the internet can call (586) 421-6579 (Mon-Fri 3pm-
        7pm; Sat 8am-1pm; Sun 9am-1pm). You can also sign up online through the Smart Bus portal.
       Starting March 30, 2021, the Clinton Township location will expand appointment hours to 9am-8pm Tuesdays & Thursdays. All Macomb County
        locations will also be increasing the number of appointments per hour due to an increase in vaccine supply.
   Oakland
       To receive your vaccination through Oakland County, “Save Your Spot” through their Vaccine website: https://oaklandcountyvaccine.com/. You will need
        to provide your contact information and provide proof of your eligibility when you arrive for your appointment.
       If you cannot make an appointment online, you can call Nurse on Call at (800) 848-5533 for assistance.
   Wayne
       Eligible residents and workers are encouraged to register for vaccination with their healthcare provider (Beaumont, DMC, Garden City Hospital, Henry
        Ford, St. John Ascension, or St. Joseph Mercy - Livonia – Trinity) or with a local pharmacy (Meijer, Rite Aid) or with the Meijer clinic at Ford Field.

WHERE CAN I GET A COVID VACCINE? – RITE AID & WALGREENS

RITE AID
   Rite Aid stores are providing Michigan residents with COVID vaccinations.
   Eligibility is listed on their website (Ages 65+; Ages 50+ with high-risk conditions and Ages 50+ pregnant individuals; Childcare
    workers; Family childcare providers; Healthcare workers; Head Start and Early Head Start workers; Long-term care facility staff;
    Pharmacy staff; Pre-kindergarten – 12th grade educators and staff; Corrections staff). Eligibility will change as local and state
    eligibility changes.
   If you are not able to register due to eligibility, check back at a later date.
   You can check your eligibility and schedule an appointment using the Rite Aid COVID-19 Vaccine Scheduler.
   * You cannot schedule a vaccination appointment by calling local Rite Aid stores.
WALGREENS
   You can go to the Walgreens Registration site and put in your location. It will tell you if there are available appointments in
    your area.
       You will then have to create a Walgreens account to see if you meet the eligibility requirements and sign up for an
        appointment.
   If there are no appointments available, you will have to check back in a few days.
WHERE CAN I GET A COVID VACCINE? – CVS & KROGER

CVS
 Eligibility: People age 50+ ; Teachers K-12, Daycare and preschool workers, and staff; Age 16+ with a high-risk medical
   condition that increases risk of severe illness from COVID-19; Caregivers, family members, guardians of children with
   special health needs, including any physical, mental, sensory, behavioral emotional, or cognitive impairment or condition;
   Food and agriculture workers.
 Michigan Locations: Detroit, Holt, Jackson, Lansing
 Use their website (click Michigan) to check whether there are any appointments available. If there are not, check back at
   another time.
KROGER
 Available in select locations.
 Current eligibility: Individuals 50+, frontline essential workers (including grocery workers), first responders, and educators,
   16+ with medical conditions or disabilities, and caregiver family members and guardians who care for children with special
   healthcare needs.
 Schedule your appointment via their Online Portal. You may input any nearby city to see if there are appointments available.
   Check back frequently for more openings.You will be scheduling both 1st and 2nd dose appointments at the same time.
WHERE CAN I GET A COVID VACCINE? – MEIJER

 Meijer receives new vaccinations weekly.
 Visit the Meijer COVID Vaccine portal to register. Provide your information (you must create an account).You will be
   called or texted a 6 digit confirmation code.
 Text “COVID” to 75049 to register and be notified when you can schedule an appointment to be vaccinated.
 You will be placed on the list and contacted via call or text when you are able to schedule an appointment (usually the
   same week you receive the notification). Say “I Accept” to schedule you appointment offered to you.
 If you cannot be available for an appointment that week, say “I Defer” to keep your spot in line but allow someone
   ahead of you. If you have received your vaccination somewhere else, say “Remove Me” to be removed from the
   waiting list.
 As most Meijer COVID vaccine clinics will be providing the Pfizer or Moderna vaccines, you will need to make an
   appointment for the second dose of the vaccine. After you receive your first dose, you will be automatically invited
   back to a second clinic to get the second dose.
 If you want to register for the Ford Field/Meijer vaccination clinic, do not use regular Meijer sign up, make sure to use
   the Ford Field clinic only. If you sign up for a second appointment, your second sign up with supersede your first one.
WHERE CAN I GET A COVID VACCINE? – FORD FIELD

 Registration for COVID vaccine appointments at Ford Field in Detroit are now open, with appointments
   starting March 24, 2020.
 Appointment will be scheduled 7 days/week for 8 weeks, 8am-8:30pm.
 Starting April 5, 2021, all SE Michigan residents age 16 and older can be vaccinated at the Ford Field clinic.
 Sign-up to reserves a spot in line:
    Online using the Meijer portal (must create an account): https://clinic.meijer.com/register/CL2021 (this
      link is different from the main Meijer sign up).
    Test “EndCOVID” to 75049
    Call the MDHHS COVID-19 Hotline at (888) 535-6313 (Press 1) Mon-Fri 8am-5pm, Sat-Sun 8am-
      1pm (to prevent longer wait times, please only call if you cannot register by text or online).
 Once you have registered, you will receive an invitation by voice or text when it is your turn to schedule
   your appointment – appointments will be scheduled a few days in advance.
 Soon the Health Department will share how walk-up appointments will become available.
WHERE CAN I GET A COVID VACCINE? – TCF CENTER (COBO HALL)

   Any Detroiter age 50 or older can now schedule an appointment by calling (313) 230-0505 Mon-Fri 9am-6pm.
   There will be no cost to individuals for the COVID-19 vaccination.
   One caller can book for multiple people in your car (drive-thru vaccination in the TCF parking garage). You will need 6 pieces of
    information for each person when you make the call:
       Name, Address, Date of Birth, Phone number, Email, Have you had previous allergic reactions to vaccines.
   Further, any Detroit resident 50+, or 18+ who lives or works in Detroit, and must work in-person at their job site, can be vaccinated at
    Community sites:
   Appointments are available (call the above number for scheduling) at the following locations on:

   You can submit your COVID-19 Vaccine Questions through the McLaren online portal: https://www.mclaren.org/main/coronavirus-

   VaccineSpotter.org
       VaccineSpotter can be used to search locations and clinics near you that are offering COVID vaccinations. It will tell you
        whether the location currently has any open appointments, within the last 5-30 minutes. The site will link you to the clinic’s
        website to register for an appointment.
   VaccineFinder.org
       VaccineFinder can be used to search locations and clinics near you that are offering COVID vaccinations. You can also narrow
        the search by vaccine manufacturer (Moderna, Pfizer, & Johnson & Johnson). This is useful for people who would prefer the
        singular dose Johnson & Johnson vaccine.
       VaccineFinder is not for scheduling appointments, it shows where vaccine appointments may be found, but does not guarantee
        that vaccine appointment are currently available.
   If you need help finding a place to get a vaccine, or advice on scheduling, the Facebook group “Detroit Area Vaccine Hunters –
    Michigan” is very helpful. However, the members of the group are not professionals and any information they provide about the
    vaccine should be fact-checked.
