THE WELLNESS CREDIT PROGRAM IS AN OPPORTUNITY FOR YOU AND YOUR SPOUSE/DOMESTIC PARTNER
TO FOCUS ON PUTTING YOUR HEALTH FIRST.
This year, we’re starting the program earlier and adding more ways to earn points. With recent challenges as
a result of COVID-19, completing your Biometrics Screening will be optional for 2021.
HOW YOUR POINTS ADD UP:

NEW Horizon Health Watch the Horizon Health Guide video on MyEvive and enter the 50 points
Guide Video completion code at the end of the video.
NEW Flu Shot Attest to getting a flu shot and/or at least one dose of the 50 points each
and/or COVID-19 vaccine.

Gap in Care If you have a “gap in care”, that is, you have not recently completed a 300 points
Screening preventive screening recommended for your gender and/or age, you’ll
see these listed on MyEvive. (Not all participants will see this option.)*
Omada Eligible if you meet Omada clinical guidelines and complete 300 points
(for weight loss) nine sessions by the deadline. (Not all participants will see this option.)
Weight Watchers Track at least one time per week for eight weeks through Sanofi US’s 300 points
subsidized program.

Wellness Start the year strong with the eight-week 10K-A-Day challenge beginning 100 points/
Challenges in January. Stay engaged in the Spring with the six-week Work of Art challenge

Dental Exam Schedule and complete your annual dental exam. 100 points
Health Quest Complete a Health Quest challenge through MyEvive. 50 points
Challenge Note: There are multiple challenges to choose from. You can only get
credit for one. Each challenge takes at least four weeks to complete,
so you must start before September 1, 2021 (or sooner for certain
challenges) to have enough time to finish and receive the points.
Monthly Step Register for AktiveTrak on MyEvive. Sync your device or record your daily
Tracking activity to earn points.**
• 100,000–199,999 monthly steps (3,333–6,666 average daily steps) 30 points/month
• 200,000–299,999 monthly steps (6,667-9,999 average daily steps) 40 points/month
• 300,000+ monthly steps (10,000+ average daily steps) 50 points/month

NEW SmartDollar Get 2,000 SmartDollar points by completing various tasks on the 100 points
SmartDollar site.
Employee Register and participate in a Sanofi Employee Resource Group. 50 points

** AktiveTrak wellness points are sent monthly to Evive and will only be viewable on MyEvive approximately seven days after the end of
each month. All activity has to be recorded by 11:59 PM CT on the last day of the month to earn monthly wellness points, so be sure to
sync/record often. Your step count will reset at the beginning of each month.
