Pre Form 15
Post Form 16
Reasons why forms are important 1. Measures whether the program continues to be effective 2. Discovers who is participating—and who is not 3. Identifies current and new partners 4. Helps with program planning 5. Shares data for reports 6. Leads to funding opportunities 17
Talking points about forms “Thank you for filling out the Participant Information Form--it asks questions about you and your health. We follow strict rules to protect your information and keep it private, and feel free to ask us questions. We ask you to fill out this form for several reasons: To learn more about who participates in the program To measure changes in health For future program planning To provide data for continued funding” 18
Fidelity 19
Definition of Fidelity and why it is important Program fidelity refers to how closely the leaders follow the program as it was designed. Program delivery that is not true to the original design decreases the likelihood that the participants will get the desired positive outcomes. 20
