Context. Pathfield School is a Foundation school and part of the SENtient Trust. The Trust was formed in 2012 with all maintained special schools working in partnership with the Local Authority and the Cooperative College. The school is designated for 186 pupils (currently 161 on role + 15 in Pathways) aged between 3 and 19 years. In addition, Pathways is a specialist Centre for children with Social Emotional Health Needs (SEMH) and caters for a maximum of 18 students. All pupils have a statement of special educational needs or an EHC plan and dates for admission are across the year. On entry to Pathfield School, all pupils have a baseline assessment to establish their starting point for progress. Across the school we are working with pupils at a diverse range of academic levels and functional skills. We have a system for baseline assessment that ensures individual target setting is accurate, based on the Education, Health and Care Plan (EHCP) are regularly assessed to ensure progress is being made. Pathfield has a number of key partners that impact significantly on the outcomes for our pupils. These include, Integrated Children’s Services, Health Care professionals, local schools and colleges and after school providers. The catchment area for the school extends to the borders of Cornwall, Somerset and mid Devon. This plan has been developed from: - the three visioning days held during 2018/9.
