Introduction The Healthy Church Challenge is a wellness program designed to help Medicare beneficiaries reach their personal health goals and lead a healthy lifestyle. Over eight weeks, participants will have the opportunity to complete healthy activities, track their progress, and attend free online wellness events and exercise classes. The Healthy Church Challenge starts on Sunday, Aug. 29, and ends on Sunday, Oct. 24. Here’s how it works Step 1: Register your church to participate. All churches receive $800 just for registering! You can use this money to fund your health ministry or any Healthy Church Challenge-related expenses. Step 2: Create a team of individuals who would like to work on their health and wellness for the next eight weeks. Step 3: Team members complete healthy activities each week and earn points. Points are summed throughout the challenge and participants can unlock prize levels by reaching point targets by the end of the challenge. Additionally, the person with the highest number of points per week gets a $25 gift card. You can win a maximum of three $25 gift cards during the challenge. Step 4: The average number of points your team earns each week is added to the church total. At the end of the challenge, the team with the highest number of points wins its church an additional $1,000. Second place receives $500, and third place receives $250. Team member registration Once your church is registered, encourage your church members who have Medicare to form a team and register. Teams can include both church and community members, but team members must have Medicare. Individual team members must fill out a separate registration form. The form includes questions that will help us better understand the participants’ current level of activity and their overall health. Team members will bring their completed registration forms to their church contact. All registration forms must be submitted through upmchp.us/challenge-log. 1
Tracking healthy activities Participants can earn points for staying active and making healthy choices. Activity points are awarded based on the difficulty of each activity and the time spent completing it. The more time participants spend being active, the more points they earn. Bonus points will be awarded for making healthy choices, such as joining a gym, getting preventive screenings, doing telehealth visits, or eating a balanced breakfast. Participants should talk with their doctor before the challenge about which activities and lifestyle changes are right for them. This information will also be provided to participants in their welcome packet. Participants will use an activity log to track their weekly activities. Team members must provide their church contact with their completed logs on Monday of each week or use the online log to enter the information themselves. This will ensure that activity points are accurately and efficiently added to your church’s total activity points. All logs must be submitted through upmchp.us/challenge-log. 2
Activity points All types and levels of activity will earn points for the Healthy Church Challenge. Examples of activities and their point values are shown below. Participants can use this list to estimate the number of points they will earn each week. Bonus points Team members can earn bonus points by making the extra effort to lead a healthier lifestyle. Choosing healthy foods, achieving weight loss goals, attending additional events, and getting important preventive care are just a few examples. Weekly activity log Team members will complete the log below on a weekly basis. They will list the healthy activities they complete each day (including bonus points). Team members must provide their church contact with their completed logs on Monday of each week or use the online log to enter the information themselves. The church contact will enter all completed activity logs at upmchp.us/challenge-log by noon each Tuesday. Points are summed weekly. The average of the points your participants earn each week will be added to your church’s total. Individuals will earn points cumulatively over the challenge and unlock prize levels as they go. Prizes will be awarded based on individuals’ total scores after the challenge ends. Late activity logs will still contribute to the total challenge points that your team earns. Healthy Church Challenge prizes In addition to health benefits, both team members and churches will have the chance to win prizes through the Healthy Church Challenge. Here’s how: • Participants will use their points to unlock prizes throughout the course of the challenge. • The church with the highest total activity points at the end of the challenge will receive $1,000. Second place receives $500, and third place receives $250. • All participating team members will receive a certificate. • Medals will be given to the male and female participants who lose the most weight, earn the most points, and show the greatest gains over the challenge. • Church-designated contacts who have the highest retention and the highest number of participants will receive a prize at the end of the challenge. Program surveys Before and after the challenge, participants will be weighed and blood pressure reading recorded to help us better understand their current health care and challenge progress. 6
Challenge rules and regulations Please read the following rules and regulations carefully. These are the “official rules” of the program. By participating in the Healthy Church Challenge, you (“Entrant”) agree that you conclusively and unquestionably understand and accept the official rules, including all eligibility requirements. 1. No purchase or payment of any kind is necessary to enter or win the Healthy Church Challenge. The purchase of a UPMC Health Plan product does not increase one’s chance of winning. 2. Sponsor: The Healthy Church Challenge is sponsored by UPMC Health Plan, 600 Grant Street, Pittsburgh, PA 15219 (“Sponsor”). 3. Eligibility: The Healthy Church Challenge is open to any church, faith community, or civic organization (“church team”) that wishes to participate, and any Medicare-eligible individual(s) on Medicare, whether or not he or she is currently enrolled in a UPMC Health Plan product. To participate, individuals must be enrolled as part of a church team. For purposes of this contest, the participating church teams may not restrict membership on a church team to members of that organization or congregation. 4. How to Enter: The Healthy Church Challenge begins on Aug. 29, 2021, at 12:01 a.m. (ET) and ends on Oct. 24 at 11:59 p.m. (ET) (“contest period”). Participants must fill out the application to join a church team. Each enrolled organization will receive $800 for participating in the Healthy Church Challenge. 5. Winners: Team prizes will be awarded to church teams to be distributed or retained as the organization determines. Winners will be determined based on the total amount of activity points and bonus points, as detailed in the program materials, accumulated during the contest period. Individual prizes will be awarded at the end of the challenge based on the total number of points earned by each participant. Sponsor reserves the right, in its sole discretion, to change the prizes, cancel, terminate, or suspend the Healthy Church Challenge, in whole or in part, for any reason, including, without limitation, if causes beyond its control corrupt or affect the administration, security, fairness, or proper conduct of the contest. In such case, sponsor will select the prize winners from all eligible church teams properly enrolled prior to and/or after (if appropriate) the action taken by the sponsor. By entering the contest, entrant agrees to be bound by these official rules. 6. Prizes: There will be a total of three prizes, awarded at the end of the contest period: (1) a $1,000 Grand Prize; (2) a $500 second-place prize; and (3) a $250 third-place prize. Prizes are non-transferable. There will be a total of six individual prizes consisting of a $25 gift card for use at a merchant of sponsor’s choosing, awarded on a weekly basis during the contest period. An individual participant can win the Individual Prize no more than three times. If an individual participant has already won the Individual Prize three times and again totals the most in a given week, the Individual Prize for that week will be awarded to the individual participant with the next highest point total. 7
7. Notification: The prize winners will be notified at the virtual award event at the end of the program. Prizes and certificates will be mailed to participants church. If a prize winner cannot be contacted, if a prize is returned as undeliverable or is not accepted, then such prize will be forfeited and an alternative winner (“alternate prize winner”) will be selected. The prizes will be awarded to the original prize winners or an alternate prize winner under all circumstances. 8. Publicity: Except where prohibited, participation in the Healthy Church Challenge and acceptance of the prize constitutes each prize winner’s, as well as each prize winner’s individual participant’s, consent to Sponsor’s use of his/her name, hometown, and any statements made regarding the Healthy Church Challenge or Sponsor for advertising and promotional purposes in any media without further payment or consideration, except where prohibited by law. 9. Conditions: The prize winners may be required to provide Sponsor with proof of identity when claiming the prize and agree to accept all terms, conditions, risks and limitations set forth in the official rules. Sponsor makes no representations with respect to the potential tax implications associated with receipt of the prize. The prize winners will be solely responsible for payment of all applicable taxes. 10. Limitation of Liability: EACH ENTRANT AGRESS TO RELEASE, HOLD HARMLESS AND INDEMNIFY SPONSOR, AND SPONSOR’S PARENT COMPANY, EMPLOYEES, REPRESENTATIVES, AFFILIATES AND AGENCIES OF EACH FOR ANY LIABLITY WHATSOEVER FOR INJURIES OR DAMAGES OF ANY KIND SUSTAINED IN CONNECTION WITH THE USE, ACCEPTANCE, POSSESSION OR AWARDING OF ANY PRIZE OR FROM ANY PRIZE OR CONTEST RELATED ACTIVITY, INCLUDING, WITHOUT LIMITATION, ANY INJURY, DAMAGE, DEATH, LOSS OR ACCIDENT TO PERSON OR PROPERTY. 11. Official Rules and Winners List: For a copy of these official rules and/or the names of the prize winner, contact Camille Clarke-Smith, UPMC Health Plan, 600 Grant St., Pittsburgh, Pennsylvania 15219. Requests must be received no later than one month after the close of the Contest. 12. Choice of Law: The laws of the Commonwealth of Pennsylvania shall govern the Healthy Church Challenge; Sponsor and Entrant agree to submit to the jurisdiction of the state and federal courts located in Allegheny County, Pennsylvania for any dispute related to the Contest. In the event any such suit is filed in any state or federal court in the Commonwealth of Pennsylvania, Sponsor and Entrant shall not raise and hereby waive the defenses of lack of personal jurisdiction or venue. 8
