COMMITMENT TO E.A.S.D. ATHLETIC TEAM • we often must make cuts to rosters (size, uniforms, etc. • therefore, athletes are not permitted to leave an E.A.S.D. team early for travel teams, tournament teams, family vacations, etc. until E.A.S.D. season is fully completed (includes playoff games, equipment collection dates, etc. • same above applies to attending E.A.S.D. • Physical exam process - MUST be present, paperwork must be reviewed by our school nurses, doctor, and trainers • Insurance: All student-athletes will be insured by the EASD. See Sports’ Medicine Dept. for information on claim forms. Any questions call Bollinger Insurance (1-866-267-0092)
• Equipment Issuance 1. ALL equipment is numbered/lettered, issued through and collected by Equipment Managers for tracking purposes (DO NOT RETURN IT TO GRADE LEVEL OFFICES) 2. Failure to return = hold report cards, letters home, financial obligation, ineligibility for next athletic season, hold any awards earned, legal action by local magistrate 3. Common problems: exchanging equipment with others on team, numbers will not match the issued equipment = someone pays for equipment
