COVID-19 Testing at Vassar College Fall 2021 Semester WMCHealth will be providing your 4-7 day post arrival COVID-19 Diagnostic Test for the Fall, 2021 Semester. Below is a summary of what to expect: How do I schedule a COVID-19 Test? Returning Students: Please follow the same process you utilized last semester through the Healthelife App. Please refer to the attached “Covid-19 Scheduling Guide for Returning Students” on page 2. New Students: Please schedule your appointment at www.labfinder.com/wmchealth/ by selecting the Vassar College location. Please refer to the attached “Covid-19 Scheduling Guide for New Students” guide on page 3. Please use either Google Chrome, Safari, Firefox or Microsoft Edge when scheduling your appointment. What should I do if I have questions regarding the labfinder site or using the Healthelife App? Please send an email to COVID19CCOPS@wmchealth.org with your first name, last name, date of birth, the best contact number to reach you along with a summary of the issue you are experiencing and a member of the WMCHealth COVID Operations Team will reach out to you. Where will testing take place? Testing will take place in Aula Auditorium located in Ely Hall. What should I bring on my test date? When you come to you appointment, whether you are a returning student or new student, you will need to bring a government issued photo identification card along with a copy of your health insurance card. How will I be notified of my test results? Positive results will be called to both the student and Vassar College Health Services. Negative results can be reviewed through the Patient Portal (WMChealth.iqhealth.com) or through the HealtheLife App under “Health Record” and “Results”. 1

Covid-19 Scheduling Guide for New Students 1. Visit https://www.labfinder.com/wmchealth/. Please note only new students should follow this pathway. 2. Select your preferred Date and Insurance Carrier 3. Select a date and time that fits your schedule under the Vassar College Location 4. Sign-Up for a Username and Password and Log-In to the system 3

You will be prompted to answer the 9 New York State required COVID-19 screening questions below and agree to the WMCHealth Notice of Privacy Practices: (Please note that you can decline to answer any of these questions) a. First Covid-19 Test? b. Are you employed in healthcare? c. Are you symptomatic for COVID-19? d. Are you hospitalized? e. Are you in an intensive care unit? f. Are you a resident in a congregate care setting? g. Are you pregnant? h. Please specify your race i. Please specify your ethnicity 6. You will be prompted to fill in your home mailing address and insurance information to finalize booking your appointment. 4
