INTRODUCTORY                    LETTER
Dear Parent/s,

Thank you so much for expressing an interest in our school. We hope that the information in the next few
pages will answer some of the questions that you have regarding the school, however, please feel free to
contact the school on 010 543 5413 or email admissions@centurionps.co.za at any stage should you have
further questions.
We recognise that raising a child is essentially a parental responsibility and we are here to assist you in this
endeavour.

Please complete all the pages of this Application / Registration Form in order for us to process your
application / registration without delay.
Please provide the following documents with this application form.

   1. A copy of your child’s birth certificate or updated passport and study permit.
   2. A copy of your child’s latest school report.
   3. A copy of your child’s immunization form and growth chart.
   4. Transfer card or letter from previous Pre-school or school. (Once the learner is accepted.)
   5. A copy of both parent’s ID documents / updated passports and work permits.
   Proof of address must preferably be a municipal/ utilities account with your name and ID number.
      Telephone accounts/ bank accounts that have been FICA/ RICA checked and formal rental agreements
      are also acceptable. Applicants staying with relatives need to supply an affidavit to this effect, the
      relevant municipal account, and the certified ID copy of the relative. (Not older than 3 months.)

PLEASE NOTE: Incomplete application / registration forms will not be processed.

ADMISSIONS PROCESS FOR GR R AND GRADES 2 – 7 FOR 2022

The admissions period will be from 16 August 2021 to 15 October 2021.

Placement of learners will be prioritised as per the new Admissions Regulations i.e.

First - Home address closest to school within the Feeder Zone
Second - Sibling at school
Third - Work address closest to school within the Feeder Zone
Fourth - Home address within 30km

All applicants in each category are placed before the next category is considered. Late applications will only be
    considered once the applications received during the admissions period have been placed.

It is thus no longer first-come-first-served and it is not necessary for parents to queue to hand in applications
    on the first day.

Forms are available here – Centurion Primary Learner Information Form and from Security at the small gate.

Completed forms must be handed in at security in a sealed envelope with the child’s name and surname and the
   grade applied for. All information must be completed as this is required by the GDE.

Security will not be responsible for checking documents but an enrolment register will have to be signed. Please
   bring your own black pen.

Documents will be checked and verified by the admin office.

Parents will be mailed a confirmation that documents have been received and alerted to any documents that are
    missing/ incomplete/ incorrect. Corrected/missing documents must be submitted before the end of the
    admissions period.

Waiting lists will be compiled at the end of the admissions period according to the proximity of home addresses to
   school.

No e-mailed forms will be accepted.

At the end of the admissions period, Grade R applicants will be offered placement which must be accepted in
    writing within 7 days.

Applicants for grades 2 – 7 will only be placed once we are notified of learners leaving as the school is currently
   full for all grades. This usually only happens at the end of the year.

Grade R: the child needs to be at least 5 turning 6, in the year in which they want to join

DUE TO THE LIMITED NUMBER OF AVAILABLE SPACES, THIS APPLICATION DOES NOT
GUARANTEE YOUR CHILD A POSITION AT THE SCHOOL AND THE FINAL DECISION IS
AT THE DISCRETION OF THE SCHOOL EXECUTIVE COMMITTEE.

Once the application has been considered, you will be notified accordingly.

If your application is successful, an entrance fee equivalent to R2000 school fees is payable. Only
on receipt of this deposit can we assure you of your child’s place in the school.

PARENT AND PAYMENT CONTRACT

PROMPT PAYMENT of school fees is essential for efficient tuition of the learners, administration of the
school and assists us in keeping the fees as reasonable as possible.

   •   Please email proof of any additional EFT payments for any other transactions to
       finance@centurionps.co.za or contact the school office for any queries.
   •   Fees for 2021/22 are as follows:
       a) Grade R 1409.00 p/month
       b) Grade 1 - 7 R966.00 p/month

   •   Payments can be made into the school account and proof of payment provided to the school:
       Bank Name: Standard Bank - Centurion                 Account Name: Wierda
       Park Primary Account No: 241356954           Reference: Child’s Name,
       Surname and Grade
        The school will provide you with your family reference number, once your child is admitted.

   •   The annual school fee amount is spread over 10 months for your convenience, therefore, please pay
       promptly during the holiday periods as well.

   •   Please note that the school does not have any aftercare facilities alternative arrangements need to
       be made for your child/children.

a)      If you wish to transfer your child from the school for any
    reason, please note that a ONE CALENDAR MONTH NOTICE IS REQUIRED
    IN WRITING. (Telephonic or verbal arrangements will not be considered).
          b) The written notice must be submitted by the last day of a month and school
              fees for the notice month following would be payable in full, even if the
              child does not complete the month in the school.
          c) The refund of any pre-paid fees will be treated the same, in other words, a
              full month will be allowed to action the refund of any pre-paid school fees or
              any other fees. No other monies will be refunded via CASH, eg. School trips,
              fundays , etc.
