COVID INFORMATION • All testing by visiting teams must be completed prior to the team’s arrival on campus at Minot State University. • For non-NSIC competition, pertinent COVID protocol from the visiting institution must be received by Minot State Athletics a minimum of 3 days prior to competition. Minot State University COVID-19 Attestation Form for Non-Conference Competition Minot State University requires all teams competing against our teams to meet the weekly testing requirements as set forth by the NSIC protocols. Two members of the Athletic Department staff must sign this form within 48 hours of competition. One of the two must be affiliated with the athletic training staff (Athletic trainer, team physician, sports medicine manager). Return the completed form to Andrew.petko@minotstateu.edu By signing and dating this form, you attest that your institution has satisfied the policy requirements outlined below: 1. Your institution has complied fully with the local public health protocols, conference protocols for testing, surveillance, quarantine and isolation protocols during the week leading into competition with Minot State University. 2. All student-athletes, coaches, support staff, medical staff and travel party who will participate in the contest (hereinafter referred to collectively “travel party”) have been tested for COVID 19 consistent with conference or NCAA protocols. 3. No individual with a positive test result, or an individual who should be in isolation or quarantine has traveled with the team or is currently participating with the team. 4. No members of the travel party have had a verified positive test and have been released from isolation or have been identified as being in close contact to a person who is subject to quarantine under the NCAA guidelines. 5. Upon date and time of signature, no member of the travel party has developed new symptoms that might indicate an infection of COVID-19 between the time the most recent test was collected and the beginning of the contest. 6. Participating institutions agree to comply with game management protocols, outlined in the NCAA guidelines and Return-to-Play protocols. 7. If a member of the Travel Party develops symptoms or tests positive within 48 hours of the end of the contest, Trinity Health Sports Medicine (athletic.training@minotstateu.edu or (701) 858-3274) must be notified immediately. 8. Minot State will not provide positive test isolation. Teams should self-screen prior to arriving at Minot State University and shall not bring any persons who tested positive or experiencing symptoms on our campus. WWW.MSUBEAVERS.COM 5
COVID INFORMATION COVID-19 Screening • Every student-athlete and all athletics personnel should to be mindful of COVID-19 symptoms and practice self-health evaluations. Any individual who is feeling unwell should avoid participating in any aspect of in-person athletics activities. NSIC Adopted COVID-19 Protocols COVID-19 Testing • Please review the list of eight items and receive as best practices. • To be eligible to compete in NSIC competition and conference championships, testing must occur per NCAA SSI guidelines for competition. At the time of publication, SSI states that for competition a PCR/NAAT test within three days of the first competition of the week or an antigen test within one day of each competition is to be performed for all Tier 1 unvaccinated individuals. Note: Institutions may test their own Tier 1 individuals at a higher frequency than the competition testing requirement stated above/highlighted in yellow below, if desired. However, institutions may not impose different testing standards than stated above/highlighted in yellow below on opposing teams in order to compete as it is a conference-wide testing standard to be used for all NSIC competition. • Institutions are encouraged to follow the other NCAA SSI testing recommendations for Tier 1 individuals outlined in the table below.
