addition to your health coverage, Aetna is proud to offer many exclusive bonus
benefits and services at no cost to Aetna and SKY members!

Aetna Better Health of Kentucky:
No-Cost Bonus Benefits and Services
Monitor Your Blood Pressure at Home All members with diabetes or high
blood pressure can receive a blood
pressure cuff to monitor their blood
pressure at home.
Asthma Home Care Members with an asthma diagnosis
can get one free set of hypoallergenic
bedding and up to $150 for a yearly
carpet cleaning.
Home Delivered Meals Free healthy meal delivery for eligible
members to help with certain
conditions. Includes access to
nutrition education to support health
goals.
Health Runs Deep 12-week program that helps members
reach their health goal. The class
meets once a month as a group and
there’s a 1:1 check-in call. Rewards
are offered throughout the program.“Keeping Kids Safe” Opioid Lock Box Medicine lock box to keep kids safe.

Healthy You, Healthy Baby High Risk Provides at home monitoring tools
Pregnancy Program like a blood pressure cuff.
Momentum Members diagnosed with at least one
of the following chronic conditions
can get up to $375 quarterly to help
pay for utilities and services not
covered by Medicaid: diabetes,
asthma, cancer, COPD, heart disease,
or kidney disease and are enrolled in
supportive or intensive care
management.
GED Certification & Job Skills Training Online job skills training with the
chance to highlight skills to local
employers, GED prep course and GED
test voucher.
Getting on T.R.A.C.K (Transition Life skills program that includes
Ready Assistance & Core Knowledge) education on budgeting and more.
Digital Diabetes Solution Eligible members will receive
diabetes education and self-
management tools like a blood
glucose meter to help slow down or
stop the progression of diabetes
Alternatives to Opioids Eligible adult members can get up to
$150 to use towards acupuncture,
massage therapy, dry-needling, and
yoga.
Enhanced Transportation Eligible adult members can get up to
10 free round-trip rides per year. Can
be used for job interviews or training,
grocery stores, getting to community
health services, COVID vaccination
transport and more.
Back to School Assistance Program Free backpack with school supplies
for members aged 18 and under.
Health Literacy Program Free slow cooker course and free 6-8
weeks basic diabetes nutrition
course. Slow cooker course includeshow to make affordable, healthy
meals using a slow cooker. Get a no-
cost Crock Pot by finishing the
course.
OTC Kids Children and youth (0-18) supported by
ABHKY will be eligible for a $15 monthly
stipend covering over-the-counter
needs such as pain relievers, bandages,
thermometers, and more. (Only non-
SKY)
Period Promise Female members in Region 5, ages 15-
50, will receive a $20 monthly stipend
to spend on period products like
tampons, pantyliners, maxi pads,
flushable feminine wipes, and hand
sanitizer. (Only non-SKY)
Smoking and addiction recovery Programs are in place to provide
services tobacco cessation and behavioral
health services as a covered benefit
for members
Sports Physicals Annual sports physicals are provided as
a covered benefit for member
Start Strong Re-Entry Program 90-day re-entry program including
treatment, housing, bus passes, and
job training.
Simple Necessities Vending Machine Vending machine for members
experiencing homelessness. The
vending machine has items like
personal hygiene items, socks,
underwear, baby wipes and warm
hats.SKY Program:

Connections for Life (13-17) Free smartphone and wireless plan
Connections for Life (18-26) Free laptop
SKY Duffle Bag Program Duffle bags filled with personal
hygiene items, supplies, and a
blanket to SKY members that are
going from one home to another
Birthday in a Box SKY members get a “birthday in a
box” that includes party supplies and
a gift once a year (request must be
made to Care Manager)
Calming Comfort Collection Members with high adverse
childhood experiences (ACEs) score
or an anxiety diagnosis can receive
supplies to help calm the impact of
trauma. Items include a sound
machine, aroma therapy, and light
therapy products
LACES Program (Language, Access, SKY members in out-of-home care
Communication, Empowerment, who have been identified as deaf or
Support) hard of hearing will be screened for
unidentified language access needs
by an experienced Speech
Pathologist.
Referrals for this program must come
from DCBS, Department for
Behavioral Health, Developmental
and Intellectual Disabilities (DBHDID)
and/or a SKY Behavior Health
SpecialistYMCA Family Memberships Annual YMCA Family Membership
for eligible SKY members and their
household.
Limits and restrictions: Once per
calendar year; memberships start
January 1, 2023 and end December

Aetna Better Care Program™
$10 for Completion of Diabetic Dilated Members can receive a $10 gift card
Retinal Eye exam for completion of a dilated
retinopathy eye exam.
Eligibility: Members aged 18-75
years with diabetes
Limits & Restrictions: One gift card
per year
$20 for Follow-up visit with Mental Members 6 years and older will
Health Practitioner receive a $20 incentive card for a
follow-up visit.
Eligibility: Aged 6 and older
Limits & Restrictions: Members are
eligible after EACH visit with no
annual limit. Visit must occur within 7
days post discharge.
$25 HRA Incentive Members who are pregnant or newly
eligible will receive a $25 incentive
card for completing the Health Risk
Assessment (HRA).Eligibility: Members who are
pregnant or newly eligible
Limits & Restrictions: One gift card
every 12 months for pregnant
members. One gift card within the
first 30 days of enrollment for newly
eligible members.
$25 for completion of Dental exam All members can receive a $25 gift
card for completion of a yearly dental
exam.

Maternity Matters Program
Whether a member is newly pregnant or just had a baby, Aetna’s Maternity Matters
program offers the following benefits and incentives to help on their motherhood
journey! Members will earn rewards via a reloadable card that can be used for
healthy foods, maternity supplies, and diapers at a variety of local and online stores.$25 for completion of Initial Prenatal Visit Pregnant members can receive $25
for completing their initial prenatal
visit.

Limits & Restrictions: One per
pregnancy
$10 for completion of Pregnant members can receive an
Subsequent Prenatal Visits additional $10 for each visit.

Cribs for Moms Pregnant members can earn a
portable crib at 37 weeks or more of
pregnancy for seeing their doctor
regularly (at least 7 visits) during their
pregnancy.

pregnancy; exceptions are made for
multiple births (twins)
Family Transportation Members enrolled in Maternity
Matters program can receive
transportation provided for entire
family that includes a car seat for
children. Limited to 10 round trips (up
to 60 miles total per round trip) per

SKY Rewards
