B O A R D O F B E N E V O L E N C E A N N U A L R E V I E W | 2 0 1 9 -2 0 2 0 03 OUR REFINED Hand Heart Pocket recently embarked on a commitment to help improve outcomes for youth at risk for at least the next three years. This saw us start to direct the majority of our Significant PHILANTHROPIC and Flagship Grant funding towards programs and initiatives that aim to help at-risk youth to overcome the significant challenges they may face. Our Theory of Change explains the problem we are trying to APPROACH address for youth at risk, how we think we can help and the outcomes we hope to achieve with our charity and philanthropic partners. HAND HEART POCKET THEORY OF CHANGE THE PROBLEM WE ARE TRYING TO ADDRESS Young people at risk through trauma, disadvantage, social isolation and discrimination, are more likely to have adverse outcomes over WHO the course of their lives. These adverse outcomes might include homelessness, not completing school, unemployment, addiction, poor mental and physical health, domestic violence and interactions with the police and justice systems.
