Enrollment/Eligibility Who is eligible? You may be covered under this Plan if you are an outbound student, faculty, staff, scholar or other persons of similar description who are U.S. Citizens, permanent residents of the U.S., or Non-U.S. citizens traveling outside the U.S. with a current passport or visa to participate in studies, official business or research assignments sponsored by the University. How do I enroll? For student travel, Departments have to submit a Form RM23 in order for participants to be enrolled for this coverage. For faculty & staff, they will be enrolled when they have submitted their Trip Request in the Concur Travel System. How do I enroll my eligible dependents? Dependents are covered under this policy at no additional cost. There is no longer a need to purchase dependent coverage separately. In order to enroll your dependents, please e-mail the Risk Mgmt. Dept. (RiskMgmt@purdue.edu) that you are traveling with dependents and they will work with you to sign-up your dependents on the same plan. How do I print my ID card Online? The blanket ID card for all travelers can be printed by accessing the following link: www.gallagherstudent.com/purdue and by clicking the ID card link. This will take you to the My Benefits and Plan Information page. This page allows you access to the plan brochure, FAQ, and ID card for the International Travel plan. If you choose not to print an ID card, Purdue University’s Risk Management Department will provide you with one prior to your travels abroad. Insurance Plan Benefits What is covered under the Purdue University International Travel Health Insurance Program? The plan offers comprehensive benefits that include hospital room and board, inpatient and outpatient surgical procedures, labs and x-rays, inpatient and outpatient mental health services, physician office visits, ambulance, and emergency care and prescription drugs. The maximum benefit allowed for each accident or sickness is $500,000, unless otherwise specified in the brochure. 2

Click on “My Benefits and Plan Information” for complete details about coverage, limitations, and exclusions. How do I get my prescriptions filled? Prescription drugs are covered at 100% of the actual charges up to the plan maximum benefit. You will have to pay out of pocket for any prescriptions received while abroad and submit the itemized receipt from the pharmacy (including name of drug and dosage) to the claims company, Health Special Risk, Inc., for reimbursement of Covered Expenses as defined in the policy. What if I have a pre-existing condition, am I covered? Yes. Pre-existing conditions are covered as any other condition. Does this plan have a deductible? No, the 2020-2021 Purdue University International Travel Health Insurance Program does not have a deductible. What is covered under Worldwide Travel Assistance Services? Worldwide Travel Assistance services are covered by Gallagher Global Assistance. Gallagher Global Assistance offers 24-hour emergency travel assistance services including Medical Evacuation and Repatriation of Remains. Please note that in order for these services to be covered, arrangements must be scheduled and approved through Gallagher Global Assistance prior to receiving services. Emergency Medical Evacuation Benefit – This benefit will pay 100% of the covered expenses incurred up to $100,000, if any covered Injury or Sickness results in the Emergency Medical Evacuation of the Insured Person. Repatriation of Remains Benefit – In the event of the death of an Insured Person, We will pay 100% of the Covered Expenses up to $100,000 for the preparation and transportation of the Insured Person’s remains to his or her Home Country or home residence. Emergency Reunion Benefit – When an Insured Person is hospitalized for more than 6 days, they may select one individual to travel to where the Insured Person is hospitalized. Benefits include airfare, hotel and meals at $100 per day, to a maximum of $3,000. Political/ Security Evacuation – We will pay up to $100,000 for Covered Expenses, including transportation to the nearest place of safety or to your 3

Claims Processing If I receive a bill for services I received or need to be reimbursed, what should I do? When outside of the United States, you will likely be asked to pay for your medical care first and then will need to seek reimbursement. When you submit claims for reimbursement, you will need to have the itemized bill(s) and the completed claim form (in English.) If the treatment you received was a result of an accident, you will receive a letter from the claims administrator asking for information about the accident, i.e. was it the result of a car accident, from playing sports, etc. Your claim cannot be processed without this information, so please respond to the letter promptly. Are there any additional products available? The following product, which is optional, is available to you for an additional cost. PERSONAL PROPERTY PROTECTOR PLUS ® Personal Property Protector PLUS ® is an insurance plan that offers coverage for all types of property including; computer hardware, software, books, clothes, and electronics. This all risk policy covers fire, theft, water damage, flood, earthquake, riot, accidental damage, and vandalism. Any university property in the care, custody or control of the insured is also covered under this policy. This plan is available to students, faculty and staff.
