Introduction This document is to aid clinical services using medical oxygen to train their staff and allow for a baseline competency level to be established for its safe and effective use. For further advice or assistance with regards to safe management and use of medical oxygen please email either the Resuscitation Trainers, Medical Device Safety Officer or contact your local pharmacy representative within your service. Recording of Training All training provided in respect of medical oxygen must be recorded on the MyLearning system. Additionally, there is a record of training in medical oxygen form at the end of this document to allow for a local easily accessible record to be maintained of training provided. To record on MyLearning, the trainee must login to my learning as themselves and: • Click “Record of Learning” from the front page • Click “Other Evidence” • Click “Add evidence” • Enter the evidence name as “Local Oxygen Therapy Training” • Select the evidence type “Local Training” • Enter any comments regarding the training in the description box such as reflections • Under institution enter your Ward / Team • Date Completed should be set to the date the training was undertaken • Attach the sign off sheet at the end this document as an attachment (once complete) • Click “Add Evidence” Medical Oxygen Competencies Version 2.0 / 2020
Safety 1. Make sure your hands are clean and if you have recently used an alcohol-based hand cleaner that it has fully evaporated. If you have petroleum-based moisturiser or similar products on your hands you should not use the cylinder, for example Vaseline 2. Visually inspect the cylinder and ensure it is clean and free from obvious damage. 3. Ensure the cylinder is free from oil and grease, especially around the outlet valves. 4. Keep the cylinder away from naked flames, sources of ignition and combustible materials. Although Oxygen is not flammable it supports combustion. 5. When working with the cylinder, position yourself to the side and never lean over any of the valves. 6. Ensure that the cylinders are handled and stored in a manner that prevents them falling. Where necessary Estates are available to assist with installation correctly, in inpatient bedrooms and staff should be aware of this. 7. Ensure that any storage and use of oxygen cylinders has its ligature potential assessed and care plans, risk assessments and ligature logs are updated. 8. Always move cylinders by their approved means either by the carry handle or trolley depending on type. 9. Never lay cylinders on patient beds. 10. Be aware that oxygen can impregnate clothing and even after a period of time, clothing may ignite very easily if a staff member or patient were to smoke, causing significant burns, so smoking should be avoided at all costs. If going off site to smoke, clothing should be changed before doing so to reduce this risk. 11. Staff must not change regulators on cylinders unless they have been specifically trained by the estates team to do so. Nearly all cylinders currently in use have integrated regulators so do not need changing. Medical Oxygen Competencies Version 2.0 / 2020
Key Cylinder Components 1. Flow selector Used to set the required flow rate, it clicks between each figure to ensure correct rate is chosen 2. Contents gauge Shows remaining contents of cylinder (and when empty), green is full and red is low/empty. 3. Handwheel A dial used to turn the cylinder on and off by hand, anti-clockwise opens, and clockwise closes. 4. Fir tree outlet Attachment point for oxygen delivery devices with push on connectors. 5. Cylinder identity sleeve / Collar Label Displays the medical gas content along with important safety information. 6. Batch Label Provides details of the batch and expiry dates of the cylinder contents. Medical Oxygen Competencies Version 2.0 / 2020
New cylinder preparation procedure 1. Verify you have the correct medical gas by checking the label, you must confirm it is medical oxygen. 2. Check the expiry date on the batch label fitted to the cylinder. 3. Make sure the contents gauge is in the green zone indicating the cylinder is full. 4. Remove the tamper evident cover by pulling the tear ring. 5. Open the valve outlet cover by pulling the grey cover downwards (it is fixed to the cylinder on a hinge) to ensure it is operable and no obvious damage then close it again. 6. Ensure the flow rate selected on the top of the cylinder is set to zero and the handwheel on the side (black dial) is turned off. Medical Oxygen Competencies Version 2.0 / 2020
Administration 1. Ensure a valid prescription has been made for the patient to have oxygen therapy unless it is an emergency in which case no prescription is required and it can be administered in accordance with your level of training (ILS Level 3 should be held). A Hypoxia MAUP is available and staff should have been signed off to work under it in an emergency situation. 2. Oxygen must be prescribed on the Trust drug chart, detailing the flow rate per minute and the target oxygen saturation level. If the oxygen saturation levels are not being achieved or are exceeded, medical advice should be obtained to adjust the prescribed flowrate. 3. If intermittent oxygen is prescribed, each time the oxygen is used nursing staff should record the starting time on the drug chart and recorded in Carenotes the start time and the patient's saturation level. When the oxygen is stopped, the time the oxygen was stopped, the length of time the patient was on the oxygen, at what rate and the saturation level achieved should be recorded in Carenotes. 4. Verify you have the correct medical gas by checking the label, you must confirm it is medical oxygen. 5. Firmly push the tubing from the oxygen delivery device onto the fir tree outlet and ensure it is secure. 6. Slowly turn the cylinder on by turning the handwheel anti-clockwise until it comes to a stop (do not use excessive force) and then turn back half a turn to ensure it does not get stuck in the open position *never open the valve unless an oxygen delivery device is connected securely* 7. Set the required flow rate of the cylinder by turning the flow selector dial on top of the cylinder from 0 to the required flow rate as per the prescription. 8. Place the oxygen delivery device onto the patient as per intended delivery route a. Nasal Cannula b. Medium concentration mask (simple face mask) c. Venturi mask d. Non-rebreathe mask (must be used in an emergency situation when a flow rate of 15litres/min needs to be delivered) Medical Oxygen Competencies Version 2.0 / 2020
After administration 1. Remove the oxygen delivery device and dispose of as clinical waste 2. Set the flow rate of the cylinder to 0 by turning the selector dial on top of the cylinder 3. Slowly turn the cylinder off by turning the handwheel clockwise until it comes to a stop (do not use excessive force) 4. Ensure the cylinder is decontaminated in accordance with the cylinder suppliers’ instructions and to a level commensurate with the infection risk posed by it. 5. Ensure that the part used or empty cylinder is refilled by letting the ward's Oxygen Champion or their deputy know a cylinder has been in use. Medical Oxygen Competencies Version 2.0 / 2020
