INTRODUCTION THE HEALTH AND SAFETY OF OUR STAFF, GUESTS, AND THEIR FAMILIES IS OF GREAT IMPORTANCE TO US WE ARE CLOSELY FOLLOWING THE GUIDANCE AND RECOMMENDATIONS FROM NY STATE AND THE CDC WHILE CONTINUING TO ELEVATE OUR STANDARD OPERATING PROCEDURES IT IS OUR RESPONSIBILITY TO BE PROACTIVE DURING CHALLENGING TIMES IN A UNITED EFFORT TO KEEP EVERYONE SAFE AND HEALTHY

SOME EXCEPTIONS MIGHT BE MADE FOR HEALTH OR RELIGIOUS BELIEFS • STAY HOME IF SICK: ü STAY HOME IF YOU ARE SICK UNLESS YOU ARE LEAVING FOR ESSENTIAL MEDICAL CARE (INCLUDING TESTING) OR OTHER ESSENTIAL ERRANDS ü IF SICK INFORM DESGINATED CONTACT AND FOLLOW NEW YORK STATE DEPARTMENT OF HEALTH (NYSDOH)AND CENTERS FOR DISEASE CONTROL AND PREVENTION (CDC) GUIDANCE REGARDING OBTAINING MEDICAL CARE AND ISOLATING • PHYSICAL DISTANCING: ü PRACTICE PHYSICAL DISTANCE AS MUCH AS POSSIBLE • WEAR A FACE COVERING: ü PROTECT THOSE AROUND YOU ü YOU CAN BE CONTAGIOUS WITHOUT SYMPTOMS AND SPREAD THE DISEASE WHEN YOU COUGH, SNEEZE OR TALK ü A FACE COVERING MAY HELP REDUCE THE SPREAD OF COVID-19 ü OUR STAFF IS MANDATED TO WEAR A FACE COVER/ MASK AT ALL TIMES • PRACTICE HEALTHY HAND HYGIENE: ü WASH YOUR HANDS OFTEN WITH SOAP AND WATER OR USE HAND SANITIZER IF SOAP AND WATER ARE NOT AVAILABLE; CLEAN FREQUENTLY TOUCHED SURFACES REGULARLY; AVOID TOUCHING YOUR FACE WITH UNWASHED HANDS; AND COVER YOUR COUGH OR SNEEZE WITH YOUR SLEEVE, NOT YOUR HANDS
VACCINATION & MASK POLICY
LLAMA SAN VACCINATION AND MASK POLICY FOR STAFF • ALL OUR STAFF IS REQUIRED TO GET VACCINATED FOR COVID -19 ACCORDING TO THE KEY TO NYC MANDATE • RECORDS OF VACCINATIONS ARE KEPT IN PRIVATE RECORDS AND IN OUR OFFICES FOR OFFICIALS TO REVIEW AS NEEDED • EMPLOYEES WILL BE SCREENED FOR SYMPTOMS OF THE INFECTIOUS DISEASE AT THE BEGINNING OF THEIR SHIFT BY COMPLETING HEALTH QUESTIONNAIRE. EMPLOYEES ARE TO SELF-MONITOR THROUGHOUT THEIR SHIFT AND REPORT ANY NEW OR EMERGING SIGNS OR SYMPTOMS OF THE INFECTIOUS DISEASE TO THE DESIGNATED CONTACT. AN EMPLOYEE SHOWING SIGNS OR SYMPTOMS OF THE INFECTIOUS DISEASE SHOULD BE REMOVED FROM THE WORKPLACE AND SHOULD CONTACT A HEALTHCARE PROFESSIONAL FOR INSTRUCTIONS • ALL STAFF MEMBERS NEED TO WEAR FACE COVERINGS WHILE WORKING WITHIN THE ESTABLISHMENT • THE FACE COVERINGS MUST BE KEPT CLEAN AND SANITARY AND CHANGED WHEN SOILED, CONTAMINATED, OR DAMAGED • ALL PPP EQUIPMENT WILL BE PROVIDED BY LLAMA SAN AT NO COST TO THE EMPLOYEES • THESE POLICIES WILL BE ADAPTED AS NEW CITY, STATE AND CDC GUIDELINES OR MANDATE ARE UPDATED OR REVISED DATE: Monday September 13th, 2021
LLAMA SAN VACCINATION AND MASK POLICY FOR GUESTS • IN COMPLIANCE WITH KEY TO NYC MANDATE ALL GUESTS 12 AND OVER ARE REQUIRED TO SHOW PROOF THEY HAVE RECEIVED AT LEAST ONE DOSE OF A COVID-19 VACCINE AUTHORIZED FOR EMERGENCY USE BY THE FDA OR WHO • FORMS OF VALID THAT ARE ACCEPTED ARE: ü DRIVER'S LICENSE OR NON-DRIVER ID ü IDNYC ü STATE OR GOVERNMENT-ISSUED BIRTH CERTIFICATE ü CURRENT U.S. PASSPORT OR VALID FOREIGN PASSPORT ü PERMANENT RESIDENT CARD ü CERTIFICATE OF NATURALIZATION OR CITIZENSHIP ü LIFE INSURANCE POLICY WITH BIRTHDATE ü MARRIAGE CERTIFICATE WITH BIRTHDATE ü FOR PEOPLE YOUNGER THAN 18, A PARENT OR GUARDIAN CAN ACCOMPANY THEM TO THE VACCINATION SITE TO ATTEST TO THEIR AGE
LLAMA SAN VACCINATION AND MASK POLICY FOR GUESTS CONTINUED • AT THE TIME OF RESERVATION GUESTS MUST CHECK IN WITH THE FRONT DESK. ALL GUESTS MUST BE WEARING FACE COVERINGS AND BE PREPARED TO SHOW ACCEPTABLE PROOF OF VACCINATION AND VALID ID • ALL GUESTS MUST WEAR MASKS WHILE MOVING WITHIN THE INDOOR DINING ROOMS. THIS INCLUDES ENTERING AND EXITING THE RESTAURANT AND MOVING AROUND TO GO TO THE BATHROOM • REASONABLE ACCOMMODATION IS AVAILABLE FOR UNVACCINATED GUESTS IN SOCIALLY DISTANCED DINING AREAS. THESE ACCOMMODATIONS WILL ONLY BE MADE FOR UNVACCINATED CUSTOMERS BECAUSE OF HEALTH ISSUES OR SERIOUSLY-HELD RELIGIOUS BELIEFS • PROOF OF THE COMPLETED VACCINATION INCLUDES THE PHYSICAL VACCINE CARD, A PHOTO OF THE VACCINATION CARD, THE EXCELSIOR PASS, OR OTHER GOVERNMENT ISSUED DIGITAL PASS DATE: Monday September 13th, 2021
CLEANING & ADJUSTMENTS OF OUR FACILITIES
CLEANING SANITIZING AND DISINFECTING BEFORE SERVICE , AFTER SERVICE & DURING SERVICE • OUR FACILITIES WILL BE DEEP CLEANED WITH CLEANING SOLUTIONS EVERY NIGHT BY OUR OVERNIGHT CLEANING COMPANY • IN ADDITION, BEFORE THE RESTAURANT OPENS EACH DAY, THE FOLLOWING WILL BE UNDERTAKEN: ü THOROUGH DISINFECTION OF ALL TABLES & SHARED SURFACES ON A DAILY BASIS ü PERIODIC CLEANING CHECKS OF BATHROOMS & RECORDING CLEANING IN LOGS • ALL RESTROOMS, FOH AND BOH STATIONS WILL BE PROPERLY STOCKED WITH: ü HAND SANITIZER (FOH AND BOH STATIONS) ü DISPOSABLE FACE MASKS & N95 MASKS (FOH AND BOH STATIONS) • DURING SERVICE IT WILL BE ESSENTIAL TO KEEP CLEANLINESS AND DISINFECTION PROCEDURES ONGOING WE WILL FREQUENTLY DISINFECT AREAS OF HIGH TRAFFIC ü HIGH- TOUCH, NON-FOOD CONTACT SURFACES AND EQUIPMENT ü WAIT STATIONS AND BAR ü ALL FOOD CONTACT SURFACES AND EQUIPMENT ü BATHROOMS
FACILITIES UPDATES & ADJUSTMENTS • WE HAVE IMPLEMENTED AIR FILTERS WITH RATING OF MINIMUM EFFICIENCY REPORTING VALUE (MERV) 13 TO OUR HVAC SYSTEM DIRECTLY • WE HAVE INSTALLED AIR PURIFIERS WITH HEPA CERTAFIED FILTERS IN POOR VENTILATION AREAS SUCH AS INDOOR DINING ROOMS, OFFICES AND SMALL BATHROOMS • WE ALLOW NATURAL VENTILATION BY KEEPING DOORS OR WINDOWNS OPEN DURING SERVICE AND ADDING WINDOWS TO OUTDOOR STRUCTURES FOR MAXIMUM AIRFLOW AS POSSIBLE

INTERACTING WITH OUR GUESTS • IN THESE UNPRECEDENDENT TIMES WE WILL BE FORCED TO ASK OUR GUESTS TO COMPLY WITH SOME GUIDELINES THAT ARE IN CONFLICT WITH OUR APPROACH TO HOSPITALITY • ALL GUESTS MUST COMPLY WITH THE FOLLOWING SAFETY PROCEDURES: ü GUESTS SHOULD NOT MOVE AROUND INDOOR PREMISES BEFORE GETTING THEIR VACCINATION AND ID CHECKED AT THE FRONT DESK ü ALL GUESTS MUST WEAR MASKS WHILE MOVING WITHIN THE INDOOR DINING ROOMS. COMMUNICATION • SIGNAGE WILL BE POSTED AROUND THE PREMISES TO REMINDS US OF GUIDELINES AND EVERYONE’S SAFETY ü HYGIENE ü SOCIAL DISTANCING ü RULES & GUIDELINES • CONTACT TRACING & QUARANTINE AFTER INFECTION ü IN CASE OF AN EMPLOYEE TESTING POSITIVE FOR COVID-19 THE EMPLOYEE WILL BE SENT HOME AND ONLY ALLOWED TO RETURN WITH A NEGATIVE COVID TEST RESULT AND MANDATED QUARANTINE TIME HAS BEEN COMPLETED ü MINIMUM QUARATINE TIME MANDATED BY NYC STATE IS 10 DAYS AFTER FIRST SIGN OF SYMPTOMS/INFECTION ü WE WILL TRACE EVERYONE WITH WHOM THAT EMPLOYEE CAME TO CONTACT WITH AND LET THEM KNOW ü WE WILL TRACE ALL AREAS THAT THE INFECTED PERSON OCCUPIED & SANITIZE
