The decision to include a patient should be based on the following 3 criteria being met: 1. Trauma patients: Irrespective of age* *Military personnel injured on active duty are excluded *Exclude iatrogenic injuries i.e. rib fractures caused by CPR 2. Who fulfil one of the following length of stay criteria: TA In hospital for >3 overnight stays Admitted to a Critical care area (regardless of LOS) Transferred out for specialist care or repatriation* (total LOS >3 overnight stays) R Transferred in for specialist care or repatriation* (total LOS >3 overnight stays) Deaths (including deaths in ED, even if COD is medical) N *Patients admitted under care of Rehabilitation team only: Not included in TARN 3. AND whose isolated injuries meet one of the following criteria>>>
HEAD Injuries Include Exclude (in isolation) TA Skull fracture Traumatic haemorrhage Scalp injuries Spontaneous haemorrhage (stroke, aneurysm ) Brain Contusion R Brain Laceration/penetration Loss of consciousness Traumatic Brain swelling Vascular injury N Nerve injury Diffuse Axonal Injury (DAI)
FACIAL Injuries Include Exclude (in isolation) Unstable fracture Minor skin injuries TA (usually fixed in theatre) Orbital Blow out fracture Le fort fracture (I, II or III) Closed/stable fracture/s (even if multiple) All other injuries to eye Pan-facial fracture Injury to branches of external carotid R All injuries to ear artery Optic/ Facial nerve injury Eye Avulsion N Traumatic Retinal detachment Globe rupture
NECK Injuries TA Include Injury to major vessels: Carotid and Exclude (in isolation) Minor skin injuries Vertebral arteries, Jugular vein Organ injury Hyoid fracture R Nerve injury N
THORACIC Injuries Include Exclude (in isolation) Vagus Nerve Injury Minor skin injuries TA Injury to major vessels Organ injury Traumatic Sternum fracture Traumatic Rib fracture/s R Flail chest (unstable chest wall) Haemothorax (blood in thorax) Pneumothorax (air in thorax) Haemomediastinum (blood in mediastinum) N Pneumomediastinum (air in mediastinum)
ABDOMINAL Injuries TA Include Exclude (in isolation) Vagus Nerve Injury Injury to major vessels R Minor skin injuries Organ injury (including genitourinary organs) (contusion, laceration, transection, avulsion, perforation, rupture) N Retroperitoneal haemorrhage
SPINAL Injuries TA Include Exclude (in isolation) Traumatic Vertebral fractureR Spinal strain (whiplash) Vertebral dislocation Disc injury Nerve root injury N Ligament Brachial Plexus injury Cord injury
FEMORAL Injuries TA Include Hip fractures Aged 65 years old Hip fractures aged >65yrs old Neck of Femur, Intertrochanteric, R Pertrochanteric or Greater trochanteric, basi-cervical, intracapsular, subcapital Head, Shaft, Distal or Subtrochanteric #: Any age Femoral Vessel injury N Femoral Nerve injury
PELVIS/ACETABULUM Fractures Pubic Rami Include Exclude >65 years old Fracture(s) of a single pubic rami
UPPER LIMB or LOWER LEG Injuries Excluding hands & feet Upper Limb: Scapula, Clavicle, Humerus, Radius, Ulna Lower Limb: Patella, Tibia, Fibula, Calcaneus, Talus Include Exclude (even if multiple) TA Open (compound) fractures &/ or dislocations Total Crush injury Closed fractures &/or dislocations of 1 limb (even if multiple) All other Nerve injuries Traumatic Amputation (incl. partial) Muscle injury Fractures &/or dislocations of multiple limbs R Tendon injury Transected vessels Sciatic Nerve injury N Ligament injury Sprain Minor skin injuries Crush = Total destruction of bones, vessels/nerves & soft tissue
HANDS & FEET Injuries Include Exclude (even if multiple) Crush of entire Hand Hand or Foot fracture/s (including carpals, metacarpals and phalanges) TA Crush of entire Foot (including tarsals, metatarsals and phalanges) Amputation: entire Hand or entire All injuries to digits (fingers & toes) (including crush and amputation) Foot R N Crush = Total destruction of bones, vessels/nerves & soft tissue
BURN or INHALATION Injuries Include Exclude (in isolation) Full thickness burn (any %) Any burn (Not submitted to IBID) (Submitted to IBID) >10% TBS burn Inhalation injury (Not submitted to IBID) (Submitted to IBID) Inhalation injury
OTHER INJURIES Include Exclude (in isolation) Electrocution injuries Bruises Full thickness Frostbite Abrasions Asphyxia (hanging, strangulation, choking- Minor Skin lacerations patient must lose consciousness) TA Drowning (patient must lose consciousness) Skin lacerations: Blood loss >20% Penetrating injuries to skin: Blood loss >20% Minor Penetrating Injuries to skin Hypothermia Major Degloving injury R Blood loss can be determined by using the volume of blood given as a proxy for the volume N of blood lost- 3 units of blood = more than 20%/ 1 litre All injuries must be documented in the submission, even if those not eligible for TARN in isolation
Inclusion Criteria: Quiz 1. Should you include? Answer: No 50 year old Fell Injuries must meet criteria, regardless of outcome Injuries: Closed fractured tibia & fibula on left Died after 2 days 2. Should you include? 67 year old Tripped In hospital for 4 days TA Answer: Yes 65+ Neck of Femur # with any other (non Hand or Foot) fracture is included. Injuries: Right # Neck of Femur & Right Radius # R 3. Should you include? 68 year old RTA Fractured Pelvis & Femur N Answer: Yes (if MTC) as patient received acute care. No: If Trauma Unit Patients admitted for rehab only are not included in TARN. Treated in MTC for a week then admitted to local Trauma Unit for rehabilitation only .

Collapsed with possible stroke or traumatic Any ambiguity as to Traumatic or Spontaneous - Subarachnoid haemorrhage exclude 5. Should you include? TA Answer: Yes as injury is TARN eligible with medical condition. R 45 year old admitted to hospital on 20th February Date of Arrival & Date of incident should be the date the Trauma occurred in hospital (1st March), not the Fell on Ward on 1st March & sustained Fracture to skull Discharged on 10th March N original medical admission date.
