Portage County Human Resources 1462 Strongs Avenue Stevens Point, WI 54481 Phone (715) 346-1327 Fax (715) 343-6274 Memorandum To: Portage County Human Resources Committee From: Laura Belanger Tess, Human Resources Director Date: February 4, 2021 Re: 2021 Wellness Incentive Check-In On January 26, 2021 Portage County, Willis Towers Watson and WellRight had a 2021 Wellness Incentive program check-in meeting. Some of the items discussed included the following: 1. 2021 On-site Biometrics – the County’s target date to host on-site biometrics with this program is April/May. For 2020 that obviously did not happen, and it was discussed, and decided to not even offer on-site biometrics later in the year due to COVID. The question was asked if Portage County intends to hold on-site biometrics for 2021. With COVID still on-going we discussed not holding on-sites again in 2021. I discussed with County Executive Holman and Director Przybelski and they both agreed. Employees can download a Physician’s form and submit their biometrics to WellRight via that form. 2. Challenge Dashboard – attached is a copy of the Challenge Dashboard year to date. 3. Review Grow a Garden Challenge – attached is a copy of the ‘2021 Portage County Wellness Incentive Program’ that includes a description of all the activities and challenges. At our check-in meeting we reviewed some of the submissions that were submitted for some of the new challenges such as Blood Drive, CSA, Grow a Garden and Nutrition Class. Attached is the summary for ‘Grow a Garden’ challenge on the website. In comparing that to what was discussed during Committee meetings and as is listed on the ‘2021 Portage County Wellness Incentive Program’ we wanted to have a discussion in regard to the intent of the Grow a Garden challenge. Portage County Wellness Incentive Program 01/01/2021 – 12/31/2021 Portage County WellBeing: The Goal WellBeing is more than just staying in shape—it means paying attention to nutrition, understanding the importance of personal relationships, keeping good financial practices, appreciating the value of your work, and recognizing your purpose. Portage County employees, and spouses, who are enrolled in the health insurance plan are eligible to utilize the Portage County Wellness Incentive Program – a collection of activities to help you form healthy, holistic habits through WellRight. Enrolled employees are eligible to earn incentives. As you participate in various activities online or through the mobile app, your progress gets tracked and the system, will display milestones you complete. Once you have accumulated enough points, you can redeem them for rewards. Rewards are paid out the second paycheck of the month following the month the reward is redeemed at WellRight. Levels Points Rewards Diamond 510 $600 Platinum 408 $480 Gold 306 $360 Silver 204 $240 Bronze 102 $120 Annual Activities Points Age Gage 100 Complete the health risk assessment at WellRight Measure Up Complete your biometric screening. Submit your physicians form to WellRight if completed 100 with a physician. Biometric Screening submissions must be received by December 15, 2021 Preventative Exam Complete your annual exam or age-appropriate screening. Submit your proof of visit (visit summary, EOB, etc.) to WellRight 100 Screening Examples: Primary Care Visit, Women's Visit, Colonoscopy, Mammogram, Prostate Exam, etc. Please do not submit any Personal Health Information. Open Wide Complete at least one dental preventive exam per year, then submit your physician form 100 and/or other form of proof (visit summary, EOB, etc.) to WellRight
Wellness Challenges Points Move It Goal: 30 minutes of physical activity (any type) 100 days/year 1 point per day/100 Sync to your fitness tracker or log the information at WellRight Monthly Weigh 5 points/ 60 total Weigh yourself monthly County U 10 points each/60 total Complete 6 University Courses Blood Pressure Check 1 point each/60 total Log at WellRight you completed a Blood Pressure Check up to 60 days/year Donate Blood 20 each/40 total Submit a comment stating the date and location of donation. Up two submissions. 20 each Cooking Class/Weight Management- up to 2 for 25 points each. Attend a Virtual classes or in person class 25 each/50 total Upload a comment stating the name of class or program and date you attended the class. Join a Consumer Supported Agriculture (CSA) program 50 points Upload a photo of your CSA receipt or box of CSA food Grow a garden 50 points Upload a photo of your garden, at any stage from Planting to Bloom to Harvest County Challenges Points Team Challenge Event announced in April. 50 Winning team receives an additional 100 points Personal Challenges Points Participate in a 30-day habit-based personal challenge to continue your Personal NA Wellness/Wellbeing Journey Activate your Portage County Wellbeing Account To set up your Portage County Wellbeing account, click the Activation button in the email from noreply@wellright.com with the subject line, Portage County Wellbeing Registration–Sponsored by Portage County. Grow a Garden January 01, 2021 - December 31, 2021 The Grow a Garden Challenge invites you to grow a plant or garden. You can grow one that is large or small, outdoors or indoors, in the 330 days remaining Submit quit challenge ground or in a pot. If gardening is new to you, ask a friend or family member for help. Gardening is pretty simple—you’ll catch on quickly and will enjoy the process.
