FHS CHARACTER Scholarship Application 2019 Dear Student: The Frenship Foundation for Leadership has granted over $708,600.00 to 276 Frenship High School graduates since 2003. The purpose of The Foundation’s scholarship is to recognize consistent effort, to promote leadership, and to develop character in students of Frenship schools, while stimulating a desire to serve others and further their education. At FHS, a scholarship recipient selection is based on the following criteria: 1. Character: Promptly fulfilling individual pledges and responsibilities; demonstrating high standards of honesty, fairness and tolerance; cooperating with rules and regulations; upholding principles of morality and ethics. 2. Leadership: Evidence of participation and/or holding offices in extra-curricular activities (in/out of the school environment), positive influence on friends and classmates; assuming responsibility for tasks. 3. Service/Activity: Evidence of serving at school, in the community, through recreation activities, employment, and with your family and friends. 4. Scholarship/Effort: Attendance, behavior and GPA are reported from the counselor’s office. In order to evaluate students for selection, the Foundation for Leadership Committee is asking that you complete the enclosed Student Information Forms. The forms will be critical in helping the committee make the most objective decisions possible. All scoring is done blindly. All names are redacted, and replaced with an identification number for applicants prior to being scored by the judges. Leadership is also a criterion by which the committee will evaluate students for selection. You will need to develop a leadership statement in which you discuss your role as a leader or the role leadership plays in your life as a FHS student. The school’s guidance office will provide the committee with the credits, attendance & behavior information. In order to establish minimum definitive objective criteria for determining a student’s cooperation with rules and regulations, the FFL Board has determined that students who have engaged in the following shall not be considered for a character-based scholarship: Please initial in each line. Your initials state that you HAVE NOT been guilty of the offense in the statement. ____ Pled guilty or no contest/nolo contender to, been adjudicated for, or received probation, deferred adjudication, or any other type of deferred prosecution for delinquent conduct or conduct indicating a need for supervision under the Texas Juvenile Justice Code; ____ Pled guilty or no contest/nolo contender to, been convicted of, or received probation, deferred adjudication or other type of deferred prosecution for a felony or misdemeanor offense (other than minor traffic violations) in the adult justice system; or ____ Been removed from participation in extracurricular activities or received some other regulatory sanction as a result of anabolic steroid testing conducted by the UIL. By completing and returning the attached application, you are acknowledging that you have not engaged in any of the above activities. Moreover, you agree that, should you engage in any such activity after the date of submission of your application and your graduation from FISD, you will withdraw your character-based scholarship application and/or return any scholarship funds delivered to you. The Foundation for Leadership Selection Committee, made up of selected FFL Board of Directors, will meet to screen all eligible students’ applications. We congratulate you on your achievements and are looking forward to receiving your Student Information Forms. Scholarships will be distributed upon proof of enrollment in a trade school or higher education institution, within 5 years of graduation from FHS. INSTRUCTIONS 1. Establish a personal ID number. It will be the first letter of your last name and the last 4 digits of your social security number. Ex. – My ID number would be S1234 because my last name is Stephens. 2. Follow ALL directions exactly as described in the following. There are 4 parts to this application: your application, teacher recommendation, letter of recommendation and office information. All will be digitally submitted by person completing the form, EXCEPT the office form. It must be printed, signed by application and given to counselor. The counselor will turn form in to Mrs. Stephens. 3. Applicants should complete and submit application to Cindy Stephens at cstephens@frenship.us. The application is due no later than MARCH 5, 2019 by the end of the school day. Please check for spelling and grammar mistakes before submitting. 4. *Please ask ONE teacher to fill out the Character Scholarship Recommendation Form. It must be a current teacher at the high school. The teacher will send the filled-out form directly to cstephens@frenship.us. The teacher will need to submit form by March 5, 2019. It is your responsibility to check back with that teacher and ask if they have returned the form to cstephens@frenship.us via email. You will have to give the teacher your ID# for this form. 5. You will also need another Letter of Recommendation. This recommendation letter is not required to be from a teacher but may be from someone outside of the school system. Please do not ask the same teacher for both recommendations. This letter of recommendation will be emailed to: cstephens@frenship.us. The letter needs to come from the person writing the recommendation and the subject line should contain your ID#. This letter of recommendation must be received no later than March 5, 2019. 6. *Your FHS class counselor will fill out the office information page. You will need to print and sign this page and turn in to your counselor. They will turn it in to Mrs. Smallwood or send to cstephens@frenship.us. 7. Page 3 is your Leadership Essay. Your essay MUST BE typed and no more than 1000 words. You MUST include your word count. Essays over 1000 words and/or without a word count not included WILL NOT be scored. 8. Page 4 is your Service & Character Involvement. You will only be allowed 5 so choose carefully. 9. Page 5 is a checklist that must be completed and turned in with your application. 10. Page 6 is a scoring sheet for the Selection Committee. It is included for you to have a better understanding of the evaluation process. 11. Complete all lines in the box below. (please print) 12. The Frenship Foundation for Leadership Selection Committee will evaluate your essay. The committee will consider content, clarity, mechanics, and structure. Your response MUST be typed. If it is not typed and the word count included, your essay will not be scored.
