Welcome the Mississauga (Head Office) 2018 Summer/ Fall Calendar! Welcome to Mind Forward Brain Injury Services! Our new name reflects a forward-looking vision of support for our clients, caregivers and family members, as well as our community partners. We know that a person continues to grow and develop as a person, through a network of support! Registration, information and contacts are all identified in this calendar. If in doubt, please contact me. For those wishing to take part in our Psychosocial Groups, please make sure you contact your Case Manager/ Team Leader for more information on how to register. If you are a returning member to our groups, welcome back! I hope you find our offerings enriching, engaging and of value to you. Registration How to Register Beginning Thursday June 14th, please bring in your completed registration form with cash or a cheque to our Head Office at 176 Robert Speck, where our Day Services Leader will assist you. If you require additional support with this process you may also consult your Case Manager or seek assistance from a Day Services Team Member. You can also register on-line at www.phdabis.org on the ‘What’s Happening’ page . Please note that all groups are offered on a first come-first serve basis, and the number of participants that can be accommodated is limited for some groups. Payment Process: 1. Write in the cost of the group or activity. 2. Total the fees for groups. 3. Total the fees for special events. 4. Add the two totals together at the bottom of the registration page. Payment Method: 1. We accept payment by cash, or cheque made out to PHDABIS. Please consult with Day Program staff or CM/ Team Leader if you want to use post-dated cheques. Groups & Descriptions MONDAYS Bowling @ Classic Bowl Monday July 9th , 2018 - Monday December 17th, 2018 Fee: $158 Bowling has many physical and cognitive benefits such as muscle toning and strengthening. It improves social skills, reduces stress, and enhances hand / eye coordination. This group will run once a week, which includes two games and shoe rental. (Cost is $4.00 per game x 22 weeks). A pizza party will be held at the end of our session. Woodworking 1 (Mondays) Fee: $65 The same project , a shoe rack repeats Summer and Fall Monday July 9th — Monday September 24th, 2018 SUMMER Monday October 1st — Monday December 17th, 2018 FALL The woodworking program is designed to execute higher functioning skills towards self-rehabilitation in areas of the left and right side of the brain. Each client will be able to develop their own critical thinking, logic, and reasoning skills through guided supervision and step by step instruction to complete the project. BINGO! Monday July 9th, 2018 - Monday Aug 27th, 2018 $30 BINGO is a popular game for many! This group will aid to improve hand-eye coordination, boosts cognitive abilities, and increases socialization. Cookie Making September 10th, 2018 —December 3rd, 2018 No Fee Enjoy making a dessert with others, and having a sample. The cookies will be sold at the Mind Forward Holiday Party in December, as a fundraiser for the Day Program. TUESDAYS Mind Forward Café Tuesday July 3th, 2018– Tuesday December 18th, 2018 Fee: $80 Prepare a healthy meal for yourself and your peers at a very low fee. You must be able to take a leadership role, help to problem-solve with your team, and work with little staff direction. Case Manager referral is required. CAFÉ MEAL Fee: $110 Get your lunches PRE-PAID! 25 weeks for $110 (includes three free lunches. $5.00 per week includes meal and drink. w w w. p h d a b i s . o r g

Games Galore Tuesday July 3rd - Tuesday December 18th, 2018 No Fee Enhance your cognitive skills and engage in a variety of board games through participation in various board games, such as Scrabble, Scattergories, and Monopoly. Improve attention; information processing; reading; decision making; planning and strategizing, all while socializing with others! Cheap Date Movie Night @ Cineplex Cinema Mississauga Tuesday July 3rd - Tuesday December 18th, 2018 Pay as you go Enjoy a night at the movies with friends. The cost of the movie is reduced with an “Access 2 Entertainment” card. A Scene card will get you a 10% discount on concession stand items. WEDNESDAYS Pranayam YOGA Wednesday July 4th - Wednesday December 19th, 2018 No Fee Pranayam is a practice of yoga which includes physical postures and exercises, breathing and relaxation techniques. Ultimately it enhances emotional regulation, well-being, life satisfaction, and spirituality. Woodworking 2 (Wednesdays) Fee: $65 Wednesday July 4th – Wednesday September 26th, 2018 SUMMER Wednesday October 3rd – Wednesday December 19th, 2018 FALL See the Woodworking group description under Mondays. The project is a shoe rack. Book Club Wednesday July 4th - Wednesday December 19, 2018 Fee: $40 (two books) Join this group to explore two works of literature. Participants will engage in an open discussion about the book in relation to life experiences. This group will help to increase communication skills (listening, reading and writing) as well as socialization and interaction skills. The fee covers the cost of the books. In-House Movie Matinee Wednesday July 4th - Wednesday December 19th , 2018 Fee: $10. Come on by to catch films on our big screen TV! Page 7 Groups and Descriptions THURSDAYS Aphasia Communication Group Thursday July 5th - Thursday December 20th Run in partnership with the Halton Aphasia Centre (HAC), the group is facilitated by registered Speech & Language Pathologists (SLP). The group focuses on teaching new skills and support strategies for anyone who has difficulty communicating and expressing themselves. This group is by referral only. Please contact your Case Manager or Team Leader if you are interested. Jumpstart / Sunshine Stroll Thursday July 5th - Thursday December 13th, 2018 No Fee This ever popular group includes some warm-up exercises, stretching, strength training, and light cardio. The group ends with a short discussion on hot topics in today’s latest headlines. Artistic Creations Thursday July 5—September 27, 2018 Fee: $40 Explore a variety of imaginative artistic activities, including painting, sculpture, crafts, doodling, special occasion cards, and other projects. You will improve your creativity, cognition, and hand-eye coordination. Sports Talk Wednesday October 4th - Thursday December 13th, 2018 No Fee This group is designed for participants who enjoy playing, watching, and following sports. This will aid in enhancing social skills and will promote knowledge and discussion of current popular sports events. Script Writing (Currently Running) Thursday May 24 - August 30th, 2018 No Fee Script Writing will allow participants to explore and learn writing skills. This group will help enhance grammar skills, creative writing, communication as well as the visualization of the script coming into practice. We will prepare a script for the Drama Club. Drama Club Thursday September 6th - Thursday December 6, 2018 No Fee This fun-filled group will help you enhance your communication skills, self-confidence, imagination, and help you grow an appreciation for culture and arts. Cards and Pool Thursday July 5th - Thursday December 13, 2018 Fee: $25 This group will include a variety of card games and billiards If you are looking to enhance social skills, cognitive stimulation, hand and eye coordination, language skills and problem solving skills, then this group is for you! w w w. p h d a b i s . o r g

It promotes psychological well being, as well as boosting immunity for participants by exposure to healthy bacteria. Breakfast Club Friday September 14— Friday December 14, 2018 Fee: $60 Enhance your knowledge of recipes, kitchen safety, and cleanliness. This group will assist with improving your fine motor skills and communication. Maximum of 10 participants. Music and Me Friday July 6 - Friday December 14, 2018 No Fee This musical group allows members to sing and share meaningful pieces of music. The group members will showcase their abilities during the annual Mind Forward Holiday Social, Family Picnic and other Mind Forward social events. Karaoke Friday July 6 - Friday December 14, 2018 2018 No Fee This group will help participants improve memory, relieve stress, and boost self confidence. It also enhances creativity and promotes brain growth by exercising both sides of the brain. Come and sing with us! Dinner and a Show! Friday July 6 - Friday December 14, 2018 Fee: $168 Start your weekend with a well priced and nutritious home-style cooked meal, including a low fat dessert (costs average $7 per dinner). There will be screening of the latest release or a classic flick. Dinner and a Show is a long-time favorite and we can’t wait for you to join us! Maximum of 21 participants.
