The Alzheimer Society in Brant serves the communities of Paris, Burford, Mt.Pleasant, St. George, Scotland, Oakland Onondaga and Six Nations. The combined population of our service area is 134,000 of whom more than 2400 experience some form of dementia and another 6,000 are caring for them. In 2010 we integrated with the Societies in Brant and Haldimand Norfolk to create operational efficiencies and share best practises.

Partnerships in the Community Long Term Care and retirement homes generously provide space so we can offer education sessions throughout our large catchment area: • John Noble Home Day and Stay • Brant County Public Libraries • Lifegaurd Home Care • Adult Recreation • Tranquility Place Therapy Centre • Fairview United Church • Lorne Towers • Sherwood Restaurant • Six Nations Health • Parklane Terrace Long Term Services Care • Paris Seniors Club
Mentoring Tomorrow’s Health Care Providers In partnership with Grand Erie Learning Alternatives , Medix and Laurentian University the Society provides teaching and hands on training to many allied health students in our region. • Our educator trains students using the Dementia Experience tour. The tour gives them a deeper understanding of what it would be like being a person with dementia. • A recent graduate is currently employed by the Alzheimer Society. Many others have gone on to have a career in health care that specializes in dementia.

Caring for our Community in Brant We are the First Link™ to information and support for individuals and families at the time of diagnosis and throughout the course of the disease. Persons with memory loss or any form of dementia, their families and other caregivers access our education services : 3021 Education 547 Hours of education/year Participants/year 289 Education Sessions/year 7000 Kilometers Travelled by Educators/year
Caring for our Community in Brant The Social Work Program has been developed to meet the needs of people in our community whose lives have been affected by Alzheimer’s disease and related dementias. Our registered social workers provide office or in-home assessments. The Society is actively supporting: 250 persons with dementia/year 185 Home Visits/year 700 Caregivers/year 1225 Counselling Hours/year 800 New Referrals/year 7,000 Kilometers Travelled/year
Caring for our Community The Health and Wellness Program We know that regular exercise and opportunities for social engagement and respite benefit both the caregiver and the person with dementia. This is achieved through programs such as: • Minds in Motion - 100 % donor funded • Creative Expressions -100 % donor funded • Music for Memories Program- 100 % donor funded • Sit to be Fit Strengthen Your Mind - demand for additional sessions • Let’s Get Together – demand for sessions

What we know… In Brant and Brant County it is estimated that almost 2500 people live with dementia and another 6,000 care for them. At the Alzheimer Society, people with dementia are at the centre of everything we do – they are who we remember, honour and support each day – and as more and more people are diagnosed with this disease, the demand for our services is growing… By 2021, 10% of our neighbours over 65+ will be living with dementia.
The Challenge… For more than 33 years, the Alzheimer Society has provided vital support to people diagnosed with dementia and their caregivers. In 2016, our Society received more than 800 new referrals to its programs and services in with no significant increase in funding for educational and counselling staff resources to urgently help these individuals and their families. There has been no increase in government funding for these programs since 2011. The Ministry of Health and Long Term Care provides 80% of our funding and we recognize that the government can’t be the only answer to meet the rapidly growing needs of our aging population….
Our Plans To Meet The Need… IN-HOME EDUCATION SESSIONS Many of our clients and caregivers cannot access our offices to receive vital support and education because: Government Community Funded Need • Persons with dementia can be overwhelmed /confused when leaving their surroundings. • The caregiver will not leave the person with dementia home alone. The Society currently staffs • They don’t have a license/don’t drive. 1 Educator. 2 Educators are needed to meet the demand. Our plan is to provide an additional 500 hours of in-home education sessions a year for families who need these urgent supports.
Our Plans To Meet The Need… COUSELLING We are seeing increasingly complex family dynamics and situations in those we serve, therefore increasing the need for more in-home visits, working with all those involved to develop an individualized response to help people remain in their homes as long as possible. Often the only support a family has is an Alzheimer Society counsellor. Government Donor Community Our plan is to provide an additional Funded Funded Need 500 hours each year of more intensive supports. The Society currently staffs 2.5 Counsellors. 4 Counsellors are needed to meet the demand.
Our Plans To Meet The Need… HEALTH AND WELLNESS Increased funding for staffing, travel and supplies will allow us to improve access to Health and Wellness programs on an ongoing basis . This is so the rapidly growing number of people living with dementia and their caregivers can access these new and innovative programs that increase their quality of life . We will also increase the use of volunteers to assist staff in the delivery of programming. Health & Wellness 100% Donor Funded
Our Fundraising Plans to Meet the Need Education: $200,000 An additional $40,000 for the next 5 years to eliminate wait times by providing an additional 500 hours of in- house education each year. Support & Counselling: $200,000 An additional $40,000 for the next 5 years to provide an additional 500 hours each year of more intensive supports so clients can remain at home as long as possible. Health & Wellbeing: $100,000 An additional $20,000 for the next 5 years to expand activity programs for persons with dementia and their caregivers. Campaign Goal: $500,000
