Application Steps
STEP 1 Assemble all required documents in the exact order indicated below using the following checklist:
completed and signed application form (p. 1-15) with a passport size photo
official transcripts and copies of all diplomas (follow the requirements on page 4)
Submit TWO letters of recommendation (follow the requirements on page 12)
