You can choose up to 2 habits to practice at a time. Choose From: • Meditate • Breathe • Read • Practice Gratitude • Power Down • Stretch • …and more! Beginning in January 2022, Healthy Habit logs are worth 5 points each. 9

UNUM Voluntary Benefits Critical Illness Insurance – Supplements your medical coverage – Lump sum payment between $10,000 to $30,000 for diagnosis of cancer, heart attack, stroke, major organ failure, and kidney failure – Includes a $75 wellness benefit Accident Insurance – Supplements your medical coverage – Pays specific benefits for expenses that result from non-work-related injuries or accidents Whole Life Insurance – Supplemental life insurance that builds cash value – Rate is guaranteed to never increase – Long Term Care Policy Coverage in these supplemental benefits is portable, meaning if you leave the company, you may take them with you. 16

Important Info: Cards You’ll get a new card if… • You’re a new member to Collective Health • If you’ve changed your plan at all this year • If you enroll in the HSA or HRA for the first time • If you don’t make any changes to your medical, your same cards work for next year, you won’t get any cards. 25
