SMFCSD New Student Registration 2021-2022 Welcome to the San Mateo-Foster City School District (SMFCSD)! We are excited to welcome you to our schools and help guide you through the process of registering your child. KINDERGARTEN/TRANSITIONAL KINDERGARTEN (K/TK) REGISTRATION AGE AND GRADE LEVEL ELIGIBILITY • Kindergarten: Children entering Kindergarten must be 5 years old by September 1, 2021. • Transitional Kindergarten (TK): Children entering Transitional Kindergarten must be 5 years old on or between September 2, 2021 – December 2, 2021. • Students who do not turn 5 by December 2, 2021 are not eligible to enroll in the SMFCSD for the 2021 – 2022 school year. THE STEPS TO REGISTER ARE: STEP 1: Determine your child’s school of assignment based on your address of residence • To find your child’s school of assignment: www.smfcsd.net > Registration & Enrollment > School Assignments STEP 2: Collect all required registration documents (see next page for complete list) • Required registration documents must be complete and up-to-date in order to be accepted by the school office. STEP 3: Complete the Aeries Online New Student Pre-Registration Form • To find the form: www.smfcsd.net > Registration and Enrollment > How to Register for the 2021 – 2022 School Year > Click on the 2021 – 2022 Pre-Registration button STEP 4: Submit all required registration documents (document upload in Aeries online pre-registration form or email to the school of assignment) to complete the registration process. • Submit the required registration documents via the document upload available in the Aeries online pre-registration form. Alternatively, you may email (scan or photo) the required registration documents to the office staff of the school of assignment. • The office staff of the school of assignment will review and verify the required registration documents. • The Priority Registration window will open on January 11, 2021 at 12:00 pm (noon) and close on Fri- day, January 29, 2021 at 12:00 pm (noon). •School sites will continue to accept new student registrations after Priority Registration has ended on January 29. Those registrations are not considered to have priority. PLEASE NOTE • Registration is considered complete only after you have completed document verification in Step #4. •Step #4 must be completed by 12:00 pm (noon) on Friday, January 29 in order to be considered within priority. • The Aeries online pre-registration form can be completed on any internet-enabled device (cell phone, tablet, laptop, computer). • If your school of assignment does not have a TK program, your child will be placed at a school that does for their TK year and then return to their school of assignment for Kindergarten the following school year. 4
REQUIRED REGISTRATION DOCUMENTS The following documents and forms are required to register in the San Mateo-Foster City School District. 1. Proof of Student’s Age: Student’s certified birth certificate, passport, duly attested baptismal certificate, certified statement by the local registrar or county recorder certifying the date of birth. When none of the foregoing documents is obtainable, an affidavit of the parent/guardian. 2. Child’s original, up-to-date immunization record 3. Child’s original Report of Health Examination for School Entry (encouraged at Kindergarten, but not required until 1st grade) 4. Parent/Guardian current photo identification: current California State Driver License, current California State ID Card or Valid Passport or Consulate Issued Picture ID, or other photo identification. 5. NEW STUDENT TRANSFER REQUEST PROCESS • All families wishing to request a transfer MUST FIRST COMPLETE the New Student Registration process. • The online transfer request form will become available on the District website at 12:00 pm (noon) on Monday, January 11, 2021. • Parents/guardians may request up to two (2) schools of choice. • All transfer requests received by the deadline of 12:00 noon on Friday, January 29, 2021 will be entered into the school/program lottery. • Transfer request status emails will be sent out by the end of the business day on Friday, February 26, 2021. • Transfer requests received after the lottery deadline will automatically be added to the transfer request list in the order received. • Please note: in order for the request to be processed, you will need to enter your child’s enrollment ID number found on the completed Aeries online pre-registration form. A transfer request will not be pro- cessed for a new student if the new student registration process has not been completed at the school of assignment. • Plan to attend the school of assignment unless you receive communication that the transfer has been granted. • Transfer requests are granted only if there is room in the school and grade level requested. • A transfer request is not a school registration and is not a guarantee of enrollment. • All transfer requests expire the day before the first day of school for the 2021- 2022 school year. Transfers will not be granted after that day. 5
PARENTS’ GUIDE TO IMMUNIZATIONS R EQ UIR E D FO R S CH OOL E N T RY Starting July 1, 2019 Students Admitted at TK/K-12 Need: • Diphtheria, Tetanus, and Pertussis (DTaP, DTP, Tdap, or Td) — 5 doses (4 doses OK if one was given on or after 4th birthday. 3 doses OK if one was given on or after 7th birthday.) For 7th-12th graders, at least 1 dose of pertussis-containing vaccine is required on or after 7th birthday. • Polio (OPV or IPV) — 4 doses (3 doses OK if one was given on or after 4th birthday) • Hepatitis B — 3 doses (Not required for 7th grade entry) • Measles, Mumps, and Rubella (MMR) — 2 doses (Both given on or after 1st birthday) • Varicella (Chickenpox) — 2 doses These immunization requirements apply to new admissions and transfers for all grades, including transitional kindergarten. Students Starting 7th Grade Need: • Tetanus, Diphtheria, Pertussis (Tdap) —1 dose (Whooping cough booster usually given at 11 years and up) • Varicella (Chickenpox) — 2 doses (Usually given at ages 12 months and 4-6 years) In addition, the TK/K-12 immunization requirements apply to 7th graders who: • previously had a valid personal beliefs exemption filed before 2016 upon entry between TK/Kindergarten and 6th grade • are new admissions Records: California schools are required to check immunization records for all new student admissions at TK/Kindergarten through 12th grade and all students advancing to 7th grade before entry.
