Paper copies of W-2 Forms are mailed to all employees except those who chose electronic-only delivery. Everyone may access tax forms through MyUW. For security reasons, the social security number isn’t printed on the MyUW W-2, but it is printed on the one that was mailed out. To access your W-2 in MyUW: • Go to my.wisc.edu and log in with your NetID and Password • Click the Tax Statements icon on the Payroll Information tile • Click 2018 and later in the W-2 Forms row on the ViewW-2/W-2c Forms page • Click the View Form button • You can view or print your tax form. Since a printed copy of your W-2 forms includes personal tax information, use extreme caution when printing this document in a ‘public’ place such as your work environment. Be sure to sign out when you’re finished. Resources: Visit Tax Statement Help (https://uwservice.wisconsin.edu/help/payroll.php#tax-help) for resources regarding the various tax forms. Filing Taxes: Employees should use their W-2 Form, not their final 2021 earnings statement, to file taxes. Title & Total Compensation Project (TTC) TTC Appeals Extended to February 4th – Employees who believe their TTC position description does not match their current duties can now submit a formal appeal anytime through February 4, 2022. The appeal should be submitted through the electronic appeal system. The previous deadline of December 31st has been extended to provide employees with more time. Informal resolutions are due by Friday, January 28, 2022. If you prefer to file your appeal on paper, a paper appeal form (including translated versions) is available online at https://hr.wisc.edu/title-and-total-compensation-study/appeals/. The links are on the right side of the screen. Some paper versions will also be sent to break spaces for employees who are not regular computer users. Employees are also encouraged to work with their supervisors to try and resolve any questions or concerns before reaching the formal appeal process. COVID-19 COVID-19 Town Hall: January 21 at 12:00pm – UW-Madison experts will answer questions about the spring semester and how the university plans to navigate the continuing COVID-19 situation in a town hall event this Friday, January 21. Visit UW’s Guide to Testing website for help determing your next steps for testing and which testing option (PCR or Antigen) may be best for your situation: https://covidresponse.wisc.edu/a-guide-to-testing/ PCR Testing • If you have not provided proof of full vaccination, you must complete a PCR test at least once per week • Located at the University Club (803 State St. – intersection of State Street Mall and East Campus Mall) • Testing available by appointment, Monday – Thursday, 8:30am – 4:00pm (closed from noon – 1pm for lunch), and Friday 8:30am – 12:30pm. Antigen Testing • Beginning January 25th UW-Madison will begin distribution of take-home antigen tests to staff. • Starting January 25th, UW-Madison staff may pick up one antigen test per week from Memorial Union or Union South with a valid Wiscard on weekdays between the hours of 11:00am to 6:00pm. • Pickup locations may change after February 4th – watch for more information: https://covidresponse.wisc.edu/testing/#which-test-and-results Positive COVID Tests / Exposure to COVID-19 – If you test positive or are exposed to someone who tests positive for COVID-19, please follow the advice of your healthcare provider and/or University Health Service. More information can also be found on UW-Madison’s Covid Response website at https://covidresponse.wisc.edu/ Employees are not required to report a COVID positive test result or potential exposure/close contact, to their supervisor or HR. Mask Mandate Extension – UW-Madison is extending the indoor mask mandate through March 1st. Contact Tracing Updates – UHS continues to conduct contact tracing for students and employees but there are some delays given the increase in cases. Off-campus contact tracing is also experiencing delays. Everyone who tests positive on campus will receive an email to their wisc.edu account that includes instructions for isolation. Public Health Madison & Dane County advises informing your close contacts immediately if you test positive. COVID-19 Childcare/Caregiving Flexibilities & Leave Usage – On December 31st the Chancellor’s Office sent an email to all staff recognizing the childcare/caregiver needs that some employees may have, particularly in light of the Madison 3
School District delayed the return from winter break/virtual classes. In the email, departments were encouraged to provide workplace flexibility when possible and appropriate. Although the Madison school district returned to in-person instruction on Monday, January 10th, there may still be situations where employees need to serve as a caregiver for COVID-19 reasons. As a reminder, UW-Madison’s Sick Leave Policy has an addendum that is currently in place until May 22, 2022. This addendum allows employees to use sick leave: • If the employee is caring for an individual who is subject to a quarantine or isolation order or has been advised to self-quarantine. o Written documentation of the quarantine or isolation order or written documentation from the individuals’ health care provider advising quarantine is required. • If the employee is caring for their child under 18 years of age if any of the following apply: o The child’s school is closed or participating in virtual or hybrid instruction due to COVID-19. o The child’s childcare facility is closed due to COVID-19. o The child’s childcare provider is unavailable due to COVID-19. o ~For these three reasons, the following documentation is required: ▪ The name(s) and age(s) of child(ren) to be cared for. ▪ For children over the age of 14, a statement that explains the special circumstances requiring the employee to provide care during daylight hours. ▪ Name of the school(s) that is closed or participating in virtual or hybrid instruction, childcare facility that is closed, and/or childcare provider that is unavailable during the period for which the employee is requesting to use sick leave. ▪ Statement from the employee that no other suitable person is available to care for their child(ren) during the period of time that they are using sick leave. Please contact University Housing Human Resources for the form to complete to use sick leave for any of these reasons. Uploading your COVID-19 Booster Shot Record to MyUHS – Employees who have received the COVID-19 vaccine, including a booster shot, can upload their records to MyUHS. COVID-19 Booster & Vaccine Shots – UHS is providing COVID-19 vaccination and booster shots to UW–Madison employees and students ages 18 and older by appointment through the MyUHS app or web portal. Vaccination is free and you do not need to present identification. Note that UHS no longer has the Moderna vaccine available. • UW-Madison COVID FAQ’s: https://covidresponse.wisc.edu/faq/ Testing on Work Time – Employees who are required to test once every 8 days can do so on work time. A reasonable amount of time during their normal on-site work hours, including reasonable travel time from on-site work location to testing site, will be provided. Covid-19 Testing Requirements – Effective August 30th any student or employee that does not have a Covid-19 vaccination on record with University Health Services (UHS) is required to complete a Covid-19 test on campus weekly (every 8 days). This applies to employees who are not vaccinated as well as those who may be fully vaccinated but have not shared their vaccine record with University Health Services (UHS). Fully vaccinated is still considered as two weeks after you receive a one-dose vaccine (Johnson & Johnson) or the second of a two-dose vaccine (Pfizer or Moderna). At this time employees do not need a booster shot to be considered fully vaccinated and exempt from weekly campus COVID-19 testing. The test is a self-administered PCR nasal swab. Employees that are required to test but fail to do so will receive notifications from the UW-Madison campus Office of Human Resources if they are not in compliance. These notifications will come via email and paper letters. Failure to test weekly will result in disciplinary action up to and including termination. • Employees who have an approved remote work agreement and are working 100% remotely are exempt from the testing requirement because they are not working on campus. If you believe you fall into this category you must complete the exemption form that is included in the emails from the UW-Madison campus Office of Human Resources. • If you are required to test and you plan to travel away from Madison for one week or more and are unable to meet your testing requirement, you must complete a temporary exemption form found in the emails you’ve received from the UW-Madison campus Office of Human Resources. • If you are part of this group required to test and test positive for COVID-19, you will be exempt from testing for 90 days from the date your sample was collected at a testing location. If your test was done off-campus, you must submit the positive result via the MyUHS app or website to become exempt. After 90 days, if the campus testing requirement remains in effect, you must resume testing due to your potential to be re-infected with COVID-19. Vaccination provides the longest lasting protection against COVID-19. For more information, please visit the UW-Madison Covid-19 Response website. Miscellaneous Chancellor Search Listening Sessions – The committee leading the search and screen process for a new chancellor will be holding a series of listening sessions. These listening sessions will provide the UW-Madison community and opportunity to provide their thoughts about what an ideal candidate will bring to the position. Join the virtual livestream at https://www.wisconsin.edu/regents/. Those attending virtually are encouraged to enter their comments in the chat box on the streaming page. Note that WebEx registration is required. • A virtual-only listening session for community members and alumni is set for Tuesday, January 25, from 4:30- 5:30 p.m. 5
