PAGE 9 Health Office News Nonmedical Exemption For the 2021-2022 school year: Beginning in the 2021-2022 school year (7/1/2021), the process of submitting a nonmedical exemption will change. There will be two ways to submit a nonmedical exemption. Be sure to check with your student’s school, childcare, or college/university to find out if they need a printed copy of the Certificate of nonmedical exemption or if you submit to CIIS, the school is able to access the Certificate of nonmedical exemption from CIIS. Submit the Certificate of nonmedical exemption WITH a signature from an immunizing provider who is a medical doctor, Doctor of Osteopathic Medicine, advanced practice nurse or delegated physician’s assistant OR Submit the Certificate of nonmedical exemption received upon the completion of CDPHE’s Online Immunization Education Module. • Parents of students in preschool or childcare must submit nonmedical exemptions at 2, 4, 6, 12 and 18 months of age. These exemptions expire when the next vaccines are due or when the child enrolls in kindergarten. • Parents of students in grades K-12 claiming a nonmedical exemption must submit one annually. Nonmedical exemptions expire June 30th each year. If you submit a Certificate of nonmedical exemption on or before June 30th, it will not be valid for the upcoming school year unless you submit the exemption during early registration. https://cdphe.colorado.gov/vaccine-exemptions Medication for 2021- 2022 If medications are needed for 2021-2022 school year, you may obtain new forms from the PSD website. You can click here: Authorization and Release For Administering Medicine to Student at School or School- Sponsored Activity You may also visit PSD visit www.psdschools.org, click on Parents, click on Student Health (scroll down on right side). Scroll down to #6 of the Health Services/Guidelines. You can find forms for the following: • Health Care Action Plan- Basic- (Spanish version- Health Care Action Plan Basic) • Health Care Action Plan- Allergies • Health Care Action Plan- Asthma • Health Care Action Plan- Migraines- (Spanish version- Health Care Action Plan Migraines) • Health Care Action Plan- Seizure Disorder- (Spanish version- Health Care Action Plan Seizure Disorder) Please fill out the appropriate Health Care Action Plan form along with the Authorization to Administer Medication Form. All forms must be signed by the student’s health care provider, legal guardian, and contain clear dosage and administration instructions.
Zach Elementary School ~ Together We Inspire! P A G E 11 5th Grade Eco Experience at Zach Elementary Over the recent years, EcoWeek has become more difficult to plan, facilitate, and execute. There are a variety of factors contributing to this including rising costs, personnel and staffing, and policies to protect risk liability. Each year we struggle to have enough paid staff willing to be away from their families for two nights. We value the long-standing tradition of EcoWeek in PSD, and we also have realized there is room for improvement to this tradition. In 2020, we received new national science standards and we know we can meet these standards in a new and better way. Due to the pandemic, we were unable to host EcoWeek and it prompted PSD elementaries to think outside our traditional box. Beginning in the 2021-22 school year, Zach will move away from an overnight to a daytime eco experience. Welcome to Eco-X!! Our 5th graders will be able to achieve mastery of the science standards through: • A full day experience at CSU Mountain Campus including: • Ropes course • Stream ecology • Short hike • Eco journaling • Activities experience at Zach including: • Learning activities in partnership with the City of Fort Collins, Larimer County, and more! • Possible nature hike in the Fort Collins/Loveland area • A week of Eco-X based activities in Specials’ classes All our students will be able to create their traditional Eco tie-dye shirt! We would love to have parent volunteers to attend all aspects of our new Eco-X! If you are interested, be sure to get registered as a volunteer through PSD. We will send an email once school starts to start organizing volunteer roles. The dates of our Eco-X is still to be determined in our planning process, but it will happen sometime during the first quarter of the school year! P A G E 12 PSD Summer Music Program The Poudre School District is proud to offer an intensive summer music program for the band and orchestra students in PSD. For students just starting their experience, this program is the first step to success – with guidance on everything from opening the case and assembling the instrument to their first performance. For students who already play a band or orchestra instrument, the summer session allows students to continue to refine their technique and continue to bond with their friends and teachers over the summer. It is our sincere hope that we will be able to offer summer music classes during the summer of 2021. Our first priority will be to the safety of the students and staff, so we are asking people to save these dates with the understanding that if Poudre School District or the Larimer Health Department determine it in the students’ best interest to delay or cancel the program. We will make the adjustments and inform parents and students as quickly as we can. As regulations shift, please know, that all requirements regarding numbers of students, distancing and masks will be followed. Summer Beginning Music (for any band or orchestra student who will be starting an instrument in 2021-2022); Monday-Friday, July 19-August 6 Classes are offered in the mornings (beginning classes are 50 minutes; intermediate and advanced classes are 80 minutes) at numerous sites around Fort Collins and Wellington. We anticipate schedules and early registration for summer classes will begin after spring break. Registration information will be distributed by our current music teachers, or 5th grade classroom teachers, and our summer music web-site will go live at that time (https://sites.google.com/a/ psdschools.org/psd-summer-music/). Questions about the summer music program should be directed to the music teacher at the school your child will attend in the fall, or to Scott Schlup (sschlup@psdschools), summer music coordinator.
