Physician and Health Administration Brief: Small and Medium Size Enterprises (SME) Relaunch Grant 1 (current at June 30) Source: https://open.alberta.ca/dataset/6cac661f-5a82-41ce-a42e-15b6efec60d2/resource/498338d2-e6e4-441d-bd3e-2cbc3d35a902/download/jei-small-medium- enterprise-relaunch-grant-guidelines-2020-12.pdf The Small and Medium Enterprise Relaunch Grant offers financial assistance to Alberta businesses, cooperatives, and non-profit organizations that were ordered to close or curtail operations as a result of public health orders, and that experienced a revenue reduction of at least 50 per cent in April and/or May 2020, due to the COVID-19 pandemic. Eligible organizations can apply for 15 per cent of their pre- COVID-19 revenue, up to a maximum of $5,000 in funding. Key Considerations Checklist: Could This Grant Apply to You? This grant may apply to physicians should they meet the primary eligibility requirements. Individual consideration must be given to the business structure in place which may include physician corporation, sole proprietorship, or partnership or other as referenced (p.5), Must have less than 500 employees Must have been carrying on business or eligible to on Feb 29, 2020 Must have been ordered to temporarily close or curtail operations 2 Must have experienced a reduction in revenue of at least 50% in April and/or May 2020 in comparison to April 2019, May 2019 or February 2020 as a result of public health orders. Are open or plan to open with Alberta’s public relaunch Have not received any payments, grants, or amounts from any other sources, including insurance, to replace or compensate for the loss of revenue/earned revenue other than amounts from these government assistance programs: • Alberta Workers’ Compensation Board premium relief • Business Credit Availability Program (BCAP) • Canada Emergency Business Account (CEBA) • Western Economic Development Regional Relief and Recovery Fund • Canada Emergency Wage Subsidy (CEWS) • Canada Emergency Commercial Rent Assistance (CECRA) • Government of Alberta grant relief funding for the child care sector Application Considerations 1. There are specific application windows to handle volume based on geographic location through until July 4th. After July 4 Anyone can apply. (p12) 2. Applications must occur online through My Alberta Digital ID (MADI) which includes a sign-up process. https://account.alberta.ca/ 3. Grants are confirmed by attestation and are subject to Audit 4. The Following document walks throught the process steps to complete the newly released Small and Medium Size Enterprise (SME) Relaunch Grant as launched by the provincial government June 29, 2020. Information you will need: My Alberta Digital ID Account If you are not already signed up, this service allows for participation in provincial programs, grant information access and is intended to expedite and digitize interactions with government programs. This is the only way to access the grant. https://account.alberta.ca/ Alberta Corporate Access Number: (if incorporated) This number is your corporate access number and may be found on your certificate of incorporation or your annual corporate filing documents. Your lawyer will have this number as well. Federal Business Number:This is the number attached to your tax filings with Revenue Canada and will be on your notice of asessment or your accountant or bookkeeper will have it. It will start with RC and often end in 0001. Example: RC 112346789 0001 Revenue Reduction Calculation: (pp 8-9 in PDF guide) A = The intent of this calculation is to estimate what revenue/billing you likely would have achieved if COVID-19 did not occur in order to subtract your actual revenue/billing to record the difference. The difference of the two is used to multiple by 15% to determine grant eligibility. You can only use 1 of the months April or May so to maximize the grant, choose the month which has the largest loss. (with consideration that it needs to be 50% reduction to be eligible) You have a couple of different ways to calculate pre-covid revenue/billing: 1. Use your monthly revenue/billing from April or May 2019, (for example use $20,000 revenue in May 2019) 2. Use revenue/billing from February 2020 B = The intent of this calculation is to determine what your actual revenue/billing for April or May was in 2020 (for example use $10,000 revenue/billing in May 2020) Calculation: 1-(B/A) = % Revenue/Billing Reduction A May 2019 revenue/billing $20,000 B May 2020 revenue/billing $10,000 1 - (10,000/20,000) = 50% This physician is eligible for a grant as revenue/billing has reduced 50% from May 2019 compared to May 2020 Payment Calculation: (pp 8-9 in PDF guide) 3
A = Pre COVID revenue/billing ($20,000 as example above) A x .15 $20,000 x .15 = $3,000 Grant Eligibility (maximum $5,000) Based on this scenario the physician is entitled to $3,000 in grant funds if all other criteria are met. Add the actual business name in the legal name box exactly. This is the name that is used to file taxes etc. Will also ask for permanent address, mailing address, primary contact and organization details. Also willl need Alberta corporate access number, Federal business number, year end date etc. These may be found from accountant and lawyer or by from tax and legal filing information When choosing industry sector choose healthcare and social assistance. 8

Most will not be sole proprietor (if incorporated) so social insurance number likely not required. 11
Enter differences in revenue-you can only choose 1 month; choose month with largest reduction in revenue/billing (minimum 50% to qualify) 12
Enter banking info for direct deposit. Application states funds to be deposited within 10 days. 13
Click through remaining attestation, privacy etc and submit. 14
