Introduction The aim of this document is to provide guidance to GP practices on how to run a database search in EMIS to identify patients who may be suitable for a serial prescription. This document is one of a set of resources created to assist with the identification of patients suitable for serial prescribing. See other related documents below. Related documents SR02: Screening a Database Search in EMIS SR03: Switching a Repeat Prescription to a Serial Prescription in EMIS A serial prescription (SRx) is a prescription for medicines to treat a long-term condition. It looks like any other prescription and normally lasts 56 weeks. The use of serial prescribing is encouraged by the GP contract as it has potential to significantly reduce GP practice repeat prescription workload, both from an administrative and clinical point of view. Pilot work has shown that practices do experience an appreciable reduction in repeat prescription workload. In the long term, it is important that GP practice teams have processes in place to regularly identify, review and change suitable patients from repeat prescribing to Medicines Care and Review (MCR) (previously CMS) serial prescribing. Initially, patients are identified by running a serial prescribing database tool; they are then screened against set criteria for suitability. If approved the patient is provided an SRx. SRx are kept in the pharmacy and dispensed to the patient every four or eight weeks. The Serial Prescription Tool is used to help identify patients who may be suitable for an SRx. The data within the tool is based on an extraction from the GP practice systems (EMIS) and relate to the previous day (or earlier) within the practice. Any work undertaken today will not appear or impact upon the database results until the following day. When refreshing the information, the tool has a large number of complex searches to run in order to generate the information. This means that a refresh can take 10 – 15 minutes to run in a larger GP practice. If there are performance issues (such as a very long refresh time) the tool can be moved to the PC desktop to refresh then back to the practice shared drive once refreshed – how to do this is detailed further on in this document. 2
The following process will provide clear instructions on how to run a database search on EMIS including instructions on how to: 1. Install the MCR Serial Rx Activity EMIS Template 2. Run the search on EMIS 3. Check and install the Scottish Therapeutics Utility (STU) 3
Process Checking and installing the Scottish Therapeutics Utility N.B. - Only install STU if not already installed. Step 1. Log onto the practice computer. Step 2. Double click on the Scottish Therapeutics Utility (STU) icon on the desktop to open the programme. Minimise it but keep it running in the background. (If the icon is missing from the desktop the installation guide can be found in the practice-shared drive S:\eHealth\Pharmacy-Services\STU). STU reports data from the previous day (or earlier) in the practice. It does this by running an overnight job to copy the data from the GP clinical system. On occasions, the overnight job can stop running (due to server conflicts) which results in STU data being out of date. If this has happened, a warning box will appear on opening STU: To resolve this (or other STU error messages) contact the Escro Helpdesk on 01463 572000, describing the issue. 4
Installing the MCR Serial Rx Activity EMIS Template Step 1. Open and log in to EMIS in the usual way. Step 2. Go to TD Template Designer (in Configuration section at bottom of screen) N.B. - If you do not have this option, see the practice manager. Step 3. If necessary, click on the General tab then Explorer. Identify a suitable folder to import the template. For example, Pharmacy and click to select. 5
Installing the MCR Serial Rx Activity EMIS Template Step 4. Click on I – Import Template. Step 5. Select template to import window will open. Click on Computer then double click Shared then RepeatPrescribingSerialRx2020 then Coding Templates then Coding Templates then EMIS. Finally, click MCR_Serial_Rx_Activity.tex to select then click Open. Step 6. The egg timer will briefly appear then a confirmation box will pop up, click OK. The template is now imported. N.B. - If when updating the template a message box appears (for example installing on notification of a newer version), click Yes to overwrite the previous template. Check and install the Serial Prescribing tool if necessary. 6
Installing the MCR Serial Rx Activity EMIS Template Step 7. Go to the practice computer shared drive (S:) and look for the folder RepeatPrescribingSerialRx2020. If the folder or tool is available, go to Step 15. If the folder or tool is missing, firstly check that it is not stored elsewhere on the PC. For example, within a Pharmacy folder. If the tool is not on the PC, download the latest version as follows. Step 8. Open the practice shared drive and go to S:\eHealth\Pharmacy-Services\Serial Prescribing. Step 9. Double click on the folder to open. Double click on the file to open. A dialogue box will appear, click OK. 7
Installing the MCR Serial Rx Activity EMIS Template Step 10. Click Unzip (the default location is the practice shared drive. If necessary, this can be changed by clicking Browse and then browsing to a specific location. For Example a Pharmacy folder within the shared drive). N.B. - If changing the default extract folder, ensure that colleagues are aware of where the tool is saved. Step 11. Unzip successful box will appear, click OK. Click Close to close the WinZip Self-Extractor window. Step 12. The tool will now appear in the practice-shared drive. 8
Running the search on EMIS Step 1. Double click the folder RepeatPrescribingSerialRx2020 then double click on the Repeat Prescribing tool. Step 2. Enter the database password mmles and click OK. Step 3. If a Security Warning appears in the upper left of the screen: click on Options, click Enable this Content, and then click OK to open the programme. 9
Running the search on EMIS Step 4. Enter the five-digit GP practice number into the yellow box. This should be the number for the practice that the tool is being run in. N.B. - This should only need to be completed when the tool is installed for the first time. Step 5. Click Refresh Data. (This may take up to 15 minutes to run in a practice with a large list size). Once successfully run, a box will pop up Analysis Complete. Click OK to open the Serial Prescribing REPORTS page. Step 6. In the REPORTS page, you can Set Your Own Requirements. The defaults are number of repeats between one and four and age range between 18 and 120. To change the defaults, click on the drop down arrow to the right of the number then click a number to select it. Repeat as necessary. 10
Running the search on EMIS Step 7. Click Select Pharmacies. You have the option to choose specific pharmacies to start with. Tick to select pharmacies as appropriate then click Close Form. Step 8. Click on the Microsoft Excel icon. This will export the data to an excel spreadsheet. Step 9. Save the spreadsheet to the correct folder on the shared drive of the practice computer or print the list as per individual practice agreement. Step 10. Now follow SR02: Screening a Database Search in EMIS. 11
