NURSING PROGRAM 4601 College Boulevard Farmington, NM 87402 (505)566-3224 harrisjl@sanjuancollege.edu APPLICATION INSTRUCTIONS AND INFORMATION PACKET FOR THE BSN FALL 2022 NURSING PROGRAM Thank you for your interest in the San Juan College Nursing program. The current application period is for the Fall 2022 ADN and BSN program. This application packet includes instructions and information for the BSN program selection for the Fall 2022 class. Please read the information carefully and complete all sections of the application. Incomplete applications will not be processed. It is your responsibility to be aware of any new prerequisite changes. PROGRAM DESCRIPTION BSN PROGRAM: This is a 4-year program leading to a Baccalaureate Degree in Nursing (BSN). This nursing program is offered in the traditional semester format over the course of 8 semesters. Class and clinical sessions meet mainly during daytime hours. In January 2015, San Juan College in partnership with the University of New Mexico (UNM), began admitting BSN students into the statewide common curriculum through participation in the New Mexico Nursing Education Consortium (NMNEC). Students in the UNM partnership BSN program of study (POS) will graduate with an associate degree in nursing (ADN) from San Juan College and with a baccalaureate degree in nursing (BSN) from the University of New Mexico. In preparation for BSN program application, the prerequisites and general education courses for application, selection and admission to the baccalaureate degree nursing program are included in this packet. A minimum 30 institutional credit hours are required to be taken through the University of New Mexico in the degree requirements for graduation. Please review the BSN program curriculum with this application or contact Jana Harris (harrisjl@sanjuancollege.edu) for more details. Students who graduate from the San Juan College/University of New Mexico Dual Degree Nursing Program will be eligible to take the National Council Licensing Examination (NCLEX-RN) for licensure as a Registered Nurse (RN), and may continue on to enroll in a master’s degree in nursing (MSN) program offered at select four-year institutions. Applications will be accepted no later than February 15, 2022.
Application Process – BSN The following must be received by the application deadline: Tuesday, February 15, 2022. Application to the NMNEC San Juan College/University of New Mexico BSN program is a dual application process. 1. Apply to San Juan College, if not an SJC student, and complete admission enrollment process as an SJC student. The college application can be completed in person or online. 2. Submit official transcripts from all schools attended or currently attending to the San Juan College Enrollment Services (Admission and Records Office), 4601 College Blvd., Farmington, NM 87402 or sent electronically to transcripts@sanjuancollege.edu. Prior college transcripts will be evaluated for possible transfer credit. If you have previously submitted official transcripts to SJC and have not attended school at that location since last sending transcripts for evaluation, do not request official transcripts to be sent again to SJC. An updated official transcript will need to be requested at the end of the fall 2021 semester if course work is completed at a college other than San Juan College. Do not send official transcripts to the SJC Nursing Department. It is student responsibility to confirm that all requested official school transcripts have been received by SJC Admissions. If credit from an AP or credit by exam course has been received, an official score transcript is required for transcript credit. 3. All transcripts must be received by the February 15, 2022 deadline. Transcripts received after the application deadline may not be processed for prerequisite completion. 4. Submit a completed San Juan College BSN Nursing Program Application to the San Juan College Nursing Department. The application can be submitted in person during business hours (Monday-Friday, 8am-5pm) or by postal mail at any time during the current open application period (October 15, 2021-February 15, 2022). Faxed or emailed applications submissions will not be accepted. 5. Apply to the University of New Mexico College of Nursing by submitting a nursing application through the NursingCAS online nursing application system. Follow the companion NMNEC SJC/UNM Dual Degree NursingCAS instructions provided with this SJC application packet and the online NursingCAS specific instructions. The SJC/UNM NursingCAS instructions can also be accessed from the UNM College of Nursing website. You are required to request official transcripts from all schools attended or currently attending (including SJC) be sent to NursingCAS including score transcripts. It is student responsibility to confirm that all requested official school transcripts have been received by NursingCAS. UNM College of NursingCAS application fee is required. 6. Apply to the University of New Mexico. UNM admission fees apply. UNM application should be made as a transfer undergraduate since prior prerequisite college coursework has been completed. You are required to request official transcripts from all schools attended or currently attending (including SJC) be sent to UNM admissions. If you are a current University of New Mexico student, request any updated official transcripts from other schools to be sent to UNM Admissions. It is student responsibility to confirm that all requested official school transcripts have been received by UNM Admissions. 7. If you are reapplying to the SJC/UNM Dual Degree nursing program, any updated official transcripts will need to be submitted to SJC Enrollment Services and also requested per UNM and NursingCAS application instructions. A new nursing application is required to both SJC Nursing Department and UNM College of Nursing per NursingCAS. 8. Complete all prerequisite courses (26 credit hours) by the end of the fall 2021 semester. If you have transfer/credit courses, verify credits have been posted to your SJC and UNM transcript as applicable. 9. Level 1 Entry prerequisites must be completed/in process for completion by end of the summer 2022 semester. Verification of completion through official transcripts and verification of enrollment through school schedule may be submitted. 10. Complete the enclosed Consent to Disclose Student Education (FERPA) document, sign and submit with the nursing application to the SJC nursing department. This allows SJC to communicate with UNM concerning your nursing application and courses.
11. Complete the TEAS Exam for the nursing program and submit unofficial score results to the SJC Nursing Department. All applicants are required to have current test scores for the application period. Previous scores may be eligible if taken during the prior spring application period (June 15, 2021-September 15, 2021) but only the two most recent scores will be considered for the current application period. Students re-applying to the program will be required to retest if prior spring application score is below the minimum eligibility score. Topics for this exam include reading, math, science, English and language usage. Study guides can be ordered on the ATI (Assessment Technologies Institute) website: http://www.atitesting.com. Additional preparation resources include practice exams and TEAS online study course available through the ATI website and tutoring assistance in the San Juan College Student Achievement Center. The exam may be taken once per test period and early scheduling of testing date is strongly recommended. Testing seats are limited. Testing dates may be scheduled during: First testing period: Wednesday, October 27, 2021 – Saturday, November 20, 2021 Second testing period: Wednesday, January 19, 2022 – Saturday, February 12, 2022 Verify exam being scheduled and administered on test day is the ATI TEAS for nursing exam. Instructions for scheduling the TEAS for nursing exam: On-Campus testing: provided in the San Juan College Testing Center. Applicants will need to establish an ATI account prior to testing. Review the testing instructions located on the SJC Testing Center “ATI TEAS for Health Science Programs” webpage tab. Schedule testing date through the SJC Testing Center website. Contact the SJC Testing Center at 505-566-3139 should you have any issues scheduling the test online. Print unofficial score results through the student ATI account for submission to the nursing department. Unofficial scores are not printed from the SJC Testing Center. Official scores will be obtained by the nursing department if applicant has tested in the SJC testing center and then matched with submitted unofficial scores. Off-Campus Site testing: provided through the San Juan College Testing Center. Follow SJC Testing Center instructions for ATI TEAS off-site testing located on the SJC Testing Center webpage. Additional testing centers can be located through the Consortium of College Testing Center: https://www.ncta-testing.org/list-of-certified-centers. If scheduling the exam at a testing center that will not be conducted through the San Juan College Testing Center, the applicant will need to request an official score result transcript(s) to be submitted to SJC Nursing Department through the ATI online store. Applicants will need to establish an ATI account prior to testing. Unofficial score results for all off-campus site testing will also need to be submitted to the nursing department and can be printed through the student ATI account. Official scores are matched with the unofficial scores. 12. Activate San Juan College email account through the “Student Resources” tab on the SJC website. Notices, important updates and/or questions will only be sent to your official SJC student email account regarding your application from the nursing department. If you do not include your SJC email on your nursing application or routinely check your SJC email, important information and instructions will be missed that could affect completion of the application process and application eligibility. 13. Indicate your BSN application preference on the nursing application. • BSN as first preference but also requesting consideration for ADN program as second preference: choose this option if you would be willing to enter either the BSN or ADN program if selected. If the final selection point total does not place the BSN applicant in the BSN ranking for selection, selection points will be recalculated based on the ADN 17 credit hour prerequisites with all other points and then ranked in the ADN total point selection. • BSN preference only: choose this option for consideration for the BSN program only. If final selection point total does not place you in the BSN ranking for selection, points will not be recalculated or BSN applicant ranked in the ADN total points selection. BSN PROGRAM SELECTION REQUIREMENTS To be eligible for the nursing program selection process, the following program requirements must be completed by the current application deadline: 1. Be enrolled as a San Juan College student.
2. Attend a Fall 2022 Nursing Info Session. Sign-up when you return your completed application to the nursing department. Info Session dates will be via Zoom and will be offered TBD. You will be required to attend ONE of the sessions. The session will be one-hour in length and attendance is mandatory. If you do not attend a session your application will be considered incomplete and you will be ineligible for selection for this application cycle. 3. Completed all BSN prerequisite courses (26 credit hours). Achieved a minimum cumulative GPA of 2.75 in the BSN 26 credit hour prerequisite courses. The GPA requirement must be fulfilled at the end of the fall 2021 semester prior to the application deadline. All prerequisite courses require a minimum grade of ‘C.’ 5. Achieved a minimum cumulative GPA of 2.0. The GPA requirement must be fulfilled at the end of the fall 2021 semester prior to the application deadline. 6. Taken the ATI TEAS for nursing (Test of Essential Academic Skills) for nursing exam and obtained the minimum eligible Total Score of 58.7%. All applicants are required to have current test scores for the application period. If an applicant has tested during both current testing periods or has two scores within the total eligible time frame, the higher of the two most recent scores will be used for calculation. Exam scores submitted that were taken outside of the current testing timelines are not eligible. 7. Submitted a current completed BSN nursing program application to San Juan College Nursing Department with the Consent to Disclose Student Education Records (FERPA) form. Note: If reapplying for selection, a new application and FERPA from are required. Applications received in the nursing department after the application deadline will not be eligible for the selection process. 8. Submitted a University of New Mexico College of Nursing application through the NursingCAS online nursing application system. 9. Submitted all official school transcripts to San Juan College Enrollment Services, University of New Mexico Admissions and NursingCAS as per all application information instructions. 10. Apply to University of New Mexico and complete admission process or reapply/update information requirements as indicated if a prior UNM student. 11. Level 1 program entry prerequisites (19 credit hours) must be in process for completion by end of the summer 2022 semester in anticipation of program entry in fall 2022 if selected. The 19-credit hour BSN program entry prerequisites are listed on the BSN curriculum page in this packet.
SELECTION Selection of students to the San Juan College nursing program is based on a point system. Applicants are ranked according to total points. Nursing Program slots are filled beginning with the highest-ranking students. Completing all requirements does not guarantee acceptance to the BSN/ADN program. Selection points are awarded based on the following: 1. GPA. GPA is calculated from grades achieved in the required 26 credit hour prerequisites. 40% of points will be awarded based on the cumulative 26 credit hour prerequisite GPA. 2. ATI Test of Essential Academic Skills (TEAS) for nursing Total score. 50% of points will be awarded based on the highest eligible Total Score. 3. ATI Test of Essential Academic Skills (TEAS) for nursing Proficiency score. 3% of points will be awarded based on achievement of a Total Score of 64% or higher. 4. Residence Status: 2% of points will be awarded based on San Juan County residence status OR American Indian residence status OR completion of minimum on one SJC Health Care Certificate. 5. San Juan College Student Status. 3% of points will be awarded to San Juan College students who have completed a minimum of 12 of the required 26 application prerequisite credit hours at San Juan College. 6. Science GPA. 2% of points will be awarded for a combined 3.5 GPA or higher based on 8 credits in basic and health science courses. The total points of the aforementioned selection components are calculated as follows: GPA calculation: GPA – 2.0 = P P x 400 = Points for GPA 2.0 TEAS for Nursing Pre-Admission Examination: Total Score x 500 = Points for TEAS 100 TEAS for Nursing Pre-Admission Examination Proficiency: Total score 64% or higher = 30 Points Total score 58.7% to 63.9% = 0 Points San Juan County Residence Status: In-county residency status or American Indian residence status or Health Care Certificate = 20 Points Out-of-county residence, non-American Indian residence status, non-completion of Health Care Certificate = 0 Points San Juan College Student Status: 12 or more prerequisite course credit hours completed at SJC = 30 Points Less than 12 prerequisite course credit hours completed at SJC = 0 Points Science GPA: Combined GPA or 3.5 or higher from 8 credits in basic and health science courses = 20 Points Less than 3.5 combined GPA from 8 credits in basic and health science courses = 0 Points SUM OF COMPONENTS = TOTAL POINTS
ACCEPTANCE Students will be notified of nursing program acceptance status through postal mail in early April 2022 for a fall (August) 2022 program entry. Applicants who have not met the basic program eligibility requirements will also be contacted by mail regarding their status. When selected students have confirmed program acceptance, a detailed nursing admission information packet will be provided with specific instructions and guidelines regarding the requirements (i.e. immunizations, CPR, criminal background check/fingerprinting, etc.) to be completed prior to starting the Nursing Level 1 semester courses. Selected students are required to attend a one-day Summer Institute Orientation at San Juan College in early August 2022 prior to the fall semester start date and the scheduled date will be included in the admission packet. Students accepted into SJC/UNM BSN Dual Degree Nursing Program maintain selection status by completing all Nursing Level 1 course prerequisites (19 credit hours), post selection program entry requirements and maintain the minimum GPA of 2.0 post selection for entry into the Nursing Level 1 semester. A grade of “C” or higher must be achieved in every course in the nursing curriculum. In order to meet clinical requirements to successfully complete our programs, proof of vaccinations, including the COVID 19 vaccination may be required.

CONSENT TO DISCLOSE STUDENT EDUCATION RECORDS If any person other than you needs or will need access to your education records maintained by San Juan College, complete this form and return it to the Enrollment Services Office. Other College officials may take this form directly from the student and forward to the Enrollment Services Office. Family Educational Rights and Privacy Act of 1974 (FERPA) FERPA defines the requirements for access to and release of student education records. Student education records are defined as records that are directly related to a student and are maintained by an educational institution. San Juan College has designated certain parts of a student’s education record as “Directory Information,” (see below) which may be disclosed to third parties without the student’s written permission. Student education records include, but are not limited to, course grades, billing records, and disciplinary records. In general, education records that are not designated Directory Information may be disclosed only to the student unless the student provides written consent to disclose those records to other persons. However, disclosure of non-directory information may be made to (1) an individual who claims the student as a dependent on the individual’s federal income tax return, as defined in Section 152 of the Internal Revenue Code, (2) to parents or legal guardians of a student who is under 21 years of age and has been disciplined by the college for any alcohol or drug policy violations, and (3) to parents or legal guardians of a student who is in a health and safety crisis. NMNEC SJC/UNM Dual Degree Fall 2022 NursingCAS Instructions As a prospective applicant into the NMNEC SJC/UNM Dual Degree Option starting Fall 2022, you are required to submit a completed NursingCAS application by February 15, 2022. 1. Go to https://nursingcas.liaisoncas.com/applicant-ux/#/login. Create an account if you have not previously used NursingCAS or login. 2. Search for the NMNEC SJC/UNM Dual Degree Fall 2022 Application under the “Add Program” tab. Search for University of New Mexico under school Select the NMNEC SJC/UNM Dual Degree Option for Fall 2022 3. Complete all fields in the “Personal Information” section. 4. Under the “Supporting Information” section, you will not be required to upload or submit any items. Achievements- select “I am not adding any Achievements” Experiences- select “I am not adding any Experiences” Licensure & Certifications- select “I am not adding any Licenses or Certifications” 5. Under the Academic History section, High School Attended- Add your high school Colleges Attended- Add all institutions attended Transcript Entry- you are required to enter in your coursework including W’s and F’s 6. Under the Program Materials section, Upload a current class schedule if you are in classes. Answer questions located under “Questions”. Upload an unofficial copy of your TEAS test report in the documents section. Please make sure it is either in a .PDF or .JPEG format 7. All sections must be completed and green prior to submission. 8. Submit application and pay application fee. 9. Submit all official transcripts directly to NursingCAS for verification and processing by February 15, 2022. This is not a postmark deadline, if sent by mail.
