The Coalition of Homeless Services Providers (Coalition) is Monterey and San Benito Counties’ designated Continuum of Care (CoC) Coordinator. A CoC represents the totality of services and housing available to individuals and families in crisis/homelessness within a defined region. The Coalition is a nonprofit organization whose board of directors is comprised of local agencies that provide housing and services to the homeless population. The Coalition serves as the lead agency for the Coordinated Assessment and Referral System, Homeless Management Information System, Homeless Census, assists with the development of strategies to address homelessness and is the gateway for many federal and state homeless funding programs. The CAP is designated by the County Board of Supervisors to specifically provide a focal point to coordinate and plan for the provision of community services that support, assist, and empower low-income people and to improve their quality of life. The CAP engages in three major activities to achieve this mission: contracting with nonprofit and public agencies for services, collaborating with community stakeholders, and supporting educational community activities. INTRODUCTION In partnership with the Coalition of Homeless Service Providers, the Community Action Partnership has compiled this guide to assist the community in connecting to resources that serve the homeless and at-risk populations in Monterey County. This guide to homeless services in Monterey County is comprised of public and non-profit resources. Although updated annually, the guide is not exhaustive and every effort has been made to include accurate and current information. It should be emphasized that services in this community are frequently changing and the details regarding specific services provided should always be confirmed directly by the individual agency. The inclusions of services into this guide do not imply recommendation or endorsement. COORDINATED ASSESSMENT AND REFERRAL SYSTEM (CARS) Coordinated entry is a consistent, community-wide process to match people experiencing homelessness to community resources that are the best fit for their situation. In a community using coordinated entry, homeless individuals and families complete a standard triage assessment survey that identifies the best type of intervention for that household. Participating programs accept referrals from the system, reducing the need for people to travel distances seeking assistance at every provider separately. 2023 Monterey County Homeless Services Resource Guide 3

The center provides supportive services and housing navigation. COMMUNITY HOMELESS SOLUTIONS – CENTRAL COAST RESPITE CENTER Seaside, CA (831) 747-2900 www.communityhomelesssolutions.org A 6-bed shelter for homeless adults in need of respite care following stays in 3 participating hospitals (CHOMP, Natividad, SVMH). Access to meals, case management, and supportive services designed to help participants transition to permanent housing. By hospital referral only. Capacity depends on the number of children in families. Provides case management, housing navigation, meals, laundry facilities, mail service, social recreation activities, and linkages to income, education, and employment. MCHOME’s goal is to move individuals with psychiatric disabilities off the street and into housing. The SHARE Center provides supportive services, employment coordination, and housing navigation. Must have a referral or apply in person for entry into the program.
