Medical Fitness Certification The Augusta Health Lifetime Fitness Center has earned certification as a Medical Fitness Facility from the Medical Fitness Association (MFA). Only a relatively small number of medical fitness centers in the country have earned this certification. Not only is Lifetime Fitness one of the first 20 MFA certified fitness centers in the nation, but also the first to earn a perfect score of 100 during the certification process in 2011. MFA Medical Fitness Center Certification, recognized as a mark of excellence in the health and wellness industry, assures the community of the quality, safety and excellence of the facility and its staff. This certification distinguishes medical fitness facilities from traditional fitness centers. Augusta Health Lifetime Fitness goes beyond fitness equipment and group exercise classes to employ the highest qualified and credentialed staff. Our focus encompasses improved fitness, but also overall wellness and the prevention and treatment of chronic diseases like diabetes, heart disease, metabolic syndrome, depression, and certain types of cancer. Member/Guest Privileges Policy Lifetime Fitness is a private facility and as such, has the right to suspend or withdraw membership privileges from any member or guest who, in the facility’s sole discretion or opinion, engages in conduct considered to be inappropriate or unacceptable, or amounts to a breach of the facility’s poli- cies and procedures, or is in any way detrimental to the center, any of its employees, or any other member. Any membership termination or expulsion shall be effective immediately and any payment shall be refunded. If a member is suspended, that mem- ber forfeits all privileges to the facility and shall be entitled to repayment of any fees or dues while the membership is suspended. Further, the center may at any time, with or without cause, for any reason, terminate a mem- ber’s membership upon providing thirty(30) days advance written notice of such termination. As referred to in this policy, the term “member” includes the actual paying member, a trade or complimentary member, primary or sub-member, his/ her family member and any guest. Guest Policy Augusta Health Lifetime Fitness welcomes guests to visit our facility. All guests must be at least 18 years of age unless they are accompanied by their parent/legal guardian. All guests with medical concerns are subject to refusal of guest visits until physician clearance is obtained. Guests must abide by all rules/polices of Lifetime Fitness. Adolescent guests aged 13-17 may use the facility with direct parental su- pervision, but guests under the age of 13 will not be permitted to use the facility. Parent or guardian must sign a children’s facility usage agreement and be present for the duration of adolescent’s exercise program unless the adolescent is with a supervising staff member. Guest Passes Available: Prospect Pass: A free 2-week trial is available for prospective members interested in joining the center. This allows potential members the oppor- tunity to try the center and a variety of our programs and services before making a decision about a membership. Prospects must live in the local area. The pass is valid for 2 full weeks consisting of 14 consecutive days only. Once the 2-week free trial is complete, prospects may either decide to join or pay additional guest visit fees. Member Guest Passes: Lifetime Members may bring out-of-town guests to the center for free. This is a courtesy to our members who may have out- of-town family or friends visiting the area for holidays or other occasions. There is not a limit on the number of out-of-town visitors a member may bring, but we limit each particular visitor to 14 free visits per calendar year. 4
These guests must check in with the front desk and show valid proof of out-of-town address.
Guess visits for Purchase: Aside from the options on the previous page, the guest fee to utilize the center is $12 per visit, to be paid upon entry to the facility. 5-visit and 10-visit passes are also available for purchase at the front desk. Membership Formats Aquatic Membership Use of our 6 lane junior Olympic pool, therapeutic pool, aquatic fitness classes, whirlpool, sauna, and steam room. Total Fitness Membership All the benefits of the Aquatic Membership, as well as access to all weight and cardio equipment, racquetball, fitness and group cycling classes, and indoor walking track. Universal Membership Designed for tennis lovers, this membership offers access to all of our fit- ness and aquatic facilities, as well as tennis court time on our 3 indoor courts and 4 outdoor clay courts. 6
Programs and Services The Kid’s Place • Child watch is available for children 6 weeks to 8 years old, for up to 2 hours at a time. • All children must be registered prior to bringing them for their first visit. • Reservations are not required, but space may be limited based on capacity. *Please see the childcare brochure for current information regarding payments and general rules. Hours may change based on utilization, so please confirm with the Kids Place Staff. Courtyard Café Members receive 10% discount (540) 332-8644 The café features signature sandwiches, wraps, salads, and more. In addi- tion, the Courtyard Café proudly serves Green Mountain® coffees, 100% real fruit smoothies and homemade baked goods. 7
New Member Appointments Health Evaluation A great way to determine your current level of fitness! You will be meas- ured on the following: resting heart rate, blood pressure, body composition, muscular strength and endurance, cardiovascular endurance, and flexibil- ity. Your results will indicate how you compare with others of your same age and gender. Alternate Health Evaluation This is a “more gentle” fitness evaluation for older members or anyone with orthopedic or other physical limitations. The test measures agility and bal- ance, upper body and lower body strength, flexibility, height, weight, resting heart rate and blood pressure. Assessment New members who have physical ailments, chronic diseases or conditions are recommended to meet with one of our Exercise Specialists for an assessment. This assessment will last approximately 10-20 minutes, at which time you will discuss your limitations or any modifications that need to be addressed when prescribing an exercise routine. The Exercise Specialist may make recommendations as well as create a workout program designed specifically with your needs in mind. Equipment Orientation Every new member at Lifetime will have the opportunity to complete an equipment orientation with a member of the Fitness Floor Staff. This orien- tation allows the member to have a workout created for them specifically based on the goals they are looking to achieve! The member is then shown how to operate a variety of different pieces of equipment that will assist them in achieving those goals. 8
Additional Services Personal Training When you hire a personal trainer you are not just purchasing their physical presence and guidance through your workouts but also their education, experience, knowledge and expertise with program development and design. Our bodies have more than 200 bones, over 600 muscles and approximately 230 moveable and semi-moveable joints. Personal trainers have been educated in these complexities and therefore, are uniquely qualified to help you achieve your personal fitness goals as safely and quickly as possible. Let one of our Personal Trainers assist you with your exercise regime today! Micro-Fit Test Have you ever wondered about your fitness level? A Micro-fit test meas- ures: Resting Blood Pressure, Body Composition, Body Weight, Aerobic Capacity, Back Flexibility, Muscular Strength and Muscular Endurance. These tools can be used to properly prescribe an exercise program for you. Your health/fitness profile scores are included in your printed report showing how you compare with others of your same age and sex. In addi- tion, your report will include important information on ways to improve your health and fitness. The Micro-fit system maintains your current results so follow up tests can be scheduled to measure your progress.
Medical Programming Lifetime Fitness offers several monitored medical exercise and educational programs for individuals who want to exercise. Participants may have special needs, chronic conditions or physical limitations. These may include but are not limited to individuals with chronic illnesses such as stable cardiovascular disease, stroke, diabetes, severe obesity, pulmonary disease and/or orthopedic limitations. Transition Program Once discharged from therapy, give yourself the gift of fitness by joining Augusta Health Lifetime Fitness. Our Medical Fitness Coordinator will meet with you to help you formulate fitness goals. You may enter our Medical Programming or begin your own workout program. Our Fitness Exercise Specialists will provide a full equipment orientation and answer your questions. Transition Programs are available in one-, two-, or three-month memberships. Contact a Membership Services Representative at (540) 332-5843 for more details. Client Training Plus One on one supervised exercise program for individuals with medical conditions or individuals who may not be able to exercise on their own. This program offers a “helping hand” without providing constant progression. Sessions are up to 60 minutes, focusing on exercises, skills or movements on land or in the water. Session times are scheduled with Exercise Specialists. A maximum of 8 sessions may be purchased unless approved by the Medical Fitness Coordinator. RxEX Programming RxEX Program This is a monitored medical program by physician referral only. Once you are referred by your physician, you will take part in an 8 week program, exercising with our qualified staff twice per week. You will also receive pre & post health evaluations to see your improvements throughout the program. Exercise sessions may be in a small group or independently, with the focus on meeting your goals set prior to the program. Each workout is created specifically with your needs in mind. Participants in this program are also eligible to utilize Lifetime Fitness throughout the 8 week program. RxEX Aqua This is a monitored medical program by physician referral only. Once you are referred by your physician, you will take part in an 8 week program, exercising
with our qualified staff twice per week in the therapy pool. You will also receive pre & post health evaluations on land to see your improvements throughout the program. Exercise sessions may be in a small group of 2 or independently, with the focus on meeting your goals set prior to the program. Each workout is created specifically with your needs in mind with staff assisting you in the water, or instructing you on the pool deck. Participants in this program are also eligible to utilize Lifetime Fitness throughout the 8 week program. RxEX Cancer Lifetime's Prescription Cancer Exercise Program, also known as the Cancer RxEX Program, is an 8 week program available to members of the community who have had or currently have Cancer. The program is designed to improve a person's stamina, strength and fitness needs through monitored exercise. Each participant will receive a pre and post-program evaluation to monitor progression and success throughout the program. Participants will also meet with a Certified Cancer Specialist twice a week to be assisted with an individual workout routine. Nutrition Services Nutrition is a very important component of health and wellness, particularly if you are trying to lose weight. We offer a variety of nutritional information and educational services to help you achieve your goals. Nutrition counseling combined with personal training can be particularly effective. Please see our website or brochures in the lobby for more information on any of the above services.
Fitness Classes Group Fitness Classes There are many types of fitness classes offered for participants from 13 and older. The focus of our classes is on exercises that will make every- day functional activities easier. Our instructors are certified professionals who will make sure that you are exercising properly and having a fun and enjoyable experience. The schedule of classes is updated each month and may change daily due to the availability of the instructors. Notes will be located throughout the center and on our website if a change occurs. Specialty Classes Our specialty classes are included in your Total Fitness membership. These classes are a great way to supplement your exercise routine. Specialty classes may include: Yoga, Tai Chi, and Chi Kung. These classes are very special; try one and you will see why. They help enhance the immune system, develop strength, increase flexibility, reduce stress, improve circulation and digestion, increase energy levels, improve concentration, promote sounder sleep and improve respiration – just to name a few of the many benefits. After only a few sessions, you will begin to notice changes. Many physicians recommend these excellent forms of exercise as supplementary therapy for many conditions. Aqua Classes The aquatic program at Lifetime Fitness is an excellent way for anyone to exercise. Whether a beginner or an expert, you will experience a refreshing workout! Come and add a new way to exercise to your present regime. You will be glad that you did! Levels vary from a slower, Range of Motion (ROM) class to faster paced, Aqua Power and Deep Water Conditioning. Cycling Group cycling is an exciting training program that brings a different dimen- sion to stationary cycling. Cycling offers a means to improve aerobic and anaerobic fitness, as well as enhancing cycling efficiency. Your instructor will take you through a series of cycling commands, which will have you climbing mountains and racing to the finish line. Classes vary from 45 min- utes to an hour. Cycling is included in your Total Fitness membership. Class schedules are available online and in the front lobby 59
Specialty Programs (Extra fees may apply) Martial Arts Okinawan Karate is a defensive art steeped in tradition, discipline, history and culture of the 14th century. Under the teaching and instruction of Dr. Tom Muncy, Grandmaster, students will learn the discipline forms while developing coordination, internal discipline, physical stamina, mental alertness, increased awareness, and confidence in their ability to defend themselves. Classes are taught Monday and Wednesday evenings. Additional Fees apply Medical Recovery/Enhancement Classes These classes include: Tai Chi for Cancer Recovery, Arthritis, Diabetes, Multiple Sclerosis, Meditation and Fibromyalgia! Aqua Programs Aqua Group Fitness Classes - please see the previous page for details. Private & Semi-private Swim Lessons Private & Semi-Private swim lessons may be purchased as individual packages or packages of 5 or 10 sessions. These lessons are available for all ages and abilities (kids under the age of 12 must be children or grandchildren of Augusta Health employees or Lifetime Fitness members) Group Swim Lessons Our group swim lessons use the American Red Cross Learn-to-Swim program. Group lessons are available to children and grandchildren of Augusta Health employees or Lifetime Fitness members and are offered at various times throughout the year. Information will be located throughout the center and on the website. For all aquatics programming, please see the program registration area for more literature and details. 10

Appropriate fees will apply. • Only one reservation may be made at a time in your name. Subsequent reservations may be made after your scheduled reservation is completed. You may only have one reservation on the schedule at a time (does not include contract time) in your name. You must play out a court reservation before scheduling another court in you name. • A Member may not use another Member’s name to schedule a reservation in an effort to have more than one court reservation scheduled in a 7-day period. 11
• Courts may only be reserved on the hour or half-hour. Reserva- tions will not be taken for 7:15, 7:45, etc • Court reservations may not be made that interfere with clinics, leagues, lessons, tournaments, contract time, or special events. • • Outdoor court reservations that are unable to be kept due to inclement weather conditions will not automatically roll over onto the indoor courts. • All players must check-in at front desk to confirm reservations, and get court assignments before entering the court for play. • Players must leave the court promptly at the end of their scheduled time unless the court is available following your time. • Non-Members must pay the facility guest fee along with their portion of the court fee (length of time divided by number of players). • Non-Universal Members must pay the appropriate court fees when using the courts. • Adolescents aged13-15 years may utilize courts with direct parent/ adult supervision. Adolescents 16 and older may utilize the courts unsupervised. • Students with the Student Racquet Membership do not have to have a parent who is a Member to join, and may utilize the facility as stated in the Student Racquets Membership requirements. • Abusive language or actions will not be tolerated by anyone. Offenders will be asked to leave. • Non-marking soled tennis shoes only on all 3 indoor courts. Court appropriate (non-cleated, or heavy treaded) shoes only on all 4 outdoor courts. • Water only allowed on courts, no Gatorade, PowerAde, Sodas, Coffee, or any colored drinks. • Fees must be paid at front desk in advance of playing. 12

Lifetime Policies & Procedures Member Conduct Members are expected to treat others with respect at all times. Profanity and disrespectful behavior will absolutely not be tolerated. All policies and procedures are to be followed at all times. Failure to comply may result in membership termination. Building Access and Front Desk • All members MUST check in at the front desk or the free weight room near the back entrance. • You must use your key tag to scan in each time you enter the fitness center. If you forget to bring in your card, you may be asked to show a driver’s license to the front desk attendant to check into the facility. Lifetime will allow a member to enter the facility 3 times using the above method. After that, the member will be asked to purchase a new scan card. • Membership cards are non-transferable. • Unauthorized use of the membership card by someone other than a Lifetime member could result in termination of membership. • Please report all lost or stolen cards immediately. A fee of $5.00 will apply to replace a lost card. • Smoking is not permitted in the center, or anywhere on Augusta Health property. • Eating and drinking is only permitted in the lounge and café areas with the exception of plastic water bottles. • Children over the age of 2 years must use a gender appropriate locker room or the family locker room. • Please be respectful of other members. 14
Attire Proper footwear for the area of the facility being used must be worn. Only court shoes (non-marking soles) are allowed in the tennis area, and on the racquetball courts. No shoes that mark the floors will be permitted. No open toe shoes will be permitted unless in the pool area. Proper attire, including shirts, must be worn while in the Fitness Center. Exercise and swim attire must not be inappropriately revealing. Towels, Lockers & Equipment • Daily use towels and towel rentals are issued at the front desk reception area. • Members receive one exercise towel per visit. • Larger bath towels may be rented monthly. Please see Member- ship Services to do so. • Member who rent a locker will be allowed to use one complimentary bath towel per visit. • Complimentary 16” x 36” lockers are available to use by members on a first-come, first-serve basis during each visit to Lifetime Fitness. • Members may opt to RENT a dedicated 16” x 36” or 16” x 72” personal locker through Membership Services. Locker rentals are only available through a 12-month agreement. Prices are pro-rated depending on membership date. (Limited number available). • If equipment is issued, (by the front desk), to a member, he/she will be responsible for returning items to the front desk. If you fail to return items used, you will be billed for the items. Locker Rooms • No food or glass bottles allowed. • Showers are required before entering the spa and pool. • Please return towels to towel bins located at the entrance of each locker room. • Please do not use any type of video device while in the locker rooms. • Please wipe down counter top and benches after using them. • Please take all items from your locker when leaving and close the locker door. • Please place a towel on the bench prior to sitting unclothed. • Please do not dye your hair in the showers. • Please do not leave any items unattended. Place all items in a locker. • Please respect other Lifetime members and help us keep the locker rooms as clean as possible by cleaning up after yourself. • Dry feet before walking on carpet or using scales. *Lifetime Fitness is not responsible for lost or stolen items. 15
Infection Control Policy Recurrent concerns with any of the following may be cause for termination of your membership. • If you are emitting any potentially infectious bodily fluids, such as blood, stool, respiratory secretions and others, please wear appro- priate barriers. If any bodily fluids contaminate equipment, please notify Lifetime staff immediately. • Members or guests should not use the aquatics areas if emitting any potentially infectious bodily fluids. • Respiratory illnesses of all members and guests should follow Respiratory Etiquette by CDC Guidelines. Members or guests may be asked to wear a mask by staff if conditions persist. These guidelines are located at www.cdc.gov or a copy is available to view at Lifetime’s Front Desk. • Members or guests should not utilize the pools or steam rooms if they have any open wounds or sores. • Members or guests with Chicken Pox or Shingles should not use Lifetime Fitness until all lesions have crusted over at least 24 hours. • Members or guests with Tuberculosis should not utilize Lifetime Fitness until cleared by a Pulmonary or Infectious Disease Specialist. • Members or guests with blood borne infections (such as viral hepatitis, HIV, etc.) should not utilize Lifetime Fitness with any open wounds. • If members or guests are wearing an ostomy bag, please wear an approved aquatic ostomy cover prior to entering the pools. Outdoor Walking/Jogging Path Length – 1.25 miles, starts by ambulance building The outdoor walking track is available to members of Lifetime Fitness, as well as the communities we serve – Staunton, Waynesboro, and Augusta County. This facility is committed to the promotion of health and fitness within our communities through an easily accessible and popular exercise modality. Anyone using the track should review and adhere to the following guidelines for the benefit of all: • The track is open from sunrise to sunset seven days a week. No one is allowed on the track after dark. • If you bring your pet, please be sure to dispose of any pet waste. • You must be 16 years of age to use the track. An adult must accompany anyone under this age at all times. (i.e. parent or guardian). • No bikes, roller blades, skateboards, etc., are allowed on the track. • Please be aware of your health condition or any limitations that you may have. If you do have any compromising health condition, the buddy system is strongly advised. • Be cautious of wildlife in the area towards evening hours.
Fitness Floor • All members have the option to complete an equipment orientation before using any of the equipment. • Individuals must be 16 years or older to use the Fitness Center without direct parental supervision. • All personal items must be kept in a locker. The fitness staff will not hold any items and are not responsible for items left on the Fitness Desk. Please do not leave gym bags on the fitness floor, in the studios or loose in the locker room. • Equipment may only be used for its designed purpose. • Always ask for assistance before attempting any new exercise. Please wipe off cardio and weight equipment after use. Antibacte- rial wipes will be provided to use on the equipment and are located in several locations throughout the facility. • No food or drinks are allowed except water and sports drinks. • Allow other exercisers to ‘work-in,‘ or take turns between sets. • Use control – dropping weights can injure you, others and damage the equipment. • Report any malfunctioning equipment to the Floor Staff. • Please respect the equipment and make sure all safety guidelines are followed. Free – Weight Area • Please do not sit on equipment between sets to rest. • A circuit workout has priority on all selectorized/plate-loaded equipment. Members doing multiple sets must allow single-set exercisers to continue through the circuit without interruption. • Plate-loaded machines must have weights removed immediately after completion of exercise. Plates and equipment must be returned to their specified locations. • The use of collars is mandatory. • It is highly recommended that you utilize spotters when necessary (i.e. squats, overhead lifts, bench press, dumbbell exercises and incline bench.) • NO outside personal training is permitted in this facility. • Items that may damage the equipment upholstery may not be worn while using the equipment (weight belts, jewelry, etc.) • Dumbbells may not be used outside of the designated dumbbell area. • Return plates and dumbbells to their appropriate rack according to their weight. • The dropping or slamming of weights is prohibited. • Shirts and close-toe shoes must be worn. • All members have the option to complete an equipment orientation 17

Indoor Track: (10 Laps in middle lane = 1 Mile) • Please be aware of other runners and walkers using the track. • Joggers and walkers have the right-of-way, so please use caution before crossing the track. • Please use the track in the direction indicated. The track direction changes daily. • Track directions are located at the top of the stairway. • Inside and middle lanes are used for walking. The outside lane is used for jogging. Pool & Spa • Please shower off before entering pool or spa. • NO DIVING ALLOWED! • Floors are slippery when wet. Please use caution. Appropriate footwear is required when walking on the pool deck. • Please return pool equipment to the storage area. • During a thunderstorm, pool and spa must be exited. Staff will re-open the pools and spa no sooner than 15 minutes after the storm has passed. • Proper swim attire must be worn at all times while in the pool area. Swim attire must not be inappropriately revealing. • Consult your physician before spa usage if you are, or think you may be pregnant, have uncontrolled hypertension, any heart/lung conditions, infections or an open wound. • Individuals with open wounds or sores should not enter the pool. • Please limit spa use to 15 minutes. • Pool temperatures are kept between 82-86 degrees (lap pool), and 88-90 degrees (therapeutic pool), and are posted daily. • Spa temperature is kept at 100-102 degrees Fahrenheit and is posted daily. • Please do not remove lane dividers. • NO RUNNING OR PLAYING is allowed on the pool deck. • Children under the age of 16 must have direct parental supervision and are not allowed to use the pool when no lifeguard is on duty.
• Allow yourself at least five minutes after exercising to cool down before using the spa. • Lifetime recommends that everyone use the buddy system. • Pool and Spa are monitored regularly by surveillance cameras. • Children under the age of 9 are not permitted to use the spa. • Children under the age of 18 are not permitted to use the Therapy Pool. • The Physical Therapy Department of Augusta Health has reserved certain hours in the Therapy Pool each week. Please see posted schedule in the therapy pool area. Members are welcome to use the Therapy Pool during these hours for QUIET workouts with the understanding that Therapy staff reserves the right to ask members to leave at any time during said hours. • There is an emergency pull Cord located in the Fitness Pool and one in the Therapy Pool. Members are to be aware of their locations for emergency purposes. • Members are to adhere to all posted signs and to the instructions of the lifeguard on duty. • Lifetime strictly prohibits the practices of “breath holding”, “extended underwater swimming” and any other form of hypoxic training. • All bathers who are not toilet trained or who suffer from inconti- nence shall wear a swim diaper. Sauna Rules and Regulations • Individuals at high risk (pregnant women, individuals who take prescription medication or use alcohol, and individuals with high blood pressure, diabetes, open wounds and heart disease) should be advised not to use the sauna. • The sauna room is kept between 155-170O Fahrenheit with 5% relative humidity • Please shower before entering sauna. • Limit yourself to a maximum of 10 minutes in sauna. Prolonged exposure can result in hazardous hypertension. • Allow at least 5 minutes after exercise before entering sauna. • Individuals at high risk (as listed above) should be advised to not use the steam room unless authorized by their physician. • No food or drink allowed in sauna. • Drink plenty of fluids before and after leaving the sauna. • If you feel faint or dizzy in the sauna, please exit immediately. • Users are required to wear a bathing suit or shorts and to place a towel under their body when utilizing the sauna. • The sauna is an electrical appliance; do not pour any liquids on the rocks or place newspapers, towels, or any combustible material on the heater or guard fence. The heater gets extremely hot during 19
heating cycle and should not be touched or burns may result. • No one under the age of 18 is permitted to use the sauna. Steam Room Rules and Regulations • Wet surfaces in the sauna may be slippery. • Please shower before entering steam room. • No shaving in steam room. • Do not pour water on the steam room sensor; this can damage the sensor. • The steam room is kept between 100 and 110 degrees Fahrenheit (humidity 100 percent relative). • Limit yourself to a maximum of 10 minutes in steam room. • Prolonged exposure can result in fatal hyperthermia. • Allow at least five minutes after exercise before entering steam room. • Drink plenty of fluids before and after leaving the steam room. • If you feel faint or dizzy, please exit immediately. • Users are required to wear appropriate bathing suit or shorts and to place a towel under their body when utilizing the steam room. No street clothes or shoes. • Wet surfaces in the steam room may be slippery. • No one under 16 years old is permitted to use the steam room. • Steam room will be closed during family swim. Family Day Rules and Guidelines • Family Day is provided at Lifetime on Saturdays and Sundays from 2:00PM - 4:00 PM. • A copy of the birth certificate or social security card must be pre- sented to membership services during Membership office hours. At this time, a form will be completed and the child’s picture will be taken. If the child is not present, please bring a picture of the child. • Family is defined as members of your immediate household, (i.e. spouse, children under 18, and grandchildren under 18. Fees apply to all others). • All members and Family Day participants must check at the front desk before utilizing the facility. • Children under 16 years must have direct supervision at all times. • If, for any reason, we do not have a lifeguard available, the fitness pool will be closed to all children under 16 years. • There will be 1 lifeguard on duty. • Lifeguards will be posted at the lifeguard station 10 minutes before entrance of any family day participants. • Lifeguard will open up three lap lanes on the step end of the pool. This will be the only space allowed for families to utilize. Children 20
must not cross through the lap swim lanes for any reason. Permission may be granted for older children to lap swim, if the lanes are not being utilized. • The lifeguard will blow a whistle if needed to get the attention of a child in an attempt to change the child’s behavior and/or actions in the pool if they are deemed unsafe or disruptive. If the lifeguard has to blow a whistle three times to address the actions of a child during a pool session, the child will be asked to exit the pool area. • Children under age 16 are not permitted in the spa or steam room. • Parents must check-in family members with the lifeguard and will be asked to remain with their children in the pool area. 21
Change of Address • Members shall give a written notice to Membership Services of any change in address. Change in Medical / Health Condition • All members are required to complete a medical history questionnaire and sign an informed consent for exercise. • Lifetime staff may reserve the right to require a physician’s release before participating in any exercise program or you may sign a release/waiver form. • Members are required to notify Membership Services of any change in their medical history, which may affect their ability to safely participate in an exercise program. Change in Marital Status In Virginia, the mere fact that two spouses have separated does not mean their status as husband and wife has changed. The period that the husband and wife are separated becomes the grounds for the divorce, which is filed after the appropriate waiting period. If a spouse wishes to cancel another spouse, Lifetime must have permission from the person being canceled and the cancellation will be reviewed by Lifetime management. Once a divorce is finalized, the main member has the ability to cancel a co-member from the contract. The co-member can then go on their own contract as a single member without the other’s permission. Medical Hold Policy • Members can put their membership on a medical hold with a note from their physician. • The physician needs to state the reason and the period of time the member cannot participate in the facility. • If a return date is not specified in the initial note, the member will need to get medical release before revisiting the facility. • Lifetime needs documentation of this while under the physician’s care. • You will not pay dues during this time. • Your initial membership contract will be extended for the number of months your membership is on medical hold. 22
Membership Renewal • Membership will automatically renew for the same term of the membership agreement, UNLESS member notifies Membership Services in WRITING 30 days prior to the expiration of the membership agreement. Freeze Policy • Members will be allowed to freeze their membership for a minimum of one month and a maximum of three months. Those three months must be consecutive (i.e. June, July and August). • Members can only freeze membership one time per contract year and will be charged $10.00 per member (within each membership) to initiate freeze. In order to freeze membership, membership services must receive a thirty-day written notice in advance in order to initiate the freeze and your account must be current. • You will not pay dues during this time and your membership will be extended for the number of frozen months. Lost or Stolen Items • Lifetime is not responsible for lost or stolen items. • Members are encouraged to store all personal items in a locker. • Staff members are not permitted to hold valuables. Exceptions may be made for items such as motorcycle helmets that do not fit in a locker. • Items found should be turned in to the front desk . • For those items identified as belonging to a particular individual, Lifetime will make one attempt to contact the owner regarding the lost item. • Items not claimed within 14 days will be donated to charity. • If an item of perceived value is reported as stolen, hospital security will be contacted. Payments • Billing is processed on the 1st day of each month. • Payment is due on the 15th of each month. • Automatic electronic payments via bank account or reoccurring credit card draft will take place on the 1 5th . If the 1 5th falls on a weekend or holiday, the draft will occur on the next business day. • Accounts not settled by the last day of the billing month will be as- sessed a late payment charge of $5.00 per month until payment is current. • Membership privileges will be suspended for all members with de- linquent accounts. • If mailing payments, please allow 7-10 days for delivery. not responsible for late payments delayed by mail. • A charge of $25.00 will be added to your account for all returned checks, bank drafts and debit or credit cards. • Please remember that all changes to your account require a 30 day written notice. ** A charge of $25.00 will be added to your account for all returned checks, bank drafts and debit or credit cards. 24
Rules and Security Violations • Violations of posted or written rules may result in membership suspension. • Any violation of security such as stealing will result in suspension from the center and possible criminal prosecution. • We have cameras located throughout the center for safety concerns. Inclement Weather Policy • The Director of Lifetime Fitness (under the advisement of Administration) will make an appropriate decision concerning facility closure/delayed opening or early closing due to inclement weather. • A message announcing the decision will be placed on the Lifetime Fitness phone line, (540)-932-5433 as well as the website: http://www.augustahealth.com/lifetime-fitness/announcements to be accessed by Lifetime Fitness staff and members. An early morning decision to delay opening or to be closed will be available before 4:00 AM. But possibly the night prior if weather is imminent. Please check back for changes before venturing to Lifetime. Inclement Weather Policy for Fitness/Specialty and Aqua Classes: • Classes will be canceled in the A.M. hours with closing of The Waynesboro City School System when appropriate. If schools are closed multiple days, a decision will be made by Coordinator to determine class schedule and will be posted on the above website as well as on the communication board at the front desk. Noon classes will follow morning closing and cancellations. Evening classes will run as scheduled unless otherwise stated by noon via the above stated avenues. Cancellations of other programs will be at the Coordinators’ discretion and will be communicated with participants. • Kids place will be opened regular hours with facility opening unless extenuating circumstances arise. • Water fountains are located between the locker rooms on the main fitness floor. • All Lifetime employees are CPR, First Aid, and AED Certified. In case of emergency: • Emergency pull cords are located throughout the facility to pull if in need of emergency assistance. You will find them in the tennis area at the bottom of the steps leading from the café area on the left, handicap stalls of each restroom, family changing locker room, men’s and women’s lavatory area in locker rooms, on the window wall in the fitness pool, under phone in therapy pool and in both locker rooms in the therapy area. • Telephones are located throughout the facility. There is one near the water fountain on the main fitness floor, one outside the group fitness studio, one in the free weight room, one in the tennis café and one outside the specialty studio. If you need immediate assistance, please call 8043 for the front desk and help with be sent. Children and Youth Policies General • A Child Facility Agreement should be completed and turned into the front desk. • Membership at Lifetime begins at age 13, and therefore, anyone in the Center younger than 13, must be either A) in the Medical Adolescent Policy or B) in a youth program. • Youth programming can utilize various areas throughout the facility. All children within that program must remain with the group and are unable to exercise or wander off on their own. • The Medical Adolescent Policy allows children 8-12 years old to exercise, with their parent, in a supervised fashion. This program is based on physician referral only, and final approval to enter this program will be at the discretion of Lifetime’s Medical Fitness Coordinator. All children utilizing this program must enroll in membership and receive a wrist band each time they enter the facility to exercise. Please reference our medical adolescent policy in the children and youth manual for complete details. • No adolescent between the ages of 8-12 years will be allowed to: o Exercise in the Free Weight room. o Take part in any aerobic, martial arts, cycling, and water or specialty classes taught by instructors at Lifetime Fitness. o Utilize the Steam Room, Sauna, Whirlpool Spa or Therapy Pool. • All safety rules must be followed at all times. 26
• Children are expected to behave in a positive manner while attending programs/classes at Lifetime Fitness. Behavior expectations are provided to parents at the beginning of children’s programming to ensure proper behavior. These expectations can vary depending on the programming. Details will be provided at enrollment. • Any Adolescents not abiding by any of the above rules and guidelines may potentially lose membership privileges • Youth must wear appropriate attire, footwear and gear for activity. Additional guidance may be obtained at the first session. Drop Off & Pick Up Procedures • Meet in the assigned area specific to departmental programming protocol mentioned in the brochure. • Children should arrive no earlier than 5-10 minutes before the program is scheduled to begin. • For children under the age of 13, parent must make visual or personal contact with staff when dropping child off. • Children should be picked up no later than 5-10 minutes after the program is scheduled to end. • For children under the age of 13, parent must make visual or personal contact with staff when picking child up. • The Child Facility Agreement form (CFA) asks who is able, and unable to pick up the child from the facility. The instructor has that list and verifies the person is on the list prior to the child’s departure. If the person picking up is not on the list, the child will be unable to leave with that person. Childcare (The Kids’ Place) Guidelines • All parents will be required to sign their child in and out each time they visit. • Upon signing in, the parent will list the area of the facility that they will be utilizing, in case they need to be contacted to pick up their child. • Registration forms ask who is authorized and not authorized to pick up the child from the program. • Parents should take appropriate steps to ensure that their child is clean and has a fresh diaper (if appropriate) before leaving them in childcare. • No advance reservations are required, but the maximum stay in the Kids’ Place is 2 hours. Sick or Injured Children • Children should not attend the program if they have a fever over 100 degrees, are vomiting, or are contagious in any way. • Parents are required to call in any absences.
• Make up sessions will be considered on a case by case basis at the discretion of the instructor/Program Director. • Injury Policy: If child is injured during programming, Staff will initiate emergency response to handle the situation as trained. o Depending on severity of injury, parent will be called, and made aware of the situation right away. o If parent is not located, emergency contact will be called from Registration Form. If appropriate, parent will be notified upon completion of that day’s session. • Cancellation Policy: If session/class is cancelled due to inclement weather or by fault of Lifetime Fitness, the session/class will be rescheduled at a time designated by the Program Director. Adolescent Policy • Adolescents between the ages of 13 - 15 years can be added to their parents’ membership for an additional fee and must abide by all rules and guidelines that are set by Lifetime Fitness. • Failure to abide by the guidelines may result in termination of the membership. • All adolescents must go through a complete orientation prior to using any of the equipment. • All adolescents must sign a contract stating that they fully understand and will adhere to all rules or guidelines set forth by Lifetime Fitness. • All adolescents under the age of 16 must be on the same level/area as their legal parent or guardian at all times. Please reference our Child Facility Agreement and Children and Youth Manual for complete details regarding all policies and protocols for children under the age of 18.
