General Price List (Effective June 13, 2022) The prices contained herein are effective June 13, 2022 and are subject to change without notice. The goods and services shown below are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. This list does not include prices for certain items you may ask us to buy for you, such as cemetery services, flowers and newspaper notices. The prices for those items will be shown on your bill or the statement of funeral goods and services selected. Professional Services BASIC SERVICES OF FUNERAL DIRECTOR AND STAFF $3136 This charge includes funeral counseling, necessary arrangements, recording vital statistics, securing permits, filing and obtaining death certificates and other forms and claims, preparation of necessary notices, and coordination of service plans with parties involved in the final disposition of the deceased. This fee for our basic services and overhead will be added to the total cost of the funeral arrangements you select (This fee is already included in our charges for direct cremations, immediate burials and forwarding or receiving remains). EMBALMING $890 Except in certain special cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with viewing. If do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it, such as direct cremation or immediate burial. RECEIVING REMAINS FROM ANOTHER FUNERAL HOME $2756 Our charge for this service includes basic services of funeral director and staff, supervising and coordinating details; securing, preparing and filing necessary documents; clerical and record keeping, transfer of remains to the funeral home, and casket coach (hearse) for local transportation to local cemetery or crematory. This charge does not include visitation, rites or ceremonies. DIRECT CREMATION ($4537—$4732) Our charge for direct cremation without any attendant rites or ceremonies includes basic services of funeral director and staff, local transfer of remains to funeral home, care of remains including sanitary bathing & surface disinfection, crematory fee and Coroners Permit to Cremate. If you want to arrange a direct cremation, you can use an alternate container. Alternative containers encase the body and can be made of materials like fiberboard or composition materials (with or without an outside covering). The containers we provide are made of cardboard, fiberboard, or hardwood. We do not have sufficient financial resources to extend credit over a period of months or years, nor does law allow us to fulfill the functions of a bank or other lending institution. Therefore payment is expected at time of arrangements and due prior to services rendered. We encourage your frank discussion of these matters with your funeral director, and we will attempt to assist you in any way possible. PAYMENT OPTIONS Payment With Cash or Check Payment With an Approved Visa or MasterCard. Pre-paid Funeral Arrangements with Anderson Funeral Home, Subject to Verification the Policy is Valid. Financing is Available—For more information please contact our funeral home Assignment of Proceeds of a Verified Life Insurance Policy. Our acceptance of this mode of payment is dependent on the cooperation of the beneficiary and the insurance Company. The insurance policy must be given to the funeral director for verification. We must have the Insurance Company’s complete cooperation in providing information to the beneficiary, amount of the death benefit available, outstanding loans information, cooperation to get claim forms in a timely manner, and their acceptance of our written assignment. The insurance company will send the balance of any insurance proceeds directly to the beneficiary after the insurance company pays the funeral bill in full from the proceeds of the insurance policy. The purchaser must obtain a certified death certificate for each insurance policy. * Due to the complexity and time involved, we are unable to accept payment from estates. 13

Burial Options: Which is Right for You and Your Family? Which is Right for You and Your Family? Deciding on the type of burial to memorialize you is a profoundly personal decision between you and your loved ones. It’s an important way that your memory will be kept alive forever so you’ll need to weigh your burial options carefully. This section is designed to guide you through the five main memorialization and burial options available – including below and burial in a mausoleum, cremation, and above ground burial. There are six types of burial options: In-Ground Burial, Above Ground Burial in a Community Mausoleum, Above Ground Burial in a Lawn Crypt, Cremation, Above Ground Burial in a Private Mausoleum, and Natural Burial In-Ground Burial – The Traditional Cemetery Burial Option: As the most traditional burial choice, in-ground burial typically involves a casket and is marked by a memorial tribute. Below ground burials are usually preceded by a funeral service as well. Below ground burial is the right choice for you if: • You prefer traditional cemetery burials • You want to be memorialized alongside other loved ones • You want outdoor visitations • You want loved ones to place flowers and other decorations at the site • You want a marker or monument to memorialize you A traditional in-ground burial consists of five components: • Casket – displays and contains the deceased, either for burial or cremation. • Gravesite – the space or plot. • Marker– a tribute installed at the gravesite to honor and celebrate a life worth remembering. Flat bronze markers are installed at memorial parks and upright granite monuments are installed at cemeteries. • Vault – a concrete enclosure that serves as the casket's protective lining and provides safe, sure footing for your loved ones. • Opening-and-Closing – preparation of gravesite for burial service that consists of ground opening, closing (back-filling), and landscaping (to restore gravesite's appearance). Can a body be buried without being embalmed? Yes. In fact, embalming is rarely required by law. Some states require embalming if the body is crossing state lines, but for a burial it is not required. Are natural burials legal in the United States? As natural burials become more and more popular, questions continue to arise regarding the legality of natural burials. Natural burials are legal in all 50 states, but there are certain rules and regula- tions that must be followed. These rules can differ based on the state in which the body is being buried, as well as the ceme- tery. Can a body be buried without a casket? Most states require that a body be buried in some type of container such as a casket or a vault. Most states will also require some type of container for in-ground burial. Can a person bury a body on their own land? Depending on your state, burying a body on private land may be an option. However, it will likely be necessary that your state will require some notification prior to the burial, as well as assurance that all state rules and regulations are followed. Is there a shortage of grave space? There is not a shortage of available burial space. Even in areas of the United States that are highly populated, there is available space to bury your loved one if that is your wish. 17
Community Mausoleum : A public mausoleum (sometimes referred to as a community mausoleum) is an above ground building memorializing multiple individuals. Mausoleums provide you with a secure enclosure that will remain clean and completely dry without ever letting the casket or vault come in contact with the earth. Some of the most well known monuments in history are public mausoleums, such as the Tomb of the Unknown Soldier, the Great Pyramids, and Westminster Abbey. The burial site will always remain dry, your loved ones can visit in any weather, public mausoleums are in-door and above ground, and mausoleums offer the prestige of being entombed inside a structure. Memorialization in a community mausoleum crypt involves choosing a space location or “crypt.”, selecting a “ crypt plate,” a flat memorial made of various bronze and granite materials that is installed in front of the crypt and can be inscribed with specific personal information., identifying the location level (most families prefer “eye-level”)., and specifying if you want a single or companion (two-person) crypt. Please note: While a casket and opening-and-closing service are necessary, a vault is not. Private Family Mausoleums: A family mausoleum is a distinct, private, aboveground structure built specifically at an individual’s request, usually to entomb multiple members of a single family. The Taj Mahal is one of many well-known examples of the private mausoleum burial method. Natural Burial: A natural burial does not use embalming fluid, a casket, or a burial vault. Instead, the remains are placed directly into the earth, allowing the body to decompose naturally. In some instances, the bodies of the deceased are buried in a biodegradable casket or in a simple burial shroud, so long as they do not inhibit the decomposition of the body in anyway. Natural burials also do not use any machinery or heavy equipment for digging the grave site. Instead, the gravesites are dug by hand. In terms of different burial products, such as headstones, burial flowers, and memorial benches, there are slight differences compared to more traditional burial options. While headstones are used for natural burials, they are usually a natural stone with a bronze plaque. Depending on your final resting place, there may also be some different rules and regulations for a natural burial. Natural Burial Frequently Asked Questions: Will animals disturb the gravesite? The bodies are buried at a depth deep enough as to not attract animals. Is a natural burial legal in my state? Natural burials are legal in all 50 states. However, some rules and regulations may differ slightly. It’s best to consult with a funeral director for specific information. Green Burial: The terms natural burial and green burial are often used interchangeably. Although they are similar, they are actually two different types of burial option. “Natural burial” strictly refers to the actually burial process. This means the opening and closing of the grave, the preparation of the remains, and the laying of those remains n the burial plot. “Green burial” refers to this process but also to the cemetery in which the burial takes place. A “green cemetery” uses no artificial pesticides and none of the bodies buried in a green cemetery can have been embalmed or buried in a traditional casket. So a natural burial can occur within a traditional cemetery, but a green burial requires a special cemetery. 18
What is the Purpose of a Burial Vault? A Brief History of the Cemetery Burial Vaults: Burial vaults were a solution to an age old problem of the sinking or collapsing of the earth, underneath the weight of equipment, vehicles or even per- sons, due to the loss of the integrity of the casket while buried un- derground. Early attempts at providing some sort of protection from the graves collapsing included lining the grave walls with stone or brick and placing a piece of slate on top. Later, vaults were designed and manufactured specifically for this purpose. You will find that different styles of vaults were used at different periods of time and in different geographical areas. In general however, many U.S. cemeteries began using six piece or sectional vaults somewhere around the early 1900’s. These six piece vaults were initially cut from slate and assembled in the open grave. They consisted of a bottom, four sides and a top; hence the name “six piece”. Some variations had notches cut in the end pieces to keep the sides from pushing in, but the surrounding earth provided the majority of the stability for their design. Later, these slate vaults were replaced with six piece concrete vaults. During the 1920’s and 1930’s, two piece concrete vaults were beginning to emerge and by the late 1930’s, the six piece vault had to settle for almost a niche use. The two piece vault may have required the use of specialized equipment, but the installation time was substantially reduced and could now be completed by one person. I am aware of some sectional vaults still being used today however. To this day, the possibility of the earth collapsing from a burial made years ago without a vault still remains and reminds us of the safety provided by such a product. Are Cemetery Burial Vaults Required? While there is generally no state law that requires the use of ceme- tery burial vaults or cemetery liners, most cemeteries, require the use of a concrete vault or liner for all tradi- tional interments, mainly for the reasons listed above. You will find cemeteries across the U.S. that have no such requirement, and make their use optional. Many cemeteries today may also require the use of a burial vault for the interment of cremated remains, since the same principles hold true. While not as large as a tradi- tional interment, the collapse of a grave used for cremation can have maintenance and safety concerns as well. The fact that there are numerous cremation interments in a relatively small area increases the impact on safety and appearance if vaults were not used. Often times the cremated remains of a loved one will be buried in a shared grave, and may have to be removed for the burial of another family member and reburied with them. The use of a cremation burial vault insures the integrity of the remains during this process. Why is a Cemetery Burial Vault Required? When a burial is made and covered with dirt, there are certain loads placed upon the burial. A burial vault is specifically designed to withstand these loads, where as a casket alone is not. These loads are generally referred to as static loads, dynamic loads and impact loads. A static load is the normal weight load that is placed on the burial vault by the weight of the ground that is above it. With today’s burial practices, the static load on a burial can exceed 4000 pounds or two tons. A dynamic load is a load that varies in intensity. A dynamic load can be caused by cemetery equipment, such as tractors and backhoes, passing over the grave. Today’s larger cemetery equipment can weight in excess of 25,000 pounds. An impact load can be caused by the use of a tamper during the backfilling operation, and can deliver a force that can exceed that of a static or dynamic load. Although this normally occurs for only a brief period of time, this type of load can be concentrated on a very small area. 19
Types of Burial Vaults: Cemetery burial vaults can be made from concrete, plastic and even metals. They can be designed as a box with a lid on top, or as a flat bottom with a dome shaped top that seals at the bottom. Each manufacturer will extoll the virtues of their design, but it has been my experience that most, quality manufactured vaults, will provide adequate protection against the loads that a burial will encounter. Concrete vaults are generally provided in three different categories. The liner is a simple concrete box, which has no seal, and may or may not contain holes in the bottom in order to allow any water that may accumulate, to drain out. Then there is a sealed vault, which is a concrete box that may be painted or coated for aesthetics and to provide some water resistance. The sealed vault will also have a butyl type gasket that provides a water resistant seal between the lid and the box. Finally you have the lined and sealed vault. These concrete vaults contain the same properties as the sealed vault but add an additional inner and sometimes outer liner that is said to offer the greatest amount of protection. These inner and outer liners can be made from plastics, or metals and often time are very ornate adding to the visual appeal. Plastic and metal vaults offer similar level of protection within their product lines as well. It should be noted that the concrete vault industry makes a distinction between Burial Vaults and Burial Liners or Boxes. They only consider “vaults” as those in the lined and sealed category, everything else would be considered a grave liner, grave box or outer burial container. Future Disinterment: Burial vaults not only offer a measure of safety, they may also help to retain the integrity of the casket, as well as keep the earth from settling which assists the cemetery in its maintenance operations and helps to maintain a lasting beauty that would not be possible otherwise. While burial of the body or cremated remains should always be considered permanent, there are occasions when disinterment becomes necessary. The use of a burial vault when the interment was made can help ensure dignity during this process. Cemeteries generally do not require the use of a burial vault simply to sell you something, in fact there are many cemeteries that require the use of a burial vault and do not sell them at all. Burial vaults are required; to protect the integrity of the casket, for the safety of the visitor and the safety of cemetery personnel and equipment. 20
