    At Canine Companions for Independence®, our pups have a very special destiny—to assist a person with a disability in
leading a more fulfilling and independent life. We know that raising a puppy and then turning in the puppy is a difficult
and emotional experience.

    Thank you for all of your time and dedication to raising a puppy.

    We have assembled the Puppy Raiser Matriculation Packet specifically with you, the puppy raiser, in mind. In the
following pages, you will find the necessary information in chronological order to successfully turn in the puppy and enjoy
a pleasant visit to the Canine Companions for Independence Northwest Training Center (also known as the Jean and

Important Dates
    The following is a convenient checklist, calendar of events and to-do list to help with the puppy turn-in experience.

         2/2/2018 Matriculation slideshow photos due; two photos per matriculating dog.

         2/2/2018 RSVP online at www.cci.org/nwrpuppyrsvp.

         2/2/2018 Veterinary records, turn-in questionnaire and final monthly progress report due.

         2/2/2018 Shipping information due to the Canine Companions Veterinary Clinic, (707) 577-1745 (If
         applicable). Shipped puppies must arrive on a flight at San Francisco International Airport on Monday, 2/12/2018
         or Wednesday, 2/14/2018.

         2/2/2018 Begin feeding Eukanuba Adult Large Breed Formula, if not already doing so.

         If shipping the puppy, make an appointment with your veterinarian for a Travel Health Certificate within 10 days of
         flight.

Shipping Guidelines
    If you are unable to attend the graduation ceremony or you have decided that you would like to ship the puppy, please
complete the following steps:

      • Call the airlines and make flight arrangements. The puppy needs to arrive at San Francisco International Airport
         on Monday, 2/12/2018 or Wednesday, 2/14/2018. Our vet clinic must have shipping information no later than
         2/2/2018. Contact them at (707) 577-1745. Please include flight departure time, arrival time, airline carrier and
         flight number. Be sure the consignee is PET EXPRESS for Canine Companions for Independence.
      • The Canine Companions puppy must be in an airline-approved crate that meets their specific requirements.
         Check with your airline for correct crate size, water dish and food requirements.
      • Label crate with your return address and Canine Companions’ address: Canine Companions for Independence,
         2965 Dutton Avenue, Santa Rosa, CA 95407 and phone number: (707) 577-1745.
      • You must have the puppy examined by, and obtain a health certificate (usually a duplicate) from your veterinarian.
         Schedule an appointment within 10 days of shipping, as travel health certificates are only valid for 10 days.
      • On the morning of the puppy’s flight, the puppy should not be given a morning meal and only a limited amount of
         water to drink. The puppy should be toileted just prior to crating when you arrive at the airport.
      • The turn-In paperwork must be sent and received by 2/2/2018 prior to shipping the Canine Companions puppy.
         DO NOT ATTACH THE TURN-IN PAPERS TO THE SHIPPING CRATE.
      • If you have questions regarding shipping arrangements or need help, please call (707) 577-1745.

Dogs in Heat
    If the puppy you are raising is in heat or comes into heat at any time during the week of graduation, please contact the
vet clinic at (707) 577-1745 asap (Monday through Friday, 8:00 am-5:00 pm) so we can get the puppy settled into the
kennels. If she comes into heat on Friday, contact the Puppy Program Department during check-in or directly with the
veterinary clinic.

Unfortunately dogs in heat cannot attend the graduation activities or events, but we will bring the puppy out of the
kennel for a turn-in photo. If you will be attending photos, please be sure to let us know if you would like a photo with the
puppy. We understand that it is a disappointment that the puppy will not be able to attend Friday’s events or graduation,
but we encourage you to participate so that your puppy raising efforts can be honored. You will need to arrive at photos
no later than 10:15 am.

Turn-in Pictures
    Puppy photos are an informal time, where you will have the opportunity to meet other puppy raisers and dress the
puppy for graduation. Puppy photos take place at the Canine Companions Northwest Training Center, which is located
just a few miles south of downtown Santa Rosa, California.

• At the end of the ramp, turn left at the stop sign and continue along the frontage road (past several auto
         dealerships). The road curves to the right and goes over a set of railroad tracks.
      • At the first stop sign (G & C Auto Body Shop is on the corner), turn right onto Dutton Ave.
      • Canine Companions is on the left hand side near the end of the street.
      • Parking is available in the front or rear parking lots and on the street in front of the campus. Photo check-in is
         under the covered overhang outside the training buildings in the center of campus.

Puppy Photo Protocol
    To prepare for the puppy’s photo, the following is an outline regarding what to expect at the photo shoot. Puppy
pictures run from 9:00 to 11:00 am.

    Please do not take the dog out to the play yard before photos as the mud and dirt tracked in gets ground into the
photographer’s backdrop and drop cloth. Additionally, it spoils the photos for all those that come after you in the photo
schedule.

      • Check-in upon arrival.
      • Please bring the puppy’s cape(s), ID tag, collar and Gentle Leader. You
         will be instructed to turn in the cape and Canine Companions ID tag,
         and then remove any additional dog tags, which you may keep.
      • You will be given a matriculation cape, a blue leash and a collar. Please
         dress the puppy for photos. If you are using a Gentle Leader or prong
         collar and prefer to keep this on, please do so.
      • Photos are taken on a first come, first served basis. Be prepared to
         stand in line as you wait your turn.
      • You will be called when the photographer is ready for you, be prepared.
      • Unfortunately dogs in heat cannot attend the graduation activities or
         events, but we will bring the puppy out of the kennel for a turn-in photo.
         If you will be attending photos, please be sure to let us know if you
         would like a photo with the puppy.
      • Photos are exclusively for the matriculating puppies and their puppy
         raising family. No group, breeder or litter mate sittings will be allowed.
         (Exceptions are made for puppies raised in correctional facilities.)

    You must arrive by 10:15 am. in order to check in and get in line for your photo sitting. We cannot guarantee a sitting for
those who arrive after 10:15 am. Thank you!

National Graduation Day
    Puppy raisers across the United States embark on a journey with a Canine Companions puppy that lasts approximately
a year and a half. It begins with a commitment to raise a cute little fuzz ball in hopes of it becoming a Canine Companions
assistance dog and ends with the trip to a Canine Companions training center for turn-in and acknowledgment at the
graduation ceremony. The following information is provided to help make the journey’s end a little easier.

    The graduation ceremony will begin at 1:45 pm and will last approximately two hours. Below is an outline to provide
you with information regarding what to expect at the graduation ceremony.

      •   Please arrive at the graduation site at approximately
          1:30 pm. Sit near an aisle so you can easily make
          your way to the side of the auditorium and be
          prepared to go on stage.
      •   The presenting staff will ask the matriculating class
          to line up alphabetically by puppy (refer to the
          graduation ceremony’s program for order) and
          prepare to come on stage.
      •   As the presenting staff instructs, each puppy
          raiser(s) should proceed to center stage to receive
          their medallion. After receiving the medallion, exit
          the stage by the stairs at the opposite side of the
          stage.
      •   Now that your part is done, sit back and enjoy the
          rest of the ceremony.

Turn-in

Location and Directions to Turn-in
    Puppy turn-in will be held at the Jean and Charles Schulz Campus from 3:45 to 4:45 pm after the graduation ceremony.
It will take place in the South Training Room at the rear of the campus. For directions and map, please refer to the section,
“Driving Directions to Puppy Photos.”

Play Yards
    The large play yard (Gittinger Park) will be open the morning of graduation for playtime with other puppies after
your puppy turn-in photo is taken. Please note, Gittinger Park will not be open in the afternoon following the graduation
ceremony. The play yards behind the training buildings (accessed through the building) will also be closed in the
afternoon.

One on One Time
    The training staff will be providing blankets on the patio and grass (weather dependent) or inside the building for you
to spend some time with your puppy before saying goodbye. There will be training staff available to answer any questions
you may have about the turn-in process or professional training.

Dinner Meal
    Please remember to feed the puppy prior to turn-in. The pups will NOT be fed in the kennels that night (Please bring
your own water and bowl-to avoid spreading communicable diseases). Please do not feed your puppy right before or
after using the play yard as this can be dangerous to the puppy.

Capes and ID Tags
    Canine Companions for Independence ID tags, logo capes, logo vests and other Canine Companions training
equipment must be returned on turn-in day.

Turn-in
    When you have said your good-byes and are ready, you can make your way to
the South Training Room where you will be welcomed by program staff. A staff
member will then take the puppy to meet his or her new roommate and get settled
in. At this time you are welcome to tour our kennels before leaving campus.

Traveling With the Puppy
    Some, but not all airlines will allow a Canine Companions puppy to accompany
you (a passenger) in the cabin. If an airline’s policy does not allow the puppy to fly
in the cabin, it will need to fly as cargo in an airline-approved crate. An additional
fee is generally required and they will require you to purchase a crate if you do
not have one with you. It is important to note that all animals (including Canine
Companions puppies) must have a travel health certificate issued within 10 days of
the flight in order to fly in the cabin or as cargo. We highly recommend that you
review the airlines policies before booking your flight.

    The following are provided as helpful tips when making your travel arrangements.
      •   Make your reservations sooner rather than later.
      •   Speak to an agent about the airlines policies regarding a dog flying in the cabin.
      •   If they do allow, inquire about exit row seating or extended legroom to allow for more space.
      •   If they do not allow, make reservations for the dog to travel as excess baggage and inquire about crate size
          requirements.
      •   Make a veterinary appointment within 10 days of flight and request a health certificate for travel.
      •   On travel day, feed the puppy at least two to three hours before the flight and offer plenty of toileting
          opportunities.
      •   Bring wipes or towels to clean up the dog in the event of an accident inside of the crate.
      •   Bring food in a Ziploc bag so that you can feed the dog if needed when you arrive.
