IMPROVING SURGICAL TRAINING, SCOTTISH PILOT WHAT’S INCLUDED IN THE SIMULATION BUDGET? TRAINING YEAR 2021-22 v4.2, updated 21st July 2021 https://www.scotlanddeanery.nhs.scot/trainee-information/improving-surgical-training- programme-ist/ This document gives details of the simulation strategy provided for Scottish IST pilot trainees, ie all posts on the East and West of Scotland Core Surgical Training programmes, during 2021-22. A lot of dates are still to be fixed, so updates of the appendix will be emailed to you regularly. The strategy is slightly altered from last year, following a review with stakeholders including trainees and now includes a year 2 “Booster Camp”. Funding for the elements of this strategy is provided by Scottish Government through NES, as a pilot, and is in addition to the pre-existing study leave budget. CT1 Induction days in August will be organised by the TPDs and you should already have details: West of Scotland 11th August (F2F), East 12th August 2-4pm (Teams). The diagram below shows an outline of the programme thereafter. 1. MONTHLY TRAINING DAYS (YEARS 1 AND 2) Themed monthly teaching days each year, run by East and West programmes (S Yalamarthi in East and S Gibson in West). You are expected to attend at least 7 sessions each year (morning and afternoon count as separate sessions). Best to spread your topics across the 2 years. You can cross East to West and vice versa, just don’t attend a specific simulation event more than once.
There will be various blends of • pre-course materials posted on our Teams channel • Teams virtual classrooms, and • F2F/simulation components A Teams channel is being created for this purpose. There is no need to book the Teams sessions, and attendance will be recorded electronically. For F2F sessions however, places will be numbered and you need to book on Turas at Improving Surgical Training (IST) Pilot | Turas | Learn (nhs.scot). Booking opens when all details are available to the office and closes 3 days before the event. No charge to the trainee – it comes off the simulation budget directly. And no need to request study leave separately on Turas. The current table of planned monthly training days planned is in the appendix at the end of this document, which will be updated and e-mailed out regularly. Anatomy teaching was paused for COVID due to shortage of cadavers, but may resume. If so it will be for year 1 trainees and any year 2 trainees without MRCS part A. 2. COURSES FOR YEAR 1 a. Scottish Surgical Boot Camp (YEAR 1) – 100% attendance expected A 4-day simulation-rich course at the Highland Clinical Skills Centre Inverness, covering a range of technical and non-technical skills and an induction to the community of practice. Skills covered include bowel and vascular anastomosis, skin flaps, laparoscopy; prioritising and making effective phone calls; situation awareness, decision making, communication and teamwork; common error traps in surgery; leading a ward round in simulation. Writing op notes. Communications skills challenges. How to get the most from your trainer; and starting out in research. Dates: Sat 21st – Tues 24th August 2021 Sat 11th - Tues 14th September 2021 Sat 23rd – Tues 26th October 2021 Book at www.surgicalbootcamps.com or https://www.rcsed.ac.uk/events-courses/course-details- scottish-surgical-boot-camps IST trainees should not pay any course fee; the £750 will instead be charged directly to our Sim budget. There is however a returnable deposit.
Travel expenses can be claimed from the simulation budget, but accommodation is not covered. Travelodge has proved safe and economical in the past. If arranging AirBnB house-share, it must comply with COVID level at the time of the course. b. Critical Care: CCRISP (RCSEng or RCPSG) or RAPID (RCSEd) CCrISP is a 2-day simulation-rich course validated by the RCSEng and delivered in all the Surgical Royal Colleges. The RCSEd RAPID course is also accepted and fundable as a substitute. For the latter we are in the process of negotiating protected places and will let you know. Find a place and book with www.rcsed.ac.uk, www.rcpsg.ac.uk or www.rcseng.ac.uk For CCrISP, IST trainees should pay for the course and claim prospectively through your study leave budget (https://turasdashboard.nes.nhs.scot/ ). The TPD will then charge it to the sim budget, leaving your regular study leave allowance unaffected. Places have been in short supply this last year or two. If you cannot secure a place, don’t worry, we will provide critical care teaching within monthly training days. (No specific courses are required at ST3 selection – you’ll just get points for the number of relevant courses.) c. Basic laparoscopy skills – RCPSG (optional) https://rcpsg.ac.uk/events/basiclapskills Dates:- 27th Oct 2021 29th Nov 2021 Book by e-mailing melanie.sinclair@rcpsg.ac.uk and let her know you are an IST year 1 trainee. Places will be reserved for IST trainees until 10 weeks before, then opened to all comers. You should not pay a fee – it will be charged directly to the IST budget. COURSES FOR YEAR 2 a. IST2 Booster Camp – new for January 2022! Due to popular demand, we are now working on this new feature. Dates coming soon! • Will comprise 2 one-day courses on consecutive days – Managing Surgical Crisis and CoSMoS. • Choose from Dundee, Edinburgh or Glasgow – each bookable through Turas at Improving Surgical Training (IST) Pilot | Turas | Learn (nhs.scot). Managing Surgical Crisis includes classroom non-technical skills training in recognising and responding safely to unexpected or crisis events in the OR, followed by 8 simulation stations with debrief, in which we always see trainees grow in confidence and safety. Directed by Alistair Geraghty, Cons Colorectal Surgeon Forth Valley, and team.
CoSMoS – Consultation Skills that Matter for Surgeons – is a new collaboration between IST and the RCSEd ICONS course faculty. Through video and classroom material, we aim to give you some theory and tools to help with Consultation and Shared Decision Making with patients. After the event and practise in the workplace, we will send out a webcam and consent form so you can record a real consultation and upload it to us for a debrief session with faculty over Teams, which will be done in groups of 3 trainees. A further short Teams session later in the year will consolidate the learning. This is the pilot first module of a potential 4-module series for ST2-5. Book Booster Camp on Turas once it’s confirmed at Improving Surgical Training (IST) Pilot | Turas | Learn (nhs.scot). No charge to trainees. b. Basic Surgery Cadaver Skills (BaSiCS) A 2 day cadaver-based course provided by the RCPSG in the Clinical Anatomy and Skills Centre Glasgow. It puts together component skills into whole operations: o Laparotomy, laparoscopy o Laparoscopic / open appendicectomy o Inguinal hernia repair o Small bowel resection / anastomosis o Approach to the femoral artery Particularly suitable for general, paediatric, urology, and vascular-destined trainees. Dates:- 1. 25th-26th November 2021 2. 2nd course TBA, subject to cadaver availability Book BaSiCS by emailing melanie.sinclair@rcpsg.ac.uk and let her know you are an IST S/CT2 trainee. You should not pay a fee – it will be charged directly to the IST budget. c. Arthroscopy skills course in the Dundee Institute of Healthcare Simulation. A 2-day, simulation-rich course in knee and shoulder arthroscopy, run by Mr Arpit Jariwala and team. Dates TBA. We already have names of orthopaedic-destined trainees as arranged by e-mail a few months ago. If unsure, or anybody we have missed, please e-mail me at kenneth.walker@nhs.scot . d. Alternatives to BaSiCS or Arthroscopy. Non general, paediatric, urology, vascular or orthopaedic-destined year 2 trainees may suggest an alternative simulation-based course if it maps to the Core curriculum. INCENTIVISED LAPAROSCOPY PRACTICE (YEAR 1) Automation of these motor skills can be acquired outwith the OR, freeing up your attention in theatre for higher functions. At boot camp, you will be instructed and issued with an eoSim. Online modules to follow and target metric scores to achieve. Practise alone, in skills clubs or in meetings with Educational Supervisor. Video uploads for each of 6 modules, to be scored by OSATS. Final return of kit in May/June. Core level certificate of satisfactory completion expected at ARCP. No-one receives a non- standard ARCP outcome on the basis of this alone, but it may be taken into account alongside other evidence of progression. Option to include advanced modules, competing for gold, silver and bronze awards. VA SIMULATION (YEAR 2) This year we are offering a second take-home practice programme for year 2 using kits posted out to you, which include instruments and 3D-printed hydrogel blood vessel models, again with a series of online tasks. This will appeal to CT2s or ST2s intending careers in any subspecialty. This will be launched in December. Meanwhile see the existing pilot programme at www.vasimulation.co.uk. LOCAL SKILLS CLUBS (YEARS 1 AND 2) It’s been great to see skills clubs flourishing in about half of our training hospitals now. We now have a group to support this movement, chaired by Vivienne Blackhall. A typical model is regular one-hour skills practice sessions, using eoSims, dry open surgical models and other kit from the 20 skills club boxes we sent out last year. Often trainee-led but consultant- supported. These can perhaps be alternated with journal clubs and CBD clubs. The group will be working on a syllabus of suggested tasks. A range of consumables including suture supplies can be ordered from us at no charge, by e-mailing istsimulation@nes.scot.nhs.uk.
