Choose by May 13
Your needs change year to year, and for 2021/2022, so will
your benefits. Annual Enrollment is your once-a-year opportunity
to choose the best fit for you and your family. Get ready to take
action and submit your elections by May 13, 2021.

Most of your benefits will carry over to the 2021/2022 plan year,
even if you don’t choose to actively enroll. You are only required
to enroll for 2021/2022 benefits if you want to:
4 Make changes to your current coverage(s), such as select a
  different medical plan.
4 Contribute to a Flexible Spending Account (FSA).
4 Add, change or remove your covered dependents.
If you do not enroll or elect to waive your coverage, you will be
enrolled in your current medical, dental and vision plans at your
current coverage level, with no FSA – unless you have
previously waived coverage.

FSA Contribution Limit       You can contribute up to $2,750 to a Health Care FSA or Limited Purpose FSA in 2021 and up to $5,000 to a Dependent Day
                                      Care FSA.
                                      The Health Care FSA comes with a “use it or lose it” rule. You must spend any contributions you make by the end of the plan year,
                                      or you lose your unspent funds.
                                      If you wish to contribute to an FSA, you must enroll by Thursday, May 13. Your current FSA election(s) will not carry forward to
                                      2021/2022.

          Update to Dependent         Beginning July 1, 2021, except with respect to certain grandfathered dependents as described below, coverage under Spirit’s
                                      health care plans has been changed as followed: (i) relatives (e.g., grandchildren, nieces and nephews) will no longer be eligible
            Children Eligibility      dependents, and (ii) coverage for dependent children will end on the last day of the month in which a dependent child reaches
                                      age 26.
                                      Notwithstanding the foregoing, the following dependents will be grandfathered and remain eligible for coverage: (i) relatives who
                                      are covered under a Spirit health care plan as of June 30, 2021 who meet certain conditions outlined in the plan, and (ii) disabled
                                      dependent children who are covered under a Spirit health care plan as of June 30, 2021 (including relatives) who meet certain
                                      conditions outlined in the plan, as long as they are disabled and depend on you for support.

                                      When you or a covered family member receives care through a SolidaritUS Family Health Center, there are no copays,
                                      coinsurance, or deductibles to meet. This includes all types of visits, including preventive care, illnesses, minor injuries and more.
                                      Your SolidaritUS staff will perform on-site lab work at no cost, when needed. You are also able to receive certain prescriptions at
                                      no cost when they are dispensed through the SolidaritUS Family Health Center.
                                      When you need to see a doctor, SolidaritUS should be your first stop, every time – it’s the only way to ensure you’re receiving

        SurgeryPlus Concierge         When you receive a surgery treatment plan, a dedicated Care Advocate is available to help you find a top-quality surgeon,
                                      schedule an appointment and coordinate medical record transfers.
               Service Benefit
                                      This benefit is available to you and your covered dependents enrolled in a Spirit medical plan, and is best coordinated with your

ENROLL FOR BENEFITS: IT’S TIME
To make changes to your benefits, you must take action and enroll online by 11:59 p.m. Central
Time on May 13, 2021. From the home page of myspiritbenefits.com, click the Annual Enrollment
banner for helpful tools and resources and a link to enroll.
Once you have submitted your enrollment elections, print and save a copy of your online
confirmation statement for your records. A paper confirmation statement will also be mailed
to your home after the Annual Enrollment window closes.

WHAT HAPPENS IF I DON’T ENROLL?
