Welcome! FAITH HEALING HUB NETWORK Along your journey in implementing a Faith Healing Hub, you will be joining with other Faith Healing Hubs to establish a network of learning and collaboration. This is a partnership between ReCAST Mecklenburg, Mecklenburg County Public Health, and other faith-based organizations to examine a new method to increase access to care, form partnerships among various community resource centers, and increase community connections in a non-traditional way. Faith based organizations have a longstanding history as a source of community support. The Faith Healing Hub is an opportunity for faith-based organizations to expand partnerships and community resources and to build a recognized model for the critical role of the faith community in crisis intervention for community members. Faith Healing Hubs demonstrate a holistic approach to behavioral health and increases non-traditional entryways into services. Your partnership with ReCAST supports a broader vision of a thriving community-centered culture that is invested in the inclusion, success, Iglesia Cristiana and overall well-being of all citizens. NAVIGATION WHAT IS A NAVIGATOR? How To Be A Successful Healing Hub Navigator: • Support youth and families in connecting to services. • Considers the well-being of the complete person, including physical, psychological, social, and spiritual care. • Be knowledgeable about resources in the community. • Develop relationships with individuals of diverse cultures and backgrounds. SCREENING TOOL BACKGROUND • Develop relationships with community organizations. Social determinants of health (SDOH) are economic, social, and environmental conditions • Ongoing communication with individuals and organizations to monitor access and follow that impact health outcomes. ReCAST Mecklenburg has adapted the NC Department of Health and Human Services’ Social Needs Screening Questionnaire which includes 17-items that identify needs in 7 core domains: • General healthcare assistance • Mental and behavioral healthcare assistance • Food insecurity • Housing instability/Utility help • Transportation • Job Assistance • Interpersonal safety 6 | ReCAST MECKLENBURG ReCAST MECKLENBURG | 7
pre screening tool Gaining client trust and establishing a relationship is an essential step before asking sensitive questions associated with an assessement tool. Below are recommendations for talking points or conversation starters to get better acquainted before diving deep. How To Successfully Start The Screening Process Introduce Yourself (establish credibility and let them know who you are) Introduce yourself and your role Begin steps towards establishing a relationship with conversation starters such as: • Interest in helping youth and families • Church’s commitment to helping the community • Interest in helping the community know about available resources State that you’re happy they reached out for assistance and you will do your best to connect Navigator Oath them to the resources they need. IMPORTANT RESILIENCE TERMS There are some important resilience terms that support the work of ReCAST. The ReCAST Resilience Framework is based on SAMHSA’s Concept of Trauma and Guidance for a Trauma-Informed Approach. Below are key terms of the ReCAST Resilience Framework, such as resilience and the concept of trauma. Trauma Individual trauma results from an event, series of events, or set of circumstances that is experienced by an individual as physically or emotionally harmful or life threatening and that has lasting adverse effects on the individual’s functioning and mental, physical, social, emotional, or spiritual well- Resilience being. This concept can be expanded to define trauma experienced by communities that have A number of factors contribute to how well people adapt to adversities. Resilience is generally experienced civil unrest and are impacted by historical, economic, and race. - SAMHSA. recognized as the process and outcome of successfully adapting to difficult or challenging life experiences, especially through mental, emotional, and behavioral flexibility and adjustment to Trauma-Focused Strategies external and internal demands. ReCAST Mecklenburg (Resiliency in Communities after Stress and Trauma) is a 5-year grant awarded to Mecklenburg County Public Health Department by the federal Substance Abuse and Mental Health Services Administration (SAMHSA). ReCAST promotes resiliency and equity in communities to better deal with stress and trauma with the primary focus on youth violence prevention, racial equity, and equitable access to services.
