CLASS OF 2022 COMPLIANCE INSTRUCTIONS PACKET MANDATORY PRE-MATRICULATION COMPLIANCE REQUIREMENTS All compliance requirements listed below MUST be submitted by the listed due date. Many of the following requirements need to be submitted to CastleBranch. CastleBranch, is a third party web-based company that is contracted by the College of Medicine to assist with compliance management, medical student record tracking and document storage. CONSEQUENCES OF NOT COMPLETING THE COMPLIANCE REQUIREMENTS BY THE DUE DATE INCLUDE: A hold will be placed on your registration. You will not be able to attend orientation, classes or clinical activities. Financial aid disbursements will be delayed. Students who complete the compliance requirements past the listed deadlines WILL NOT BE ALLOWED TO PARTICIPATE IN THE WHITE COAT CEREMONY. DEADLINE: JUNE 18 TO BE COMPLETED IN TASK CASTLEBRANCH FOOD RESTRICTIONS/PREFERENCES Please complete the Google Form if you have any food restrictions and/or preferences (e.g., vegetarian, gluten free, kosher, etc.). NO WHITE COAT CEREMONY AND RECEPTION RSVP Please complete the Google Form. The Class of 2022's family and friends are invited to the White Coat Ceremony and Reception on Friday, August 10, 2018 at 1:45 p.m. in the Carole and Barry Kaye Performing Arts Auditorium on Florida Atlantic University’s Boca Raton campus. NO Please note - Students will receive printed invitations and an e-invitation that they can either hand out or email to their family and friends. Students should only respond once on behalf of their guests using this form. Guests do not need to respond individually. WHITE COAT & NAME BADGE ORDER Please complete the Google Form. NO OPEN YOUR MyFAU STUDENT ACCOUNT If you already have a MyFAU account, no action is necessary. For those new to FAU and who have never activated their FAU account, follow these instructions: 1. Go to the FAUNet ID Lookup Tool at: https://banner.fau.edu/FAUPdad/lwgkznum.P_DisplayID NO 2. Enter your Social Security Number (or your FAU-assigned Z number if you have one) and click "submit".
3. Record your FAUNet ID and Z number (Your Z number is generated by the University and is your student ID number). If you are able to receive this information, please go to Step 5. 4. If you are unable to receive your Z number from the link above, please go directly to: accounts.fau.edu 4a. Select ‘Forgot Your FAUNet ID’ and proceed by entering your first name, last name and D.O.B 4b. Your FAUNet ID will be seen and there will be a link below your FAUNet ID that asks you to proceed and register your account. You will then be able to activate your account. After this is completed, go to Step 6. 5. Please go to accounts.fau.edu and select ‘Activate Your FAUNet ID’ and follow the steps provided. After your account is activated, go to Step 6. 6. Then login to http://myfau.fau.edu Spend some time becoming familiar with the various tabs and features and update your account with your current contact information. If you need help, contact the MyFAU helpdesk at http://www.fau.edu/helpdesk or call 561-297-3999. You can also contact the helpdesk at their toll- free number 866-885-8325. DEADLINE: JUNE 29 TO BE COMPLETED IN TASK CASTLEBRANCH FLORIDA RESIDENCY FORM & DOCUMENTS The Office of Admissions will be contacting you regarding your Florida Residency paperwork. Please contact the Office of Admissions at mdadmissions@health.fau.edu or call 561-297-0440 with any NO questions regarding this requirement. TRANSCRIPTS Arrange for final, official transcripts to be sent to the Office of Admissions from every college and university you have attended in North America. We cannot obtain your final, official transcripts from AMCAS, so you will need to arrange to have a new set of final, official transcripts mailed directly to NO the Office of Admissions at Charles E. Make a copy for your records and YES upload it to your CastleBranch profile. ALL form fields must be filled out for form to be approved.
BACKGROUND CHECK- DRUG TEST- FINGERPRINTING All first year (M1) medical students are required to complete a background check, drug test, and fingerprinting. Students who do not complete these requirements will not be allowed to participate in patient-related, clinical activities, which occur during all 4 years of medical school. We have contracted with a third party, CastleBranch, to perform and oversee the mandatory background check, drug test, and fingerprinting. Instructions on how to access the CastleBranch website and order the background check, drug test, and fingerprinting will be sent to you by the College of Medicine. Instructions on how to complete your background check, fingerprinting and drug testing YES will be populated in your CastleBranch profile after you order the appropriate package. Students are responsible for the cost of background testing. Please review the FAU COM Policy on Student Criminal Background Checks and Drug Screening for further information. MANDATORY PHYSICAL EXAMINATION VERIFICATION FOR FIRST YEAR (M1) FAU MEDICAL STUDENTS 2018-2019 Print this form and bring it with you to your mandatory physical examination. This form must be completed and signed by a Physician (MD, DO), ARNP or Physician Assistant. Please have your medical YES provider include their credentials with their signature. Make a copy for your records and upload it to your CastleBranch profile. ALL form fields must be filled out for the form to be approved. MANDATORY IMMUNIZATIONS FORM FOR FIRST YEAR (M1) FAU MEDICAL STUDENTS 2018- 2019 This form must be filled out and submitted for EACH immunization requirement listed on CastleBranch. If the form is completed in its entirety, you will still download the completed form for every requirement. The form MUST be signed, dated and stamped by a Physician (MD, DO), ARNP or Physician Assistant. IMPORTANT: Depending on your immunization status, you may be required to return to a healthcare practitioner for additional vaccines in a series. The Physician (MD, DO), ARNP or Physician Assistant must re-sign, date and stamp next to the new requirement that was added to YES your form after initial signoff at the bottom of the form. Your medical provider must include their credentials with their signature. *Please note FAU requires all medical students to complete a separate form for FAU Immunization Office. Bring BOTH forms to your appointment. See details below. ADDITIONAL REQUIRED IMMUNIZATION PAPERWORK FOR FAU IMMUNIZATIONS OFFICE In addition to the College of Medicine MANDATORY IMMUNIZATIONS FORM FOR FIRST YEAR (M1) FAU MEDICAL STUDENTS 2018-2019 form on CastleBranch, all new M1 students are also required to submit the following University form (http://www.fau.edu/shs/PDFs/Immunization_Form.pdf) to the FAU Immunization Office. Fill out the top portion with personal information and Parts A, B, and C. Since a physician signature is needed for part C, you should also bring this form with you when you visit your doctor to complete the other listed requirements that need a healthcare practitioner’s signature. This way, your doctor NO will be able to sign off on all three forms. You can either fax (561-297-2769), email (immunization@fau.edu), or drop off this University form to the FAU Immunization Office in person (777 Glades Rd, Bldg. SU 80, Rm 114). We will not be able to register you until the FAU central Immunization Office receives this additional form and clears you for registration.
MANDATORY HEPATITIS B FORM FOR FIRST YEAR (M1) FAU MEDICAL STUDENTS 2018-2019 3 doses of vaccine followed by a QUANTITATIVE Hepatitis B Surface Antibody (titer) preferably drawn 4-8 weeks after 3rd dose. If negative, complete a second Hepatitis B series followed by a repeat titer. If Hepatitis B Surface Antibody is negative after a secondary series, additional testing including Hepatitis B Surface Antigen should be performed. See: ht1p:/Jwww.cdc.gov/mmwr/pdf/rr/rr6210.pdf for more information. YES MANDATORY TUBERCULOSIS SCREENING FORM FOR FIRST YEAR (M1) FAU MEDICAL STUDENTS 2018-2019 Documentation of a two-step PPD skin test within the last 6 months is required. For the TB testing, you will need to have 2 separate PPDs placed and read 1-3 weeks apart if your first PPD is negative. This is called 2-Step TB Testing. Each PPD needs to be read 48-72 hours after placement. Therefore, to complete 2-Step TB Testing, you will need to schedule a total of 4 appointments. Please be sure to schedule them accordingly. If your 1st PPD is positive or you have a history of a positive PPD, you will YES need to follow up with your physician to complete the work up, including a chest x-ray. Print the form and bring it with you to your PPD testing. This form must be completed and signed by a Physician (MD, DO), ARNP or Physician Assistant. Make a copy for your records and upload it to your CastleBranch profile. Your medical provider must include their credentials with their signature. ALL form fields must be filled out for forms to be approved. Make a copy for your records and upload it to your CastleBranch profile. YES ALL form fields must be filled out for form to be approved. OCCUPATIONAL SAFETY AND HEALTH ADMINISTRATION (OSHA) TUTORIALS 1) Click on this link: https://canvas.fau.edu/enroll/9EFMAG 2) You will be taken to the FAU login page where you will need to sign in with your FAU credentials. 3) After logging in, you will be taken to the Enroll in COM - Bloodborne Pathogens and Formaldehyde Training page. Click the “Enroll in Course” button. 4) After successfully enrolling, click the “Go to the Course” button to proceed to the course. Instructions: This training/tutorial is divided into four modules. Please progress through each in order. When you complete each module, you will be taken back to the Menu page, and a checkmark YES will appear to indicate that you have completed the module. You may then proceed to the next module. At various points in each module, you may be provided with Knowledge Check questions to ensure that you have understood the material. You will receive multiple attempts at each question. Once you have completed all four modules, you will be prompted to attempt the Quiz. You will need to successfully complete the quiz and submit the completion form in order receive credit for completing the tutorial. Once you have submitted the form, take a screenshot of the page showing your completion, including the date and time, and upload to CastleBranch.
DISABILITY INSURANCE You are required to have disability insurance while enrolled in school. If you are not covered under your parents’ disability insurance plan, a spouse’s disability insurance plan, or your own disability insurance plan, you may sign up for the FAU-sponsored disability insurance. If you are already covered under another disability insurance plan that is not the FAU-sponsored disability insurance plan, please provide proof of coverage in your CastleBranch profile under this requirement. YES To sign-up for the disability insurance through FAU, please go to the direct link for enrollment below: www.insuranceforstudents.com/faucom Please note: You would need to select 2018 – 2019 School Year Group Long-Term Disability. You will not receive a card for Disability Insurance. Please check the box to verify that you have purchased the Disability Insurance. This plan will expire one year after the date of purchase. In addition, you must submit a copy of your health insurance card to your CastleBranch profile under this requirement. If you are not covered under your parents’ health insurance plan, a spouse’s health insurance plan, or have your own health insurance plan; you may sign up for the FAU-sponsored Health Insurance Plan. This plan will expire one year after the date of purchase. Please Note: your Annual Health Insurance Verification Form for First Year (M1) FAU Medical Students 2018-2019 onto CastleBranch. In addition, proof of purchase or a receipt/email confirmation from the Insurance for Students, Inc. office should be uploaded to your CastleBranch profile under this requirement. PHOTO/VIDEO RELEASE FORM Please download, sign, print, scan, and re-upload your FAU Photo/Video Release Form to this requirement in your CastleBranch profile. ALL form fields must be filled out for the form to be YES approved. MASQUE SURVEY Please download, print, complete, scan, and re-upload your MASQUE Survey to this requirement in your CastleBranch profile. ALL form fields must be filled out for the form to be approved. YES M1 MEDICAL STUDENT SURVEY The M2 Class has created a survey to assist in assigning each M2 to an incoming M1 for an M1-M2 YES Students Connect session during Orientation. Please download, print, complete, scan, and re-upload
your Medical Student Survey to this requirement in your CastleBranch profile. ALL form fields must be filled out for the form to be approved. BIOSKETCH During Orientation Week you will meet your assigned Learning Community Advisor (LCA) along with the 15 students in your group. To help the LCAs get to know you, we ask that you submit a short biosketch – click here to see a sample. This information is not used for assignment purposes or to match student interests within groups. Assignments are done randomly, sorted only by gender. The biosketches will be provided only to your LCA and not to anyone else. Please answer the following questionnaire below and email it to COMOSACompliance@health.fau.edu with the subject "Biosketch". Please also attach a recent close-up professional photo or headshot. NO VARK INVENTORY ONLINE SURVEY – Complete in Survey Monkey NO DEADLINE: JULY 20 TO BE COMPLETED IN TASK CASTLEBRANCH FINANCIAL AID AWARD PACKAGE Financial Aid Offers are made to all eligible students who have completed a Free Application for Federal Student Aid (FAFSA). You must accept the funding you wish to receive no later than Friday, NO July 20, 2018 to ensure it is available to disburse on Tuesday, August 7, 2018. If you accept a Federal Direct Student Loan, you must complete both Loan Entrance Counseling and the Master Promissory
Note at https://www.studentloans.gov before your funds can disburse. This is a one-time requirement while enrolled in the COM. If you will also borrow through the Graduate PLUS Loan Program, you will need to complete Entrance Loan Counseling before the first disbursement of your first Graduate PLUS Loan. This program requires you to complete a new application each year. You also want to verify that your Direct Deposit information has been entered correctly (through your My FAU account) to prevent a delay in receipt of funds for your living expenses.
