At Ancestry, we understand that everyone has different needs, and we are pleased to offer a full range of benefits for 2019, including health, income protection, retirement savings, as well as a variety of complementary non-cash programs. You must take action, using Workday, within 30 days of your hire date if you wish to enroll in benefits. It is your responsibility to complete your benefits enrollment on time. We cannot make any exceptions if you miss the deadline or select the wrong plan, level of coverage, or fail to enroll the correct dependents. WHAT HAPPENS IF I DON’T TAKE ANY ACTION? You will not have access to benefits until January 1, 2020 unless you have a qualifying event, including but not limited to marriage, divorce, birth of child, etc.

Medical Insurance and Spending Accounts Medical coverage offers valuable benefits to help you stay healthy and pay for care if you or your covered family members become sick or injured. In addition to choosing a medical plan, you also have the option to contribute to tax-advantaged accounts that can help you save money. Choosing a Medical Plan for 2019 The medical plans available to you include a range of coverage levels and costs, giving you the flexibility to select the plan that is right for you. Key Words to Know: Copay: An amount you pay for a covered service each time you use that service. It does not apply toward the deductible. Deductible: The amount you pay before the plan begins to pay. Out-of-pocket costs: Expenses you pay yourself, such as deductibles, copays, and uncovered services. Health Savings Account (HSA): An account funded by you and Ancestry that lets you use before tax money to pay for eligible health care expenses. Out-of-Pocket Maximum: The maximum amount you pay for covered services in a year. Plan Coinsurance: Percentage of the charge that your plan will pay. Spending and Saving Accounts The $1,500 and $2,500 High Deductible Health Plans (HDHP) with HSA offer lower payroll deduction costs each month, plus the chance to put away tax free money for medical expenses in an account that remains yours for life. Health Savings Account (HSA) • Available only to participants in the $1,500 and $2,500 deductible plans. • Receive an annual contribution* from Ancestry Ancestry HSA annual contribution is the following: • $1,500 Deductible Plan - $750 individual/ $1,500 family • $2,500 Deductible Plan - $1,250 individual/$2,500 family Ancestry’s HSA contributions are deposited into your account every pay period. Ancestry’s HSA annual contribution amount is pro-rated based on your hire date. Contribute up to the annual IRS limit of $3,500 for individuals or $7,000 for family coverage (This amount includes your employer’s contribution.); the total combined contribution to your HSA cannot exceed the IRS maximums. $1,000 additional contribution allowed for employees age 55+. • Change your contribution amount or stop contributing at any time. • Works like a bank account that you manage to pay for your health care expenses. • Use a debit card to pay for eligible expenses or reimburse yourself for payments you’ve made (only money you’ve already contributed can be spent). • Unused money can be carried over each year and invested for the future — you can even take it with you if you leave your job. • Investment options available once you have $2,000 in your account. • Option to contribute to a Limited Purpose Flexible Spending Account (FSA) for additional tax savings — only dental and vision expenses are allowed until the plan deductible is met, then eligible medical expenses are allowed. 5 Build on your legacy; let your Ancestry benefits help
Spending and Saving Accounts Our Flexible Spending Accounts (FSAs) allow you to set aside money on a pre-tax basis to pay for your out-of-pocket expenses related to your health care, dependent day care, and traveling to and from work. Health Care Flexible Spending Account (FSA) • Contribute up to $2,700 annually to help cover qualified medical, vision, and dental expenses. • Choose your contribution amount once a year (if your personal situation changes, such as getting married or having a baby, you may be able to change your election during the year). • Use a debit card to pay for your eligible expenses. • Your entire annual contribution is available to you from the beginning of the plan year. • Unused money does not carry over at the end of each year and will be forfeited. Limited Purpose Flexible Spending Account (LPFSA) The LPFSA is ONLY for those who also have a Health Savings Account (HDHP plan members). The LPFSA is only allowed to reimburse you for out-of-pocket dental and vision expenses. You can contribute up to $2,700 annually into the LPFSA. Unused money does not carry over at the end of each year and will be forfeited. Dependent Care Flexible Spending Account (FSA) • Contribute up to $5,000 a year to reimburse your qualified dependent care expenses. • Eligible expenses include child care and care for dependent elders. • Unused money does not carry over at the end of each year and will be forfeited. 6 2019 Ancestry Employee Benefits – All Locations
Supplemental Medical Insurance Supplemental medical insurance can help protect you from significant or unexpected out-of-pocket expenses. Keep in mind that these plans are intended to supplement a medical plan. Consider your anticipated medical needs for 2019, along with the cost of the insurance plans available to you. Adding a supplemental plan to a lower cost medical plan may help you save money while providing important coverage. The following three supplemental medical plans are available to you for 2019. Coverage and cost can be found at www.ancestrybenefits.com. Accident Accident insurance supplements your medical plan by providing cash benefits in cases of accidental injuries. You can use this money to help pay for uncovered medical expenses, such as your deductible or coinsurance, or for ongoing living expenses, such as your mortgage or rent. Critical Illness Critical illness insurance helps protect against the financial impact of certain illnesses, such as heart attack, cancer, and more. You receive a lump-sum benefit that you can use however you see fit. Hospital Indemnity A hospital indemnity plan provides supplemental payments that you can use to cover expenses that your medical plan doesn’t cover for hospital stays. 7 Build on your legacy; let your Ancestry benefits help
Health Advocacy and the EAP HealthAdvocate can help you: Ancestry provides you and your eligible family members with confidential access to Personal Health Advocates, that can help you: • Understand your benefit plan provisions and features • Understand tests, treatments and medications • Untangle insurance claims • Find qualified doctors & hospitals Health Advocates are available to you 24-hours a day, seven days a week by calling 1-866-695-8622. Employee Assistance Program Ancestry provides you and your eligible family members with unlimited telephonic access and up to 5 confidential in-person or real-time video counseling sessions for many types of issues, including: • Family and relationship struggles • Drug & alcohol abuse • Financial hardship • Stress or depression Counselors are available 24-hours, seven days a week by calling 1-866-799-2728. Ancestry will not receive or have access to any of your confidential information. Life Insurance Life insurance provides important financial protection for you and your family. The following plans are available to you and more information can be found at www.ancestrybenefits.com. Employer-Paid Life and Accidental Death and Dismemberment (AD&D) Ancestry provides you with a base level of employee life and accidental death and dismemberment (AD&D) insurance at no cost to you. This coverage provides a benefit of three times your salary, rounded to the next highest $1,000, up to $350,000. Employee-Paid Employee Term Life and Accidental Death and Dismemberment (AD&D) To supplement the coverage provided by Ancestry, you can purchase additional term life and AD&D insurance for yourself as well as voluntary life coverage for your spouse and children. Voluntary AD&D is not available for your spouse or your children. This coverage is tied to your employment with Ancestry and typically ends if you leave the company. You must elect voluntary term life insurance for yourself in order to purchase coverage for your spouse and/or children. Disability Insurance Short-Term Disability When you need to miss work for an extended time due to an illness or accident, short-term disability insurance can replace a percentage of your lost income (up to a maximum weekly benefit) for a certain number of weeks. Long-Term Disability If you experience a disabling illness or injury that lasts longer than your short-term disability benefit, long-term disability insurance can replace a percentage of your lost income (up to a maximum monthly benefit). 401(k) Plan Ancestry offers a traditional pre-tax 401(k) or after-tax Roth 401(k) through Empower. Ancestry matches 100% of the first 3% of salary that you contribute with no annual maximum and vests at 25% each year. As a result, your savings grow even more quickly! If you’re a full-time employee, you’re eligible to participate in our 401(k) plans on the first day of employment. If you are a part-time employee, you are generally eligible after one year of service. New employees are automatically enrolled in the 401(k) plan on their eligibility date with a 6% deferral amount. Flexible Vacation Ancestry offers a Flexible Vacation Policy for full-time, salaried employees. With the Flexible Vacation policy, you don’t get a set number of vacation days per year. Instead, you and your manager work together to allow you the time off you need while meeting your work and team obligations. Paid Time Off Hourly Employees Only, Does Not Apply to Salaried Employees Our PTO policy applies to full-time hourly employees. Our PTO policy is designed with flexibility in mind, helping to balance your work and your personal life. PTO can be used for any reason you are out of the office, including vacation, illness, etc. As a full-time employee, you start accruing PTO on your first day of work and increase your PTO days with every year you work at Ancestry. PTO should be arranged in advance except for emergency situations, and all time taken must be recorded in Workday.
