Boca Raton Regional Hospital Executive Summary FY 2020 Boca Raton Regional Hospital at a Glance Born out of compelling community need in 1967, Boca Raton Regional Hospital is a not-for-profit, advanced tertiary medical center with 351 beds and more than 800 primary and specialty care physicians on staff. The hospital is a recognized leader in cardiovascular care, oncology, women’s health, orthopedics, emergency medicine and neuroscience, all of which offer state-of-the-art diagnostic and imaging capabilities. The hospital is accredited by The Joint Commission and is one of only four hospitals in Palm Beach County to be designated by the Florida Agency for Healthcare Administration as a Comprehensive Stroke Center. On July 1, 2019, Boca Raton Regional Hospital joined Baptist Health South Florida. The new partnership ensures that both not-for-profit organizations will continue to meet their shared mission and commitment to elevate healthcare within the communities they serve in an area that reaches across four South Florida counties. Implementation Plan Overview Baptist Health conducted a community health needs assessment (CHNA) in spring 2021 in the Boca Raton Regional Hospital community. This comprehensive report was compiled using various data sources, including local, state and national statistics from the Department of Health, U.S. Census Bureau and the Centers for Disease Control and Prevention (CDC). As a result of this process, Baptist Health has identified the top five health priorities for the Boca Raton Regional Hospital service area: 1. Behavioral Health 2. Chronic Disease Management 3. Access to Care 4. Healthy Lifestyles and Wellness 5. Elder Care and Geriatrics Having identified these priorities, Baptist Health and Boca Raton Regional Hospital will develop an implementation plan (IP) that will describe the actions the hospital will take, identify resources that the facility plans to commit and describe any planned collaborations to address the community’s health needs over the next three years. The IP will be evaluated for impact on an annual basis. The board of Boca Raton Regional Hospital must approve the IP within the same fiscal year as the completion of the CHNA. Behavioral Health Community Benefit: Behavioral Health Services A hospital’s community benefits are programs or activities that provide treatment and/or promote health and healing as a response to identified community health needs. In order to increase access to behavioral health counseling in South Florida, Baptist Health’s community benefit board supports organizations that provide free, low-cost or discounted counseling services to the communities of Monroe, Miami-Dade, Broward and Palm Beach. Baptist Health has partnered with Faulk Center for Counseling to help increase the number of youth and families who receive counseling in Palm Beach County provided by graduate students pursuing master’s and doctoral degrees and clinical volunteers, trained and supervised by licensed psychologists. Behavioral Health Screening Baptist Health Medical Group - North has implemented the Patient Health Questionnaire-2 (PHQ-2) in our primary care practices for all patients as a “first step” approach in screening for depression. Patients who screen positive will be further evaluated with the Patient Health Questionnaire-9 (PHQ-9) to determine if they meet the criteria for a depressive disorder. The PHQ-9 is a multipurpose instrument for screening, diagnosing, monitoring and measuring the severity of depression. Following the PHQ-9, patients are referred to additional behavioral health services, as needed. Baptist Health’s Community Health department organizes free health fairs for the communities of Palm Beach, Broward, Miami-Dade, and Monroe counties. These health fairs offer a variety of resources available to the public and occur either on hospital grounds or in partnership with community organizations. In addition to the biometric screenings conducted at Community Health events, the Patient Health Questionnaire- 4 (PHQ-4) is a four-point Likert-type scale, and will be implemented by the fall of 2021. Its purpose is to allow for a brief and accurate measurement of core signs and symptoms of depression and anxiety by combining the two-item PHQ-2, consisting of core criteria for depression, with the two-item GAD-2 (Generalized Anxiety Disorder), both of which have been shown to be valid and reliable brief screening tools. The total PHQ-4 score complements the subscale scores as an overall measure of symptom burden, as well as functional impairment and disability. An elevated PHQ-4 score is not diagnostic, but is an indicator for further inquiry to establish the presence or absence of a clinical disorder warranting treatment. The Columbia Suicide Severity Rating Scale (C-SSRS) is used to identify and assess individuals at risk for suicide. Providers should use the 10-category scale as a means of measuring suicidal ideation, intent or plan, and past suicidal behavior. Baptist Health has implemented the C-SSRS at all its hospital emergency departments in Miami-Dade and Monroe counties and Boca Raton Regional Hospital in Palm Beach County. This scale is used to guide appropriate therapeutic intervention and to facilitate safety monitoring and planning. 4
Community Education Baptist Health is committed to improving the overall health and wellness of the communities we serve. The Community Health department offers free behavioral health focused education programs, covering topic areas such as stress management, mindfulness, and overall mental health and emotional well-being. These educational programs are offered in person and virtually, through the Zoom platform, and shared across all of our communities in Monroe, Miami-Dade, Broward and Palm Beach County. Similar programs and educational resources are offered in partnership with area community organizations, on Baptist Health social media platforms and online blog posts. Additionally, in partnership with Pastoral Care, Community Health offers programs and services throughout the Baptist Health system. Virtual Care Initiatives: Baptist Health Care On Demand Care On Demand is Baptist Health’s premier virtual health platform. Mental Health Counseling on Care On Demand offers confidential and compassionate support from experts in the field. This mobile platform gives patients the opportunity to speak to a licensed mental health professional through a secure live video chat on their smartphone, computer or tablet. Mental Health Counseling offers behavioral health support to those who are feeling overwhelmed or facing an unexpected crisis. Whether the problem is anxiety, stress, loss, relationships or other issues, our telehealth-trained counselors are a confidential virtual-visit away. Psychiatry services offers board-certified, telehealth-trained psychiatrists who are available by appointment at a time and place that works best for the patient. A psychiatrist can listen, diagnose, manage and recommend treatment options for mild to severe mental health issues. 5
Behavioral Health Partnerships Baptist Health partners with Fort Lauderdale Behavioral Health Center, a fully licensed and accredited private facility, for the transfer of Baker Act patients at Boca Raton Regional Hospital and Bethesda Hospital East and West. The goal of this partnership is to ensure the safe and immediate transfer of patients under a Baker Act from a Baptist Health facility to Fort Lauderdale Behavioral Health Center, or another designated receiving facility, to ensure the appropriate continuation of care for each patient. The Recovery Village at Baptist Health, a partnership with Advanced Recovery Systems, provides a full continuum of care, including medical detox, residential services and outpatient programs. With three locations in South Florida, The Recovery Village at Baptist Health, a drug and alcohol treatment center, is dedicated to meeting the need for high-quality and effective treatment in our community. Behavioral Health Training Behavioral Emergency Response Team (BERT) Boca Raton Regional Hospital is dedicated to addressing the need for treatment of depression, anxiety and other mental health disorders not related to substance use. Toward that end, the hospital launched the Behavioral Emergency Response Team (BERT), an interdepartmental, specialized team with a unique skill set and competencies to provide pre-crisis de-escalation in potentially volatile situations. This team responds to behavioral health emergencies in both the emergency room and inpatient settings. Additional continuing medical education (CME) programs focused on behavioral health are available to physicians. Boca Raton’s Promise Mental Health First Aid Boca Raton Regional Hospital partners with Boca Raton’s Promise – an organization that educates the community on mental health issues and opportunities – to provide mental health first aid (MHFA) training programs to hospital employees and first responders in the service area. MHFA is a groundbreaking national public education program that helps the public identify, understand and respond to signs of mental illness and substance use disorders. This eight-hour course introduces participants to risk factors and warning signs of mental health problems, builds understanding of their impact and overviews common treatments. 6
Support Groups Living with a mental illness can often make individuals feel isolated and hopeless. Support groups bring together members who share common life experiences and offer them a space where they can share their stories, first-hand experiences and coping strategies in a way that helps reduce isolation and loneliness. Benefits often include learning new information and strategies for confronting problems, finding support from others, and the opportunity to help others. Baptist Health offers seven mental health support groups, with two of them – postpartum and bereavement – available at Boca Raton Regional Hospital. 7
Chronic Disease Management Community Benefit - YMCA Baptist Health funds programs and activities offered to the broad community that are designed to improve community health. In Palm Beach County, Baptist Health provided a financial contribution to the YMCA of South Palm Beach in support of the evidence-based Blood Pressure-Monitoring program. This contribution enabled 105 community members to take part in the program – which normally costs $189 – at no cost. Community Health Screenings and Health Fairs Baptist Health’s Community Health department organizes free health fairs for the communities of Miami-Dade, Broward, Monroe and Palm Beach counties. These health fairs focus on informing our community about the resources available to them to meet their needs. Health fairs are offered to the general public and occur either on hospital grounds, or more often, in partnership with our community partners. The biometric health screenings offered include cholesterol, blood glucose, blood pressure, body composition, waist circumference, bone density and mental health. Health fair participants are given their results immediately, along with educational information and possible resources to help with accessing appropriate levels of care. 8
Support Groups Living with a chronic disease can make a person feel isolated. Thanks to support groups, people with a common challenge or health concern can come together regularly to share their stories, first-hand experiences and coping strategies with people who know what they’re going through. For many people, a health-related support group may fill a gap between medical treatment and the need for emotional support. It often provides these individuals an opportunity to share resources, treatments and physician referrals. Support groups are offered on various topics and medical conditions. The aim is continue providing resources and support for our community and increase awareness and group attendance rates. Baptist Health offers more than 25 chronic disease management support groups with 12 of them –stroke, neurological conditions, Parkinson’s disease, heart disease cancer support – available at Boca Raton Regional Hospital. Discounted Mammograms Each year, breast cancer claims the lives of women we love. When it’s detected early, at a localized stage, the survival rate is 98%. In order to make prevention screenings accessible for everyone, Baptist Health offers special pricing for uninsured patients – $50 for a screening mammogram and $100 for a diagnostic mammogram – in May and October. A physician’s prescription is required. Everglades Area Health Education’s Tobacco Free-Florida initiative In collaboration with the Everglades Area Health Education Center (AHEC), the Florida Department of Health’s Tobacco Free Florida program helps reduce the use of tobacco by delivering effective, evidence-based tobacco cessation services for tobacco users and training programs for healthcare professionals. Boca Raton Regional Hospital partners with Everglades AHEC to offer support groups, free tools and service, along with referrals to the program in support of patients who identify as smokers. 9
Access to Care Community Benefit Community Clinic and Organization Support Baptist Health’s community benefit funds are directed toward organizations whose aim is to improve the health of people in the communities we serve. Caridad Center and Promise Fund of Florida are the beneficiaries of such funds in Palm Beach County. Caridad Center uses these funds and in-kind donations to support community clinics that offer medical, vision and dental care to uninsured and underserved children and families. Promise Fund of Florida directs this contribution to provide for a patient navigator who helps low-income women obtain preventive screenings and diagnostic treatment. Financial Assistance Policy Boca Raton Regional Hospital, which is committed to providing care to those in need, supports the care of uninsured and underinsured patients through its Financial Assistance Policy. The hospital offers an 80 percent discount to all uninsured patients having a primary residence in the United States. The Financial Assistance Program provides support to patients who qualify for non-catastrophic and catastrophic assistance. For non-catastrophic approval, patients may qualify for a 100 percent write- off of eligible services if their family household income is below 200 percent of the Federal Poverty Guidelines; or they may qualify for a 50 percent write-off of eligible services if their family household income falls between 201 percent and 300 percent of the Federal Poverty Guidelines. For catastrophic assistance, patients may qualify for a 75 percent write-off for eligible services if their medical bills exceed the family household annual income by at least 25 percent or greater and if they earned between 300 to 600 percent of the Federal Poverty Guidelines. Virtual Care Initiatives: Baptist Health Care On Demand Baptist Health Care On Demand, the telehealth platform and digital front door to access the system, is now available for multiple specialties to treat our patients’ needs. Patients are able to manage their health from anywhere, at any time, and even have prescriptions sent directly to their pharmacy of choice. Services include urgent care visits, pediatric urgent care, breastfeeding support, diet and nutrition, and express symptom management for Miami Cancer Institute patients. Partnership with Florida Atlantic University’s Charles E. Schmidt College of Medicine Residency Clinic Boca Raton Regional Hospital’s Community Outreach department provides screenings, education and assistance with healthcare navigation to community resources. One of these is Florida Atlantic University’s (FAU) Charles E. Schmidt College of Medicine internal medicine residency clinic, where patients identified as uninsured or underinsured are able to access free or affordable healthcare services, such as medical exams, wellness visits, imaging and laboratory tests. Community Outreach regularly conducts biometric health screenings throughout the service area for individuals who qualify for free services at the FAU residency clinic funded by Boca Raton Regional Hospital. 10
Healthy Lifestyles and Wellness Community Health Education With the support of our physicians and other clinical experts, Baptist Health’s Community Health department offers free educational programs focused on a number of chronic diseases, covering such topics as diabetes and heart disease. Lynn Cancer Institute, a part of Baptist Health, also offers a wide array of educational programs for patients and caregivers that include cancer-specific lectures with renowned experts, nutrition classes and wellness programs. In addition, community exercise classes, support groups, smoking cessation programs and pastoral care programs and services are offered in person and virtually through Zoom across all of our communities in Monroe, Miami-Dade, Broward and Palm Beach counties. Similar programs and educational resources are offered in partnership with area community organizations, on Baptist Health social media platforms and in blog posts. Boca Raton Regional Hospital’s Community Outreach department provides an eight-page booklet, The Medication Education: Get a Grip on Scripts, to all inpatient units as a resource to address medication non-adherence. This booklet outlines the best practice of using medications, side effects and provides detailed medication instructions. 11
Elder Care and Geriatrics Community Benefit: Elder Care Funding Baptist Health’s community benefit board supports 211 Palm Beach/Treasure Coast’s Elder Crisis Outreach program, which provides counseling, advocacy and suicide evaluation to seniors age 60 and older. Additionally, the funds Baptist Health contributes are dedicated to seniors in dire need of emergency financial assistance for medicine, rent, utilities or food. This funding allows for 211 Palm Beach/Treasure Coast to meet the needs of Palm Beach County seniors, as well as contribute to sustaining the operational needs of the program. 12
