Summary of Significant Changes •Plan 4 changes in deductibles and max out of pocket expenses PLAN 4 PPO Individual Family Deductible Increasing from $500 to $750 Increasing from $1,000 to $1,500 Max Out of Pocket Medical Increasing from $3,000 to $3,250 Increasing from $6,000 to $6,500 Max Out of Pocket Rx Decreasing from $3,600 to $3,350 Decreasing from $7,200 to $6,700 •Spousal carve out •These two changes reduced our rate increase from 17% to 7% Spousal Coverage •Effective January 1, 2021, spouses who are eligible for medical coverage through their employer are no longer eligible for medical coverage in the Virginia Private Colleges Benefits Consortium (VPCBC) medical plan. •If your spouse meets the requirements to continue coverage you must complete the Spousal Eligibility Affidavit •What are the requirements to continue coverage? Spouse must fall into one of the following categories: • Your spouse is not employed or is retired • Your spouse is employed but does not have access to affordable, minimum essential coverage • Your spouse is employed by Roanoke College or another VPCBC school •Loss of coverage is a ‘qualifying event’: COBRA and Life Event Letter 3

New Benefit Features Anthem Health Guides • Anthem Health Guides is the concierge customer service team at Anthem trained to help guide you through the sometimes confusing health care system. They can help you stay on top of exams, tests, or preventive screenings, compare costs of health care services, find in-network doctors, help you save money on Rx, and much more. • After January 1 when you call customer service you will automatically be connected.
